[Induction of phenotypic reverse transformation by plant glycosides in cultured cancer cells].

Crude ginsenosides extracted from the root of Panax ginseng C.A. Meyer inhibited the growth and colony forming ability of Morris hepatoma cells in soft agar suspension culture, and stimulate the serum protein synthesis of these cells, thus converting the cell characteristics both functionally and morphologically to those resembling original normal liver cells. We have called such a phenomenon "reverse transformation" or "redifferentiation" which can be regarded as decarcinogenesis. In this report, the results of our recent investigations are presented with particular reference to reverse transformation of B16 melanoma cells induced by ginsenoside Rh2 isolated from the methanol extract of crude ginseng saponin fraction and action mechanisms of ginsenoside Rh2 are also discussed.

[Infected solitary renal cyst extending to subphrenic abscess--a case study and review of the literature].

A 54-year-old female, who had been mediated for simple type schizophrenia for the past eleven years, was hospitalized with left lumbar discomfort or pain. After admission, the following abnormalities were apparent serologically; leucocytosis of 15700/mm, erythrocyte sedimentation rate of 147 mm/h, hyper alpha 2-globulinemia (675 ng/dl of haptoglobulin). Urine cultures were negative. Left renal simple cyst with infection was evaluated by intravenous urography, ultrasonogram, computed tomographic (CT) scan., and renal angiogram. In addition, left subphrenic abscess behind the spleen, which had to be penetrated through retroperitoneum, could be diagnosed. So, in our case, cystic wall resection (unroofing) and/or conservatively percutaneous cystotomy for pus drainage were all not indicated instead of radical operation for abscess removal completely. CT scan is mostly available to make differential diagnosis whether the infectious renal cyst influence to the neighbouring organs beyond the renal capsule, or not. Here, we discussed the pathogenesis on the infection of solitary renal cyst, and some problems reported in the literature.

Gastrointestinal helminth fauna of Japanese moles, Mogera spp.

One trematode, Echinostoma macrorchis, 2 cestodes, Hymenolepis diminuta and Choanotaenia spasskii and 5 nematodes, Parastrongyloides winchesi, Tricholinstowia talpae, Heterakis spumosa, Trichuris sp. and Capillaria soricis were obtained from 129 Mogera wogura, 233 M. kobeae and 28 M. tokudae, which were collected from almost all over Japan. All present cases were new host and locality records. The discovery of T. talpae in Japan shows the wide distribution of this species from Japan to Europe.

[The results of bypass with the BioPump in the surgery of traumatic rupture of the thoracic aorta without heparin].

In surgery of the traumatic rupture of descending thoracic aorta, external shunt without systemic heparinization is commonly employed to avoid the bleeding of other injured organs as well as the ischemic injury of spinal cord. However, it provides no means of controlling the flow. We employed the BioPump without heparinization in 2 cases of traumatic rupture of descending thoracic aorta and additional 2 cases of aneurysm of thoracic aorta. Significant platelet loss occurred immediately after operation, however, there was no postoperative evidence of the organ failures due to microembolization. Heparinless bypass with the BioPump is considered to be safe and simple as an adjunct means in surgery of the traumatic rupture of thoracic aorta.

[243 cases of percutaneous nephrolithotomy--result of 5 years].

Between May, 1982 and April, 1987, upper urolithiasis of 247 kidneys of 243 patients, 174 male and 69 female, were treated by percutaneous nephrolithotomy (PNL). The main stone was successfully removed from 236 kidneys (about 95.5%). Seven cases of ureteral stone could not be removed by PNL, and were treated by traditional ureterolithotomy. In 3 cases of pelvic stone, the stone descended to the ureter during the PNL procedure, and was also removed by ureterolithotomy. In one case of pelvic stone, complete obstruction of uretero-pelvic junction after PNL occurred, and pyeloplasty was needed. One pelvic stone patient who had basical hypertension and arrhythmia died soon after PNL, because of acute myocardial infarction. Blood transfusion was needed in 31 cases (12.8%). The residual stones over 5 mm were noticed in 48 cases (20.3%). The most important point to succeed in PNL is to make a nephrostomy through the most suitable calyx to the stone. The easiest way to identify the location and connection of the aimed calyx with the stone in three dimensional position, is to take X-ray films in intravenous pyelograms not only from postero-anterior but also oblique positions. Additional transureteral lithotripsy (TUL) for the fragment descending to the ureter after PNL produces a much better result.

[Estimation of the intrarenal hemodynamics in patients with primary aldosteronism].

