[Basic studies on chemotherapy of brain tumors by means of liposomes: affinity of sulfatide-inserted liposomes to human glioma cells].

In order to utilize liposomes for the treatment of brain tumors, we examined the interaction between the cells and the liposomes prepared from phosphatidylcholine, cholesterol, and sulfatide (molar ratio, 7:2:1), which were able to deliver the encapsulated materials into the brain through the blood-brain barrier. With a variety of human cell lines, the incorporation of the liposomes and the release of the liposomal contents into cells were studied by spectrofluorometry and flow cytometry by use of encapsulated 6-carboxy-fluorescein. It was found that the amounts of liposomes incorporated into cells were dependent on the dose of liposomes and type of cells. At the same concentration of liposomes, the highest incorporation was found for glioma cells, which was further confirmed by electron microscopy with ferritin-containing liposomes. These results indicate that the liposomes composed of sulfatide, phosphatidylcholine and cholesterol have a high affinity for human glioma cells and should be useful for the chemotherapy of glioma when antitumor drugs are encapsulated into them.

[Two cases of cervical disc disease with intramedullary pathological changes, which are responsible for their neurological syndromes, on delayed CT myelography].

We report two cases of cervical disc disease with myelopathy classified as of motor system syndrome type showing small contrast accumulation within the spinal cord on delayed CT myelography. In our two cases, high density spots on delayed CT myelography were bilaterally localized within the spinal cord, and believed represent pathological changes of the spinal cord, such as collection of microcavities or cystic necrosis. In case 1, the high density areas seemed to be localized in the anterior horn and corticospinal tract, and in case 2, they seemed to be localized in the corticospinal tract. The patient in case 1 produced signs and symptoms resembling motor neurone disease and lesion could not be differentiated from the latter. Delayed CT myelography showed that the cause of the upper limb amyotrophy was attributed to an anterior horn disorder and that of pyramidal tract sign to a corticospinal tract disorder. Therefore, we could differentiate the lesion from motor neurone disease on delayed CT myelography in case 1. In conclusion, we emphasize that delayed CT myelography can demonstrate the intramedullary pathological changes in the cervical disc disease and is useful in distinguishing between cervical disc disease simulating motor neurone disease and the latter.

[A comparative study of clinical signs, computed tomography, and electroencephalography after direct operation of ruptured anterior communicating aneurysms--Part I: Analysis of the cases with mental signs of mild degree].

Eight cases with ruptured anterior communicating aneurysms, who developed mental signs of mild degree lasting more than two weeks following direct operation, were subjected to our study. The following results were obtained. The mental signs of mild degree in 8 cases consisted of memory and emotional disturbances. The main findings of CT scan was low density area in the subcortical and cortical areas of the frontal lobe. There was no close correlation on CT findings between memory and emotional disturbances. Abnormal density areas were detected on CT scan, but EEG showed no localized abnormalities of slow waves. Abnormal density areas were not clearly shown on CT, but EEG showed localized abnormalities. Background activities on EEG were favorable despite lesions detected on CT. Either local or diffuse abnormalities were observed in EEG background activities while no abnormal mental signs were recognized. In cases with no apparent abnormal findings on CT and with favorable background activities on EEG, EEG showed marked build up during hyperventilation. As indicated by the above data, comparative studies of CT and EEG findings revealed differences in sites of abnormalities and degree of abnormalities. This is considered caused by a possible increase in cerebral blood flow and metabolism at initiation of chronic stage following subarachnoid hemorrhage-resulting in the disappearance of abnormalities on EEG.

[A case report of angiosarcoma of the breast].

The patient was a 24-year-old female, and had received 7-40 mg of prednisolone per day from the age of 19 because of SLE. The tumor occupied almost the entire right breast with a blue-red discoloration of the skin. Histological examination of the tumor disclosed angiosarcoma and a standard radical mastectomy was performed. No metastasis was seen in the regional lymph nodes. The patient received adjuvant chemotherapy with actinomycin D and remained well 26 months after the operation.

[An autopsy case of syringomyelia associated with Chiari malformation and basilar impression].

