[A clinical study of recurrent breast cancer].

In a recent 14-year period from December 1978 to December 1993, 180 patients with breast cancer were treated at the hospital, and out of these 180 patients, 38 patients (21.1%) who had a recurrent breast cancer were clinically evaluated. The first recurrence occurred in the local skin in 13 patients, regional lymph node in 1, bone in 13, lung in 4, liver in 4 and in other areas in 3. Histologically, the incidence of scirrhous carcinoma recurrence was higher than that of papillotubular carcinomas. There was no recurrence in mucinous carcinomas. The frequency of recurrence became higher in accordance with TNM classification, namely, 6.7% in Stage I, 23.7% in Stage II, 37.5% in Stage IIIa, and 47.1% in Stage IIIb cancers. An increasing tendency in the recurrence rate was also noted with an increase of tumor diameter or lymph node metastasis. Otherwise, there was no difference in cumulative survival after recurrence between TNM classification. There were no correlations between the estrogen receptor and incidence of recurrence or 5-year survival rate. The disease free interval (DFI) was less than 2 years in about 60% of the recurrent cases. DFI of bone metastasis was longer than for the other sites of recurrence. The survival rate increased according to prolongation of DFI. After through evaluation of these results, one should pay close attention to follow-up after mastectomy.

[Functional magnetic resonance imaging of the frontal eye fields during saccadic eye movements].

We evaluated activity-induced signal intensity changes in the human cerebral cortex during horizontal saccadic eye movements using functional magnetic resonance imaging (fMRI) based on the blood-oxygenation-level-dependent (BOLD) contrast method. Compared with central fixation, significant signal increases were observed bilaterally in the middle frontal gyrus (Brodmann area 8) during saccadic conditions. The location of the activated area was consistent with that of previously reported frontal eye fields (FEF). These results suggest that fMRI has potential merit for the study of cortical control of eye movements in humans.

[Clinical study of intraspinal neoplasms in children].

The authors report a series of 10 children under 15 years of age with primary intraspinal neoplasm who underwent surgical resection from 1981 to 1994. The tumors consisted of 4 intramedullary tumors (myxopapillary ependymoma, pilocytic astrocytoma, subpial liporna, cavernous angioma), 2 intradural extramedullary tumors (2 neurinomas), and 4 extradural tumors (ganglioneuroma, chordoma, osteochondroma, rhabdomyosarcoma). The initial symptoms in these patients were gait disturbance (50%), weakness of lower extremities (50%), pain (40%), weakness of upper extremities (10%), and tetraparesis (10%). The neurological findings on admission in these patients revealed motor paresis (90%), sensory disturbance (60%), abnormality of deep tendon reflex (60%), pathological reflex (30%), neurogenic bladder (30%), and Lasegue's sign (10%). All patients were treated surgically; laminectoy was performed in 8 patients, laminoplasty in one patient, and transoral approach in one patient. Five patients (50%) underwent a grossly total resection, 3 (30%) had a subtotal resection, and 2 (20%) had a partial resection. Mean follow-up period was 5.6 years. One (10%) patient died due to recurrence and dissemination of the tumor at eight months after surgery in a case of rhabdomyosarcoma. 9 (90%) have had no recurrent tumor and have improved neurologically. In 4 patients (40%) spinal deformity developed, and 2 patients required operative treatment for the spinal deformity. We emphasized the advantage of laminoplasty to prevent postoperative spinal deformity in children.

Thallium-201 scintigraphy to assess effect of chemotherapy in osteosarcoma.

UNLABELLED: Imaging results in patients with high-grade osteosarcoma of the extremities were reviewed to determine whether scintigraphic appearance correlated with histologic response to preoperative chemotherapy. METHODS: Histologically, the percent tumor necrosis in specimens from 30 patients were classified into three grades: grade 1 = necrosis less than 60%, grade 2 = 60%-89% necrosis and grade 3 = diffuse necrosis greater than 90% based upon whole transverse sections. Scintigraphically, we analyzed 201TI uptake before and after preoperative chemotherapy. The changes in the tumor-to-background ratio were defined by an alteration ratio. RESULTS: Of the 11 patients with a grade 1 response, the ratio showed -67.1% +/- 45.4% (mean +/- s.d.). Of the 9 patients with a grade 2 response, the ratio showed 37.9% +/- 29.9% of the 10 patients with a grade 3 response the ratio showed 105.5% +/- 12.4%. The ratios correlated well with the histologic grades (p < 0.0001; analysis of variance). CONCLUSION: Thallium-201 scintigraphy accurately assesses the effect of chemotherapy on osteosarcoma.