Intrarenal hemodynamics were estimated in patients with primary aldosteronism (PA) using the Gomez's equations and by analyzing their renal function curve. The study was performed in 6 patients with PA for 2 weeks; they were given a regular sodium diet (12-15 g/day) in the 1st week and a sodium restricted diet (1-3 g/day) in the 2nd week. Blood pressure and urinary sodium excretion (UNaV) were measured on the last three days of each stage. Glomerular filtration rate (GFR) and renal plasma flow rate (RPF) were also measured on the regular sodium diet. Afferent (RA) and efferent (RE) arteriole resistances, and glomerular hydrostatic pressure (PG) were calculated using Gomez's equations. UNaV was plotted on the ordinate as a function of mean blood pressure (MAP) on the x-axis. Assuming that the difference between MAP and the x-intercept of this renal function curve represents the effective filtration pressure, RA, PG and gross filtration coefficient of capillaries (KFG) were also calculated. GFR and RPF were 102 +/- 6, 469 +/- 27 ml/min, respectively. Estimated RA (6600 +/- 700 dyns.sec.cm-5) was markedly elevated, while RE (2500 +/- 100 dyns.sec.cm-5), PG (57 +/- 2 mmHg) and KFG (0.195 +/- 0.041 [ml/sec]/mmHg) remained normal. The intrarenal hemodynamic parameters obtained by analyzing the renal function curve were consistent with those by Gomez's equations. Recently, in DOCA-salt hypertension, an animal model analogous to human PA, PG has been reported to be increased, leading to renal damage. However, the increase in RA was as marked as seen in essential hypertension, and further the increase in PG was not observed in the patients of this study. Thus, it is not clear whether there is a meaningful relationship between glomerular hypertension and renal damage in PA as reported in DOCA-salt hypertension.

[Effect of myocardial preservation and preoperative left ventricular end-diastolic pressure upon the early postsurgical cardiac function after prosthetic cardiac valve replacement].

We investigated the effect of myocardial preservation and preoperative stroke work index (SWI) and left ventricular end-diastolic pressure (LVEDP) upon the early postsurgical kinetics of left ventricular function in thirty-four patients after prosthetic cardiac valve replacement. They received both-sided cardiac catheterization and measurement of cardiac output by dye dilution method in a standard fashion. Myocardial temperature was measured simultaneously and consecutively during aortic cross-clamping period (greater than 70 min) on open heart surgery. Thereafter, each postsurgical left ventricular function was estimated at three, six, twenty-four and forty-eight hours after weaning from cadiopulmonary bypass (CPB) by using Swan-Ganz catheter. SWI0 X 1000/AnI was used as an improved index of postsurgical cardiac function, because it was incorporated with the preoperative left ventricular stroke work index (SWI0) and anoxic index (AnI), which was the integral of myocardial temperature during aortic cross-clamping period. Otherwise, we regarded LVEDP as an estimate of preoperative cardiac function. So we compared them with the early postsurgical left ventricular SWI, respectively. As a result, the relation between SWI0 and the postsurgical SWI (SWI24 and SWI48) showed positive correlations with correlation coefficients 0.52 and 0.70, and the relation between AnI and the early postsurgical SWI (SWI3 and SWI6) showed negative correlations with correlation coefficients -0.56 and -0.52. While the relation between SWI0 X 1000/AnI and postsurgical SWI (from SWI0 to SWI48) showed positive correlations with correlation coefficients decreasing from 0.76 to 0.57. LVEDP showed negative correlations to SWI0 and SWI48 with correlation coefficients -0.48 and -0.45.(ABSTRACT TRUNCATED AT 250 WORDS)

[Increased lipid peroxides generation and calcium fluxes in hearts during reperfusion].

To investigate the evidence for free radical generation and calcium influx as reperfusion injury, we studied the time course of lipid peroxides generation and calcium content in hearts during reperfusion. The study was performed in patients who had valve or coronary disease, and underwent open heart surgery. Lipid peroxides activity in plasma during reperfusion showed a significant (p less than 0.01) rise to a peak on 5 minutes of reperfusion (1.45 +/- 0.07 nmol/ml; mean +/- sem) compared to the level before reperfusion (1.37 +/- 0.07 nmol/ml) (n = 20). The lipid peroxides generation well correlated with aortic cross-clamp time (r = 0.6384, p less than 0.01). On the other hand, the atrial calcium content after 5-10 minutes of reperfusion was 118.5 +/- 14.9% of the level before cardiopulmonary bypass (n = 15), calcium/magnesium was 124.5 +/- 8.9% (p less than 0.05). That increase ratio well correlated with aortic crossclamp time (r = 0.5948, p less than 0.05). These results have produced evidence of increased oxidant activity and calcium influx in hearts during reperfusion. These results suggested that abnormal lipid peroxidation happened during reperfusion, and these peroxidation of the cell membrane might cause the membrane damage and then calcium influx.

[Radiotherapy of esophageal cancer: the local effect and prognosis].

