A case of an aneurysm on the persistent primitive trigeminal artery (PPTA) is reported. A seventy-five-year-old woman with subarachnoid hemorrhage was admitted to the Hospital six hours after the onset. She was lethargic and had stiffness of her neck with severe headache and vomiting. Computed tomographic examination showed marked subarachnoid hemorrhage in the basal cisterns, especially in the ambient cistern on the right side. Cerebral angiogram revealed the right PPTA having a saccular aneurysm on the trunk. Her advanced age and the special location of the aneurysm did not allow direct operation in the acute stage. She died of rebleeding of the aneurysm thirty days after admission. Pathological study showed that the PPTA was originated from the cavernous portion of the right internal carotid artery and joined to the cephalad portion of the basilar artery. The aneurysm, 10 X 7 mm in size, was located at the curved midportion of PPTA, 9 mm proximal to the basilar artery. The proximal portion of the PPTA to the aneurysm had severe arteriosclerosis, whereas the distal portion showed less sclerotic change. However, there were no evidences of developmental anomaly in the wall of the PPTA. Eighteen cases of PPTA with the aneurysm arisen from PPTA itself or at its junction with internal carotid artery have been previously reported. PPTA in any case has not been examined pathologically. It has been speculated that dysplasia of the PPTA wall contributes to initiating the aneurysm on the PPTA. However our histopathological examination of the PPTA revealed no evidence of dysplasia in the PPTA wall.(ABSTRACT TRUNCATED AT 250 WORDS)
Preoperative intra-arterial infusion of adriamycin [I I A] for advanced breast cancer was performed in 22 cases, and correlation of the estrogen receptor (ER) status with the effect of I I A therapy was histologically examined. Fifty-five percent of all cases responded to I I A; 80% of ER-negative cases responded, whereas only 33% of ER-positive cases did. I I A was significantly effective in ER-negative cases (p less than 0.05). The histological effect of I I A was also remarkable in ER-negative cases and was related to the clinical effect. The ER status was thought to be a parameter not only for hormonal therapy, but also for chemotherapy. Additionally, drill biopsy was performed in order to examine the correlation between the therapeutic effect and histological type in 16 cases before I I A therapy. Scirrhous carcinoma was more responsive to the I I A therapy than other histological types.

There is now a controversy about the methods of surgical treatment in syringomyelia associated with Chiari malformation. In this report, the authors emphasize that syringosubarachnoid shunt is effective as a surgical procedure or syringomyelia associated with Chiari malformation. From 1982 to 1986, 17 patients with syringomyelia associated with Chiari malformation were surgically treated. Seventeen patients underwent 22 operative procedures for syringomyelia. Syringosubarachnoid shunt was used in eighteen patients, foramen magnum decompression with syringosubarachnoid shunt in one, foramen magnum decompression with terminal syringostomy in one, terminal syringostomy in one, and ventriculoperitoneal shunt in one. The average postoperative follow up period was 2 years and 3 months, from 4 months to 4 years and 11 months. Thirteen out of 17 patients showed neurological improvement and 3 patients were neurologically unchanged. In one patient, the symptoms deteriorated. Syringosubarachnoid shunt is an effective therapeutic procedure for the patient with syringomyelia associated with Chiari malformation. We think that shunt malfunction by arachnoiditis after operation could be prevented by inserting the shunt tube into the subarachnoid space and closing the arachnoid membrane.
We have shown that the ginsenosides Rh1 and Rh2, which are plant glycosides with a dammarane skeleton resembling a steroid skeleton as an aglycone, control the phenotypic expression of mouse B16 melanoma cells in different ways. The effects of Rh1 and Rh2 on the cell surface were studied to clarify the relationship between the control of phenotypic expression and modification of the cell surface in B16 melanoma cells. Rh2, which has the capacity to inhibit the growth of and to stimulate melanogenesis in B16 melanoma cells, causes flattening of the cells cultured in a collagen gel, leading to organized, nonoverlapping monolayers. Cell-to-cell adhesiveness and cell-to-substrate adhesiveness were markedly increased in the B16 melanoma cells treated with Rh2. In Rh2-treated cells, the binding of peanut agglutinin on the cell surface was also increased, whereas no marked changes were observed in the binding of concanavalin A or wheat germ agglutinin. In contrast, Rh1, which showed no effect on cell growth, but did stimulate melanogenesis, did not cause morphological changes of the cells and exerted no effect on cell adhesiveness or cell surface lectin binding. 1,6-Diphenyl-1,3,5-hexatriene polarization values markedly decreased in cells treated with either Rh1 or Rh2. Rh2 was found to be incorporated in the lipid fraction of the B16 melanoma cell membrane. In contrast, Rh1 was not detected in the lipid fraction of B16 melanoma cells. However, novel lipid components were found.
