Palliation in cyanotic congenital heart disease. Fifteen years' experience of various shunt procedures.

During the past 15 years, 143 systemic pulmonary shunt procedures have been performed in 117 patients. These have been evaluated for their clinical effectiveness, the need for a repeat operation and the mortality; particular attention was paid to the Teflon shunt. Variations were found in shunt performance, depending on the primary defect, the type of shunt that was employed and the year of operation. The overall shunt patency after three years was 77% (85% with the Teflon shunt). Although, in our total experience, mortality at 30 days was 12%, with 16% late deaths, "modified Blalock" (Teflon) shunts had only a 5% hospital mortality and a 5% late mortality within three years. Pulmonary atresia, without a ventricular septal defect, is often insufficiently palliated by a shunt alone. Ten of 82 patients with variations of the tetralogy complex died within 30 days of operation, and a further 11 died in the late follow-up period. Six of these 21 shunts were patent at autopsy. Less common defects, such as univentricular heart, transposition and double-outlet right ventricular connections, that are associated with pulmonary stenosis had no early mortality but led to four late deaths among 27 patients. Two of the four patients had patent shunts. Results in the early part of this experience were less than acceptable owing to inferior shunting techniques, postoperative management errors and, particularly, inadequate follow-up surveillance. With correction of these factors we find that the modified Blalock shunt provides very good early and late mortality results, with excellent clinical palliation and patency rates.

[The anti-inflammatory effect of a combination of dimethindene maleate and betamethasone in topical formulations].

The antiinflammatory activity of three preparations (cream, ointment, jelly) of Fenistil Plus (betamethasone + dimethindene maleate) was shown by means of reflection densitometry, a new pharmacodynamic model. The three preparations led to restitution of the Tesafilm-stripped skin within 4 h. There was no significant difference in the efficacy of the preparations. After application of 10 g of the ointment the plasma levels of betamethasone remained within the limits of identification of the analytical method. An influence of the drug on the endogenous plasma hydrocortisone (cortisol) levels could not be found.

Measuring the inhibition of TRH-stimulated prolactin by dihydroergocristine (DEC) in man: an indirect assessment of bioavailability.

An indirect method to assess bioavailability of dihydroergocristine-methansulfonate (DEC) is described. The method uses the inhibitory effect of DEC on TRH stimulated prolactin production. By means of this method it was possible to investigate the relative bioavailability of DEC tablets versus Diertina liquid. The results show that the relative bioavailability is complete.

Absence of a complete carcinogenic effect of phenacetin on the quiescent and proliferating urothelium stimulated by partial cystectomy. A 2-year feeding study in rats.

It was the object of the present animal experiments to test whether phenacetin (PH) exerts a complete solitary carcinogenic effect on the quiescent and rapidly proliferating urothelium of the lower urinary tract of the rat. In order to stimulate proliferative activity resection of one-third of the urinary bladder was performed which is known to induce intensive reparative regeneration in the stump. PH was administered either continuously with the diet or by gavage in three single doses when proliferative activity was highest. After an experimental period of 2 years 74-83% of the animals receiving PH continuously and 49-52% of the rats after limited gavage feeding had developed uni- and bilateral hyperplasia of the epithelium of the renal papilla. Histologically, the papillary hyperplasia exhibited urothelial differentiation and a typical endophytic growth pattern. It was always associated with healed or, rarely, fresh micronecroses of the subjacent papillary tissue. The urothelial hyperplasia of the renal papilla has to be considered not a true preneoplastic, but rather a reactive proliferative lesion in the sense of a reparative hyperregeneration due to toxic necroses. There was no evidence for a complete solitary carcinogenic action of PH on the urothelium of the entire lower urinary tract. It seems most likely that a metabolite of PH realizes tumor growth only as cocarcinogen with initiation-stimulating and/or initiation-promoting effects acting together with other causative factors during multifactorial multistage carcinogenesis. Based upon the experimental findings reported here, the concept of PH as a solitary complete urothelial carcinogen for man should be reassessed.

[Pupillometry: a non-invasive pharmacokinetic and pharmacodynamic method to study the action of trospium chloride (Spasmo-lyt) on smooth muscle].

The basic principle of pupillometry is to measure the changes of pupillar++ diameter versus time. We used a special device: split lam stand, black and white television camera with monitor fixed to a Polaroid. Trospium chloride is a compound with anticholinergic efficacy. The injection of 0.8 mg of trospium chloride followed by oral administrations of 10, 20, and 40 mg showed increasing effects on the mydriatic reaction. Comparing the area under the curves made it possible to estimate the bioavailability of the compound.

[A new model for objective measurement and quantification of the effects of topical anti-inflammatory agents].