The authors report one autopsy case of syringomyelia associated with Chiari malformation and basilar impression. The pathogenesis of syringomyelia in our case is discussed. This 37-year-old man complained of progressive difficulty in swallowing and walking for two years. He had noticed dysarthria for six months before admission. (Examination) Neurological examination showed dysarthria, down beat nystagmus, disturbance of IXth nerve, Xth nerve and XIth nerve, and cerebellar ataxia. Deep tendon reflexes were hyperactive in the upper and lower extremities. Babinski's sign was positive bilaterally. Neuroradiological examination demonstrated basilar impression and Chiari malformation. (Operation) Suboccipital craniectomy and laminectomy of upper cervical vertebra were performed with dural plasty. Postoperatively he acquired some improvement, but soon after he was worse. He died of respiratory disturbance. (Postmortem examination) Though the central canal was obliterated at the C4 level, the syrinx extended from the C5 to Th7 level. From the C5 to C8, the syrinx was present in the areas of central gray matter, extending into the left dorsal horn, where it communicated with subarachnoid space. Furthermore, the abnormal vessels were noticeable around the syrinx. At the Th2 level, they were also shown in central grey matter where no syrinx existed. (Conclusion) The etiology of syringomyelia in our case was not explained by Gardner's hydrodynamic theory. We suggested that intramedullary abnormal vessels played an important part for the formation of the syringomyelia.

[Diagnosis of a cystic lesion in the spinal cord--studies on delayed CT myelography and MRI].

UNLABELLED: The collection of contrast medium within the spinal cord on delayed CT myelography generally indicates the presence of syringomyelia. We report the cases of cystic myelopathy with intramedullary contrast accumulation on delayed CT myelography. The purpose of this report is to compare MRI with delayed CT myelogram and to discuss the pathogenesis of the intramedullary contrast accumulation on delayed CT myelography. METHODS AND MATERIALS: Thirty patients with intramedullary contrast accumulation on delayed CT myelography were studied with High Resolution Computed Tomography (Siemens Somatom II) and Magnetic Resonance Imaging (MRI) (0.15T imager, Toshiba MRT 15A). MRI were routinely obtained in both axial plane and sagittal plane. Short spin-echo sequences (30 msec TE, 500 msec TR) were used. This series included 6 cases of cervical disc disease, 5 cases of Chiari malformation, 4 cases of lipomeningocele, 4 cases of adhesive arachnoiditis, 2 cases of thoracolumbar spondylosis, 2 cases of trauma, one case of spinal arachnoid cyst, one case of spinal epidural cyst, and 5 cases of idiopathic type. RESULTS: 1) In 17 out of the 30 patients (57%), the region corresponding to the region of contrast medium collection, which was visualized on delayed CT myelography, was seen as an area of the low signal intensity on MRI. In 11 cases out of 17, the syrinx cavity was confirmed at surgery. 2) In 13 cases, delayed CT myelogram showed collection of intramedullary contrast medium, but MRI failed to reveal an area of low signal intensity.(ABSTRACT TRUNCATED AT 250 WORDS)

Revascularization surgery for 28 patients with moyamoya disease--operative methods, outcome and postoperative angiography.

51 times (sides) of reconstructive surgery using 28 patients with moyamoya diseases were evaluated concerning operative methods, outcome and postoperative angiographic findings. Twenty patients (72%) showed excellent results, 7 patients (25%) good and 1 patient (3%) poor. Development of collaterals shown on postoperative angiograms was excellent on 20 sides, good on 9 sides, and fair on 8. Abundant neovascularization was most commonly noted on angiogram in superficial temporal artery-middle cerebral artery bypass with encephalo-myo-synangiosis. Encephalo-myo-arterio-synangiosis, devised by the authors, or encephalo-myo-synangiosis alone also produced preferable development of collaterals shown on angiogram. Superficial temporal artery-middle cerebral artery bypass alone was insufficient for development of neovascularization.

Regional distribution of gastrin-releasing peptide- and somatostatin-like immunoreactivity in the rabbit hypothalamus.