Contrast sensitivity and pattern visual evoked potential (VEP) were measured in cases of ocular hypertension and primary open-angle glaucoma at various stages. The visual field of each eye was examined quantitatively and the retinal nerve fiber layer and optic disc were precisely assessed with magnified stereoscope fundus photography. This study revealed that contrast sensitivity of the eyes with glaucoma was within the normal range in the very early stage of the disease. As optic nerve damage advanced, high- or low-frequency loss developed. Further optic nerve damage produced a level type of loss. Pattern VEPs also showed increasing abnormalities as glaucomatous optic nerve damage progressed. Measurements of contrast sensitivity and pattern VEP were found not to be as sensitive as quantified precise visual field measurement or color stereoscopic fundus photography for detection of minor optic nerve damage in cases of early glaucoma. These methods may be useful, however, as an objective and subjective monitor of progression of optic nerve damage in glaucoma.
The clinical features of 31 cases of syringomyelic syndrome, confirmed by metrizamide CT myelography (CTM) or magnetic resonance imaging (MRI), were analysed. Bilateral suspended sensory disturbance, previously regarded as a characteristic feature, was encountered in only 2 cases (6.5%). Thirteen patients (41.9%) showed no motor deficits of the upper limbs and 7 patients (22.6%) did not have weakness or atrophy in any of the four extremities. Cervical segmental signs were absent in 7 cases (22.6%) and in 2 (6.5%) signs were only present in the lower limbs. Scoliosis and foot deformity, such as pes cavus or equinovarus, were seen in 17 (54.8%) and 5 cases (16.1%) respectively. These skeletal abnormalities were initial symptoms in some childhood cases. In syringomyelic syndrome, clinical features are variable with many atypical clinical manifestations, apart from the previously recognized characteristic clinical features.
The cardiac muscle of BB Wistar rats suffering from diabetes for 8 and 16 weeks (8-Wk and 16-Wk of DM) were examined by light and electron microscopy. The diabetic rats were kept alive by injections of small doses of insulin and exhibited severe hyperglycaemia, glycosuria and weight loss. The heart/body weight ratio of all diabetic groups was greater than that of age matched controls. Over the experimental period, the left ventricular myocardium of the diabetic BB rats sustained damage that was progressively more serious with the duration of the diabetic state. In BB rats after 8-wk of diabetes the myocardium contained large numbers of lipid droplets and glycogen granules around mitochondria which showed patchy swelling, and slight loss of myofilaments. Disruption of mitochondrial membranes and extensive loss of myofilaments were seen in rats diabetic for 16 wk. In addition, dilatation of the sarcoplasmic reticulum-transverse tubular system, formation of a contraction band and myelin bodies and widening of the intercellular space at the fasciae adherens of the intercalated disc were characteristically observed in BB rats after 16-wk of diabetes. However, there were no evident alterations in the capillaries of any diabetic BB rats. Morphometric analyses showed the volume percentage of myofibrils in diabetic rats to be significantly decreased when compared with controls. The loss of myofibrillar elements may be a primary damage induced by insulin deficiency. The formation of contraction bands suggests Ca2+ overload caused by diabetic metabolic disturbances.
In the present study, we purified a neutral thiol proteinase from dog PMN leukocytes and indicated that the proteinase elaborated the chemotactic factor for lymphocytes by cleavage of IgG. The neutral thiol proteinase was purified about 744-fold by ion-exchange chromatographies and affinity chromatography, and the final preparation was over 70% pure. After incubation of dog IgG with the proteinase, three distinct protein peaks were seen by the gel filtration on Sephadex G-200. Only the third peak, perhaps a dialyzable peptide, showed a significant chemotactic activity for dog lymphocytes.