A new model is presented with aid of which the antiinflammatory effect of topically administered substances can be measured. The basic measuring device is a reflex densitometer with mobile measuring head that can determine the redness of the tesa-film stripped skin directly without photographical intermediaries. The relevance of the model is demonstrated by the healing curve exhibited by the stripped skin which has been treated with a fluprednylidene acetate containing cream.

Detection of H-2 and Sendai virus antigens by chemiluminescence.

Two monoclonal antibodies reacting with Ia9 and H2.2, respectively, have been added to spleen cell suspensions prepared from mice of different H-2 haplotypes. Cellular light emission was monitored in a liquid scintillation spectrometer operated in the out-of-coincidence mode. The antibodies stimulated chemiluminescence (CL) in cells possessing the target antigenic determinant, but were inactive in cells lacking the determinant. In addition, CL could be stimulated in Sendai virus-infected spleen cells with anti-Sendai antibodies. The results demonstrate that CL measurement is a sensitive and simple method for the detection of cell surface antigens and for the screening of antibodies reacting with these antigens.

[Estimation of age from root dentine transparency (author's transl)].

Measurement of root dentine transparency according to Band und Ramm (1970) is principally suitable for determination of age. Our own investigations of 601 teeth of 50 persons resulted in a regression function of y = 23.8 + 4.5 x +/- 15.3 in a coefficient of correlation amounting to r = +0/67. The best findings were obtained in persons between 30-60 years of age. This technique is particularly useful for the fast determination of age under conditions prevailing during autopsy. Gross errors of the age of teeth is possible. Therefore, all oral criteria are still required to obtain reliable estimations.

[Stimulating effect of partial cystectomy on regeneration of rat bladder urothelium and its significance for carcinogenesis (author's transl)].

Cell proliferation kinetics of the urinary bladder urothelium have been analyzed in a total of 218 female Wistar rats after partial cystectomy. After one-third resection of the bladder DNA synthesis started 15 h postoperatively in the bladder stump and reached its maximum at 25 h with a mean 3H-TdR index of 10.1%. In the area of resection the proliferative activity increased after 20 h and the highest 3H-TdR index was found to be 24.7% after 45 h. In the case of hemicystectomy the labeling index in the stump increased 15 h postoperatively and the highest 3H-T-dR index was determined at 20 h with 16.6%. The urothelial cells in the area of resection began to proliferate synchronously with those in the stump and the maximum of DNA synthesis was measured 35 h postoperatively with an 3H-TdR index of 24.2%. After 2 weeks the proliferative activity within the stump and operative region corresponded to that of the control urothelium. The basal cells showed absolutely the highest proliferative activity, the suprabasal cells exhibited on the other side the highest regenerative potential compared with the control urothelium. Partial cystectomy might possibly serve as an experimental model for testing low potential carcinogens, in the case where carcinogenic effects would be initiated, potentiated and accelerated via stimulation of the DNA synthesis.

Log normal distribution of ovarian and placental steroid values in early human pregnancy.

Concentrations of estradiol, progesterone, 17OH-progesterone were measured by radioimmunoassay in individual serum samples obtained from a group of 304 women with normal gestations between the 4th and 17th week of pregnancy. Values obtained underwent statistical analysis in order to determine the arithmetic mean and the 95% normal range (+/- two standard deviations). It was noted hereby that the spread of values above the mean was greater than that below the mean, and that very often the mean minus two standard deviations was either close to zero or negative. Therefore, the data for each hormone were analyzed for normality of the distributions by the chi-square test. For each hormone the chi-square test was significantly different (p less than 0.001) from the normal distribution (Gaussian). Subsequently, linear transformation by rankit analysis revealed the log-normal distribution of our data. Furthermore, rankit analysis was used as a graphical method to delineate a useful clinical range, which includes the mean and meaningful 95% confidence limits for estradiol, progesterone, and 17OH-progesterone in early normal pregnancies.

[Insulin concentration in polytraumatized patients during infusion of glucose, fructose and sorbitol].

Serum insulin concentration was measured during infusion of glucose, fructose or sorbitol for several days in polytraumatized patients. The patients are divided in two groups, one group with normal glucose tolerance and a second group, where an extreme disturbance of the glucose utilization was found. In patients with normal glucose tolerance the glucose substitutes had the same metabolic effects as in metabolically healthy volunteers. In patients with disturbed glucose tolerance the glucose substitutes (fructose as well as sorbitol) effected an increase in blood glucose concentration and in serum insulin concentration. It is concluded that the increase in blood glucose concentration causes the increase in serum insulin concentration. Obviously, in a certain group of polytraumatized patients a "metabolic insulin resistence" exists. Therefore, glucose utilization is decreased despite an increase in serum insulin. In most cases the metabolic disturbance in these patients is mastered, if glucose substitutes are used instead of glucose as energy source. However, in many cases glucose can be administered only if insulin is given additionally.