The correlation between the local effect and the prognosis of 124 esophageal carcinomas treated curatively with irradiation has been examined. The local effect at 40 Gy was found to correlate well with the prognosis. A high local control rate can be expected in cases of a superficial type, a tumorous type and in some cases of an ulcerative type, even though no change was seen in Ba study at 40 Gy. In well advanced ulcerative cases, in infiltrative cases, and in cases that show no change at 40 Gy, the radiation method should be altered positively after 40 Gy to achieve better local control because metastases are relatively few.

[Radiation therapy of endometrial carcinoma].

From January 1976 to May 1988, 67 patients with an endometrial carcinoma were treated at the Department of Radiology of Tohoku University. Of these, 35 patients, including 19 state III cases, were treated with irradiation alone. The remaining 32 patients, including 14 stage I cases, where treated with irradiation after surgery. The five-year survival rate of all cases, which include cases treated with irradiation alone and cases treated with postoperative irradiation, were 44.5%, 40.8%, and 45.0%, respectively. The local control rate in the irradiation cases alone was 57.1%. It seems that elaborate planning of intracavitary irradiation is essential for improved the prognosis of an unresectable endometria carcinoma.

[A new trial assessment of left ventricular function before and after aorto coronary bypass surgery--application of ejection vector (E.V.)].

In 34 patients underwent aorto coronary bypass, left ventriculography was analyzed and ejection indexes (E.F. 100, E.F. 50, E.V. = 2 X E.F. 50/E.F. 100) were calculated. There was not difference in ejection indexes of patients between who had been complicated with preoperative myocardial infarction and not. E.F. 100 was not changed postoperatively, otherwise, E.F. 50 and E.V. showed significant increase. In 15 patients whose quality of life remarkably was improved postoperative period, E.F. 100 was not changed but E.F. 50 and E.V. were significantly increased. Four patients could not perform exercise test to full dose because of intractable ischemic calf pain. In twelve patients, whose left ventricular function was improved at postoperative exercise test, their E.F. 100 was found to be unchanged, but in contrast, E.F. 50 and E.V. were increased significantly. In conclusion, we found that E.V. could become to be more sensitive indicator of left ventricular function compared with E.F. 100 before and after aorto coronary bypass surgery.

[Studies on evaluation of the oxygen transport system function with multistage treadmill stress testing: comparison between normal control subjects and patients with coronary heart disease].

The oxygen transport system (OTS) function was evaluated with multistage treadmill stress testing on 171 normal control subjects and 80 patients with coronary heart disease (CHD). After Bruce's definition, OTS function was expressed with functional aerobic impairment (FAI), left ventricular impairment (LVI) or myocardial aerobic impairment (MAI), heart rate impairment (HRI) or chronotropic reserve impairment (CRI) and peripheral circulatory impairment (PCI). All subjects were monitored on heart rate, blood pressure, electrocardiogram and endtidal O2 and CO2 before and every one minute during the symptom limited maximal stress testing. Seventy three of 80 coronary patients were subjected to the coronary arteriography and were classified into four groups; 31 with single vessel disease (SVD), 20 with double vessel disease (DVD), 15 with triple vessel disease (TVD) and 7 with A-C bypass surgery. Comparison between normal control subjects and the CHD patients with regard to the relation of age and VO2max derived from the linear regression analysis disclosed the identical age-related decrease in VO2max in both groups. The age corrected VO2max in the CHD patients, however, was 2.2 METS less than that of normal control subjects. Therefore, the level of VO2max in CHD patients was determined not only by disease, but also by ageing process itself. Comparisons among three CHD groups with regard to FAI, LVI, HRI and PCI clearly demonstrated different functional impairments paralleling to the severity of the disease process. On the other hand, the patients with A-C bypass surgery revealed almost identical functional impairment to the patients with SVD. In conclusion, these simple and noninvasive evaluations of the oxygen transport system could give us valuable informations reasonably differentiating the clinical status of the patients with CHD.

[Squamous cell carcinoma co-existing vesical leukoplakia--case study and its clinicopathological analysis of the pathogenesis].

A 66-year-old female complained of fever and lumbar pain was diagnosed as the squamous cell carcinoma associated with vesical leukoplakia cystoscopically. Histopathological figures showed various forms of squamous metaplasia, involving squamous cell carcinoma nests within it. Now, the relationship between vesical leukoplakia and squamous cell carcinoma is not obscure. Then the treatment of the leukoplakia has been considered conservatively (fulguration, acidification of the urine and anti-microbials). Now, we can classify the lesions of squamous metaplasias into two types in the urinary bladder. The possibility of the malignant change in the leukoplakia lesions could be suggestive of the histological characteristics; irregularly down growth and disarrangement of the basal cells and/or dispersed basement membrane in rete pegs in the metaplasia.