Regional distribution of gastrin-releasing peptide- (GRP) and somatostatin (SRIF)-like immunoreactivity in the discrete nuclei of the hypothalamus was examined in the rabbit according to Palkovits' microdissection method. GRP-like immunoreactivity (LI) was detected abundantly in the hypothalamus as compared with the cerebral cortex when measured by radioimmunoassay using the antiserum recognizing the C-terminal portion of synthetic porcine GRP. On gel-filtration chromatography of the hypothalamic extracts, two major peaks of GRP-LI were eluted; the peak with larger molecular size corresponded to synthetic porcine GRP1-27 and the smaller size to porcine GRP14-27. A concentration of GRP-LI was highest in the infundibular nuclei (IFN) as well as the ventromedial nuclei (VMN), and next high in the paraventricular nuclei (PVN), suprachiasmatic nuclei (SCN) and periventricular nuclei (PEV). The content of GRP-LI in the median eminence was not so much when compared with them. On the other hand, SRIF was localized in the highest concentration in the ME, followed by the VMN and IFN, as well as the PEV. The findings indicate that porcine GRP-LI exists in the hypothalamus of rabbits with characteristic regional distribution. Concurrent localization of GRP-LI and SRIF in some parts of the hypothalamus may suggest the interaction of both peptides in these areas under various physiological and pathological status.

Establishment of a human B cell line that proliferates in response to B cell growth factor.

A human B cell line which shows a marked dose dependence on B cell growth factor (BCGF) when cultured in less than or equal to 2% serum has been established. Human B lymphocytes were obtained from peripheral blood of normal donors and cultured in the presence of anti-IgM (mu chain specific) and BCGF. Frequent refeedings with fresh medium containing BCGF and anti-IgM led to the establishment of a long term cultured human B cell line, HAB-40. Phenotyping of HAB-40 revealed that the cell population consisted predominantly of IgM-bearing (72%) and B1 (100%) positive cells. This B cell line consistently secreted IgM and IgG when co-cultured in the presence of PMA, anti-IgM and beta or gamma interferon (IFN). Also, it was Epstein-Barr virus nuclear antigen (EBNA) positive (100%). HAB-40 cells have been successfully maintained in the presence of BCGF without anti-IgM for over a year. Removal of BCGF led to the rapid loss of viable cells in cultures containing less than 2% serum. HAB-40 cells in microassays exhibited a marked dose-dependent incorporation of [3H]thymidine in response to BCGF in the absence of any exogenous stimulants such as anti-IgM or Staphylococcus aureus Cowan I (SAC). Recombinant interleukin 2 (IL-2) failed to augment the [3H]thymidine uptake by these B cells despite the low density expression of Tac antigen (IL-2 receptor) on their cell surface, or even when the cells were stimulated with phorbol myristate acetate (PMA) to express higher density of Tac antigen (48%). HAB-40 cells could be maintained in BCGF which was partially purified to deplete it of other contaminating proteins. None of the seven well established EBNA-positive human B cell lines nor two chronic B lymphocytic leukemia (B-CLL) cell lines that were tested showed BCGF dependence. The same BCGF-active chromatographic fractions that were active on HAB-40 cells also stimulated BCL1 and normal human B cells stimulated with anti-IgM. In the presence of less than or equal to 2% serum proteins this cell line provides a simple, reproducible assay for BCGF even in the presence of contaminant IL-2.

Effects of saikosaponin-d on aminonucleoside nephrosis in rats.

The effects of saikosaponin-d extracted from the roots of Bupleurum falcatum L. on aminonucleoside nephrosis were studied in rats. Urine protein excretion in rats receiving aminonucleoside alone was significantly elevated on the 2nd day after the last injection of aminonucleoside and reached a peak on the 11th day. Urinary protein on the 11th day was reduced by 48 and 46%, respectively in animals treated with saikosaponin-d from the 2nd and 8th day after the last injection of aminonucleoside. Electron microscopically, the degree of abnormality, e.g. loss or fusion of foot processes, in the glomerular epitherial cells was significantly lower in the rats treated with saikosaponin-d after aminonucleoside injection than in the rats treated with aminonucleoside alone. It is concluded that saikosaponin-d prevents the development of proteinuria induced by aminonucleoside in the rat.