[Transient heterogeneous enhancement on dual-phase helical CT of liver].

Transient heterogeneous enhancement was seen on the arterial phase of dual phase helical CT of the liver. The shape of the enhancement was appeared wedged or patchy. These phenomena without liver tumor were observed in 23 (2.3%) of 1012 patients with suspected hepatobiliary disease. Plain CT showed no attenuation difference in the liver. Twenty-two of these 23 cases were diagnosed as scarred liver, A-P shunt caused by liver biopsy, acute cholecystitis, liver abscess, liver cirrhosis, or advanced pancreas head cancer. The diagnosis of one case was uncertain. These phenomena were thought to be caused by 1) a regional direct increase in hepatic arterial flow due to arterial-portal (A-P) shunt, or hypervascular tumor ; or 2) a compensatory increase in hepatic arterial flow as a result of decreased portal venous flow caused by tumor invasion or severe liver cirrhosis.

The mammalian oviductal epithelium: regional variations in cytological and functional aspects of the oviductal secretory cells.

The secretory cells in the epithelium of mammalian oviducts produce and release various secretory materials into the lumen. Secretions from such cells provide a suitable environment for the events that occur in the oviductal lumen. This review focuses on the regional differentiation of the secretory cells in mammalian oviducts. Many histological studies have demonstrated regional variations in both the morphological and ultrastructural features of the secretory cells in the oviductal epithelium. Regional differences have been found, for example, in the number of putative secretory granules in the oviductal secretory cells. Histochemical and immunocytochemical studies have also revealed regional differences in the localization of various materials in the oviductal epithelium, suggesting the possibility of regional specificity in the production of various secretory materials by the oviductal epithelial cells. Recent biochemical and immunoelectron microscopical studies have shown that biosynthesis of specific proteins or glycoproteins is associated with region-specific variations in epithelial cells in different oviductal segments. In particular, certain oviduct-specific glycoproteins are produced by secretory cells in specific regions of the oviduct and these glycoproteins may affect fertilization, embryonic development, and sperm functions. The oviductal epithelial cell also provide physiological support to gametes and embryos. The interactions of oviductal epithelial cells with gametes support the development of embryos and the maintenance of sperm functions in vitro. Some studies using coculture systems have suggested regional differences associated with such physiological support by oviductal epithelial cells. Moreover, the expression of functional proteins, such as growth factors, show segmental variations within the oviduct. The regional variations demonstrated in these studies may reflect distinct functional differences among the various segments of the mammalian oviduct. The proposal is presented that despite the fact that the mammalian oviductal tissue is not very complex in terms of structure, the oviductal secretory cells may be highly differentiated along the length of the oviduct.

[Slow axonal transport in primate experimental glaucoma].

Axonal transport abnormality has been reported to be deeply related to glaucomatous optic nerve injury. These reports mainly investigate the axonal transport blockage using the model of acute elevation of intraocular pressure in normal primates. We studied such axonal transport abnormality especially of slow flow in laser-induced glaucomatous monkey eyes by tissue autoradiography. Abnormal silver grain accumulation suggesting axonal transport damage was seen in the areas of distorted lamina cribrosa in glaucomatous eyes. Accumulation of grains was also seen in the retro-laminar optic nerve in some areas. Normal control eyes showed almost homogenous grain distribution in the pre-lamina, lamina and post-lamina areas. Areas of slightly elevated, accumulation of grains were seen around the lamina cribrosa and disc margin suggesting physiological blockage of axonal transport.

[Atlantoaxial dislocation with ventilatory insufficiency--report of two cases].

We performed electromyography and polysomnography in two cases of atlantoaxial dislocation with ventilatory insufficiency to determine the patients' respiratory patterns. The results showed that hypertrophic sternocleidomastoid muscles (SCMs) played an important part in their respiration. The diaphragm and intercostal muscles did not display marked electromyographic (EMG) activity, but the SCMs showed a great deal of phasic inspiratory activity during the daytime that decreased or disappeared during sleep. A 6-year-old boy with atlantoaxial dislocation related to spinal abnormalities was chiefly dependent on the SCMs for respiration and required mechanical ventilation when a sleep because the SCMs were completely inactive. A 4-year-old girl with atlantoaxial dislocation as the result of an automobile accident was partially dependent on the SCMs. She had no trouble breathing during the daytime, but polysomnography recordings showed recurrent hypopnea and desaturation during the night synchronous with decreased EMG activity of the SCMs. She also required mechanical ventilation to improve her long-term prognosis. Polysomnography should be performed in severe cases of atlantoaxial dislocation with respiratory failure in addition to routine respiratory function tests.

