[Internal urethrotomy in the treatment of stenosis of the urethra. Late results and a review of the literature].

The authors report their experience with 37 patients treated by internal urethrotomy for urethral strictures. The results were appreciated on urethral calibration and flow rate after a median follow-up of 33,5 months and a minimal follow-up of one year. The rate of satisfying results was 78,3%. The authors emphasize the need of dilatations associated to low-doses of cortico-steroids in the post-operative period during the first three months, to improve the results.

Rat colonic carcinogenesis after ureterosigmoidostomy: pathogenesis and immunohistological study.

A group of 44 rats underwent the equivalent of a ureterosigmoidostomy (US), while a second group of 18 rats underwent a pediculated graft (PG) of urothelial tissue in the sigmoid wall. Histological lesions were observed in the colon near the bladder colon junction in US rats exclusively. These lesions included dysplasias (5/23), cystic glands (4/23) and 10 neoplasms (9/23), three of which were adenomas, showing elements of juvenile polyp and tubular adenoma in one case. The seven other tumors showed typical histological features of colonic adenocarcinomas, but no frank evidence of parietal tumoral invasion was observed and their cancerous nature was questionable. It is probably a true carcinogenesis since we induced the same histological changes as those in the mucosae adjacent to colonic adenocarcinomas after human US surgery. Moreover, by immunoperoxidase using antibodies against mucus associated antigens (M1 and M3C antigens) we demonstrated that US rat carcinogenesis differs from dimethylhydrazine (DMH) rat carcinogenesis. Furthermore, our results suggest that urine may be an important factor in inducing this type of US carcinogenesis.

Collagen IV staining pattern in bladder carcinomas: relationship to prognosis.

A prospective study of type IV collagen in urothelial tissues was undertaken using an immunoperoxidase method on 125 ethanol fixed specimens. In normal and non cancerous urothelium, the basement membrane was continuously stained and the same pattern was seen in the 27 superficial carcinomas. In the 48 invasive bladder carcinomas, we observed two patterns of staining for collagen IV: in the first one, the staining line was conserved or partially fragmented (28 tumours), while in the second one the staining line was widely fragmented or absent in more than 5% of the tumour area (20 tumours). We found a highly significant statistical correlation between the pattern of staining and short term prognosis. Twenty-nine patients had an assessable follow-up of three years at least. All 16 patients with pattern I staining were alive at two years while only two out of 13 patients with pattern II staining survived two years (P less than 0.0001). At three years, all the patients with pattern II staining died while 11 patients with pattern I were still alive (P less than 0.001). These data provisionally indicate that the type IV collagen staining pattern may be of prognostic value in assessing the short term behaviour of invasive bladder carcinomas. It is thus logical to envisage that the treatment decisions may be influenced by the results of collagen IV staining.

[Incision of the prostate. Apropos of 57 cases].

57 cervico-prostatic incisions have been performed, for prostatic hypertrophy in 56 cases and for dysuria without prostatic hypertrophy in one case. 36 patients underwent a mono-incision and 21 had a bi-incision. The operative technique is simple and the results are satisfactory. After 3 months, the dysuria was improved in 93.5% of cases and the pollakiuria was improved in 92.8% of cases. In 20 patients in whom the quality of ejaculation was defined prior to the operation, this persisted after the operation in 17 cases (85%); it was normal in 8 cases and diminished in 9 cases.

[Retroperitoneal fibrosis. Surgical or medical treatment. Apropos of 15 cases].

15 patients suffering from retroperitoneal fibrosis with ureteral obstruction have been treated. 6 patients were operated at the first onset, 3 after an attempt of corticotherapy and 6 patients had exclusive corticotherapy. Results of surgery referred to kidney units are 9 good results out of 12 and these of corticotherapy 11 good results out of 16. Besides, in the literature, the complications of surgery are frequent and serious. Medical treatment, with renal drainage, when needed is so to consider first, surgical treatment being kept for corticotherapy failure.

[The surgical treatment of incontinence in the male (author's transl)].

The authors undertake a complete review of the various techniques used for the surgical treatment of urinary incontinence in the male. Urethral repair (Young operation) has been abandoned. Creation of a smooth muscle sphincter in bladder gives approximately 32% continence. Use of the anal sphincter to ensure continence of the urethra gives approximately 60% good results. Urethral or bladder neck suspension by aponeurotic strips have given only 42% good results. Perineal muscular plasty procedures resulting a certain compression of the urethra have given approximately 68% good results. Perineal prostheses, despite a certain number of complications (fistulae, infections, urethral erosions) give 37% good results when all the published series are grouped together. Continence prostheses (Scott or Rosen) have given the same percentage of favourable results: 60%. In this review, the authors report the 13 operations which they have performed with insertion of a Kaufmann prosthesis with, in the long term, 6 continent patients out of the 13. Radiological analysis of micturition in these patients with a good result would appear to suggest to the authors that the Kaufmann prosthesis does not act only by simple compression of the urethra but provides a still tonic urethra with a support which enables it to play a role in continence. So much so that the prior study of the efficacy of urethral compression could be used to choose between the insertion of a Kaufmann prosthesis or the immediate recourse to artificial sphincters.

[Rénal oxalic and gallbladder lithiasis after ileal resection for Crohn's disease: a report on one case (author's transl)].

The authors report the association, in a 64-year old man with previous large ileal resection (110 cm) for Crohn's disease, of gallstone and oxalate renal stones. The oxaluria was 60 mg per day (normal, less than 25 mg) and the fecal fat excretion was 50 g per day (normal, less than 6 g). A low--oxalate and--fat diet for 3 months reduced dramatically the steatorrhea, but was totally ineffective for the reduction of hyperoxaluria. The physiopathological mechanisms and the therapeutic consequences of these metabolic complications of ileal resections are discussed.

[Impaired statics of bladder and urethra in some lower motor neurobladders and its treatment by colocystopexia (author's transl)].

In spite of sphincter paralysis, the peripheric moto-neurone lesions induces bladder retention. Secondary retention forces fail to open the bladder neck owing to detrusor flaccidity. Long-dated, abdominal straining increases this static dysfunction by inducing posterior bladder prolapsus upon a flaccid perineum. This disorder est characterized by three components: --posterior prolapsus of the bladder, --horizontal position of the proximal urethra, --deepening of Douglas peritoneum. These patients may get a micturation using a perineal compression; similarly, surgical management consists in a making-up and fastening of the bladder by means of the rectum which is used as a hammack (colocystopexia). The procedure had been used in five cases with good results.