[Post-mortem observation of glioma and/or brain tissue compared to C-T scan findings after irradiation].

C-T scan findings in glioma and brain tissue after irradiation were histopathologically analyzed with the post-mortem examination of 4 patients. High density of contrast enhancement observed in gliomas mainly corresponded to proliferation of irregular blood vessels and hypodensity reflected less cellular area or necrotic foci in the tumor tissue. A mottled lesion with high density margin in C-T of radionecrosis of the brain tissue observed in a non-glioma patient was formed by coagulation necrosis showing marked proliferation of telangiectatic blood vessels in periphery of lesion and acellular area in the center. It is occasionally impossible to differentiate the findings between delayed radionecrosis of brain and recurrence of glioma using the C-T criteria alone.

[Spinal cord evoked potential in experimental spinal cord injury: the changes of spinal cord evoked potential following impact injury, and the correlation between the change in amplitude of the spinal cord evoked potential after injury and the prognosis for motor recovery of legs].

The authors reported the changes of spinal-cord-evoked potential following impact injury by the weight dropping technique (Allen's method). The correlation between the change in amplitude of the spinal-cord-evoked potential after injury, and the prognosis for motor recovery of the legs was discussed. The spinal cord of adult dogs was traumatized by the weight dropping technique (Allen's method) at the level of Th10. The trauma consisted of a 300 gm-cm impact injury, a 400 gm-cm impact injury, and a 500 gm-cm impact injury. The spinal-cord-evoked potential following direct stimulation of the spinal cord was recorded from the epidural space for 5 hours. The recording electrode was located cephalad to the site of the impact injury. The stimulating electrode was located at the caudal site of the injured area. The motor function of the dogs was observed by clinical scale for rating hind limb motor function (modified Tarlov score) for one month after the trauma. The recovery of the spinal cord evoked potential following impact injury by the weight dropping technique could be estimated one hour after trauma in the 300 gm-cm injury, 30 minutes after trauma in the 400 gm-cm injury and immediately only after trauma in the 500 gm-cm injury. A greater reduction of amplitude of I potential was noted with the higher energy injury (p less than 0.05). There was no parallel correlation between the change in amplitude of the spinal cord evoked potential after injury and the prognosis for motor recovery of legs. However, the changes in amplitude corresponded moderately well to the prognosis of motor function.

[The prognostic significance of esophageal carcinoma based on various irradiation methods].

From 1975 to 1987, 170 unresectable esophageal carcinomas were curatively irradiated. Ninety-eight cases were treated with conventional fractionation and a small field, 32 cases to a large T-shaped field, and 40 cases with conventional 50-60 Gy doses followed by 14-28 Gy of low-dose rate telecobalt therapy (LDRT: 1 Gy/hr, 4-7 Gy/day). The one-year, three-year and five-year survival rates in all cases were, 33%, 9% and 5%, and in LDRT groups, 37%, 15% and 8%, respectively. The survival rate and the local cure rate in the LDRT group were a little better than the others. The prognosis of T-shape group was proven to be the same as conventional group.

Left ventricular structural characteristics in unilateral renovascular hypertension and primary aldosteronism.

To assess the importance of the renin-angiotensin system and plasma volume as determinants of hypertensive left ventricular hypertrophy and its anatomy, patients with unilateral renovascular hypertension and primary aldosteronism were studied by echocardiography. Blood pressure, age and sex were matched as closely as possible. The 19 patients with unilateral renovascular hypertension and the 19 patients with primary aldosteronism were similar in age, sex and blood pressure (168 +/- 19/97 +/- 11 and 163 +/- 17/99 +/- 10 mm Hg, respectively), but plasma volume was increased in the patients with primary aldosteronism. Interventricular septal thickness, left ventricular posterior wall thickness, left ventricular mass index and relative wall thickness did not differ between the 2 groups of patients. There was a significant correlation between the level of systolic blood pressure and either left ventricular mass index (r = 0.34, p less than 0.05) or relative wall thickness (r = 0.58, p less than 0.001) in both groups of patients. Left ventricular end-diastolic dimension index was increased in the patients with primary aldosteronism compared with those with unilateral renovascular hypertension (3.2 +/- 0.4 vs 2.9 +/- 0.3 cm/m2, p less than 0.02). When confined to the patients with systolic pressure greater than or equal to 150 mm Hg, relative wall thickness was significantly increased in the patients with unilateral renovascular hypertension. Patients with primary aldosteronism and unilateral renovascular hypertension of similar blood pressure levels, age and sex have almost identical degrees of left ventricular hypertrophy and anatomy. In contrast, the patients with primary oldosteronism had increased left ventricular dimension index.(ABSTRACT TRUNCATED AT 250 WORDS)