[Effect of ulinastatin on delayed neuronal death in the gerbil hippocampus].

The effect of ulinastatin on delayed neuronal death was studied in the gerbil. The animals were divided into 2 groups according to the temperatures of the temporalis muscle, tympanic membrane, and the rectum. One group consisted of 36 animals in whom the temperature was maintained at 36.5 degrees C and the another was at 38 degrees C. They were anesthetized with isoflurane and injected intravenously with 5 ml.kg-1 of normal saline solution, 100 mg.kg-1 of alpha-tocopherol, 250,000 units.kg-1 of ulinastatin, or 500,000 units.kg-1 of ulinastatin, respectively. Transient ischemia was then induced by occluding bilateral common carotid arteries for 5 min. Six days later, they were sacrificed, and the brain tissues were fixed and stained for histopathological analysis of hippocampal CA1 regions. alpha-Tocopherol and ulinastatin prevented delayed neuronal death in the 36.5 degrees C group, but did not in the 38 degrees C group. alpha-tocopherol is recognized as an intrinsic radical scavenger, and known to prevent delayed neuronal death. Compared with 100 mg.kg-1 of alpha-tocopherol, 500,000 units.kg-1 of ulinastatin had the same protective effect on delayed neuronal death at 36.5 degrees C. We concluded that administration of ulinastatin prevented delayed neuronal death at 36.5 degrees C.

[Proteoglycan molecules in scleral tissue of enlarged eyeball].

We studied scleral specimens from experimentally induced enlarged eyeball with axial elongation by the transmission electron microscopy following cationic dye (cuplonic blue) staining. The animal model was prepared by the injection of alpha-chymotrypsin into the posterior chamber of young albino rabbits. Cuprolinic blue staining was applied to scleral specimens obtained from equatorial lesion and portions of the scleral tissues were subjected to enzyme digestion by chondroitinase ABC, AC and B before cuprolinic blue staining. In control eyes, dermatan and chondroitin type sulfated proteoglycan filaments were identified. Large, electron dense, and abnormal by shaped proteoglycan filaments were seen in the transmission electron microscopy. Such abnormal proteoglycan filaments were susceptible to enzyme chondroitinase ABC and AC digestion but resistant to chondroitinase B, suggesting that they are chondroitin sulfate dominant proteoglycans. Our morphological data corroborated a previous biochemical report of abnormally induced proteoglycan molecules in sclera with enlarged, axially elongated eyes.

[Stereotactic irradiation for brain metastases using linear accelerator].

A stereotactic irradiation (STI) system using a linear accelerator was developed and its effectiveness for brain metastasis was investigated. The system consisted of a conventional linear accelerator, invasive fixation head frame (RADFRAME, Mizuho Ika Kogyo), and a tungsten collimator. The RADFRAME was developed to be used with any linear accelerators and with any dose planning systems. The accuracy of the irradiation was assured to be within +/-1.5mm. Fifty-one patients with metastatic brain tumors were treated using the system. The overall median survival time was 10 months. No difference in survival was observed between patients with a single lesion and patients with two or three lesions. Complete, partial, and minimal responses were obtained in 16 (39%), 18 (41%), and 7 (16%) lesions. These were comparable to the reported results of gamma knife treatment. A maximum dose of 25 Gy was suggested to be more effective than lower doses. Local control rate was higher in tumors with a diameter of less than 2.0cm (87.5%) than in larger tumors (60%). No difference in tumor response was observed among various histopathologic types. In a patient with a 25mm cerebellar metastasis from an adenocarcinoma of the lung, autopsy at 5 months after STI showed fibrinoid degeneration and hyaline deposition without residual tumor at the Gd-enhanced region on MRI. The efficacy of linac stereotactic irradiation was equal to that of gamma knife radiotherapy for metastatic brain tumors.

[A case of spinal epidermoid caused by lumbar punctures].

A case of spinal epidermoid caused by lumbar punctures is reported. The 50-year-old man, who had been diagnosed as having poliomyelitis and who had been treated with lumbar puncture 24 times when he was 1 year old, came to our clinic complaining of gradual worsening of atrophy and weakness in his right leg during the last 10 years. Neuroradiologically, intraspinal tumor with calcification was detected in the lumbar region and partial removal of the tumor was performed. Pathological diagnosis was that it was epidermoid. During frequent lumbar puncture at the age of 1, some small skin grafts had been implanted intrathecally and we concluded that, changing its original nature, it had grown slowly to become epidermoid. We want to emphasize especially here that, when we performe lumbar puncture, we should, as far as possible, use a small needle with stilette so as not to induce future spinal epidermoid.

[Effects of prostaglandin E1 on transient forebrain ischemia, especially in hippocampal CA 1 regions of the gerbil].

The effects of prostaglandin E1 (PGE1) on transient forebrain ischemia were studied in the gerbil. The animals were randomly divided into six groups of 6 each according to the method of administration as follows: group 1, no administration; group 2, subcutaneous administration with 5 ml.kg-1 of physiological saline, 30 min prior to ischemia; group 3, the same method with 3 mg.kg-1 of PGE1; group 4, 3 mg.kg-1 of PGE1, during 24 hours (12 hours prior to ischemia, and 12 hours following ischemia); group 5, 3 mg.kg-1 of PGE1, during 96 hours (12 hours prior to ischemia and 84 hours following ischemia); and group 6, sham operation. They were anesthetized with isoflurane and transient forebrain ischemia was induced by occluding bilateral common carotid arteries for 5 min. The extracranial electroencephalogram (EEG) was recorded from the electrodes placed at the vertex. During the experimental procedures, temperatures at tympanic membrane and rectum were maintained at 37.0 +/- 0.2 degrees C by means of a heating mat and control of the air temperature in all groups. After 6 days of survival, they were sacrificed, and the brain tissues were fixed for the immunohistochemical and histopathological analyses. The hippocampal CA 1 regions were stained for monoclonal anti microtubule-associated protein 2 (MAP 2), and hematoxylin and eosin. In the 4th group, EEG recovery was recognized earlier than the other groups. Immunoreactivities for MAP 2 and the number of surviving pyramidal cells after ischemia in the CA 1 regions were also well maintained. These results suggest the PGE1, has protective effects against degradation of cytoskeletal proteins and delayed neuronal death in the gerbil, and it might be due to direct protective action of cell membrane in addition to its microcirculatory improvement.

[Dynamic Gd-DTPA-enhanced MR imaging in evaluation of the function of transplanted kidneys].

The purpose of the study was to evaluate the potential of dynamic Gd-DTPA-enhanced MR imaging in assessing of the function of transplanted kidneys. Dynamic MR imaging was performed using the GRASS pulse sequence in 21 subjects (56 examinations), and in each examination 30 images were obtained after a bolus injection of Gd-DTPA. Imaging findings including signal increase and/or signal drop in cortex, signal drop in medulla, and signal drop in calyces were utilized for the evaluation of renal function. Renal function was also evaluated using the following parameters: Ta, time between the initial observation of signal increase in cortex and signal drop in medulla. Tb, time between the initial observation of signal drop in medulla and in calyces; Tc, Ta + Tb, and Max. C/M, maximum ratio of signal intensity between cortex and medulla among 30 images. In the group of patients with normal graft function (Ccr > or = 60ml/min), all imaging findings were observed. In the group of patients with mild graft dysfunction (30ml/min < or = Ccr < 60ml/min), all imaging findings but signal drop in cortex were observed. The time parameters of Ta, Tb, and Tc were significantly larger, and Max. C/M was significantly smaller than those observed in the group with normal graft function. In the group of patients with severe graft dysfunction (Ccr < 30ml/min), almost none of the findings were observed. The investigation in patients in the postoperative state revealed a close correlation of the imaging findings and the values of parameters with changes in graft function over time. The results indicated the usefulness of the method in semiquantitative evaluation of graft function, including that of patients in postoperative state. This study suggests that dynamic Gd-DTPA-enhanced MR imaging could be a valuable method for the management of transplanted kidneys.

[Emergency myectomy in a patient with hypertrophic obstructive cardiomyopathy complicated with left heart failure and cardiac arrest].

Major cause of death in HOCM is sudden death due to ventricular arrhythmias, and only 10% of the death is caused by congestive heart failure. We report a 38-year-old woman of HOCM complicated with lung edema and cardiac arrest due to acute left heart failure. Intraventricular pressure gradient showed 125 mmHg in previous study, and echocardiogram demonstrated evidences suggestive of terminal-staged HOCM. After unsuccessful attempts of intravenous propranolol administration, emergency myectomy of left ventricular outflow tract was carried out. Postoperative course was uneventful, and intraventricular pressure gradient was relieved. The patient is now asymptomatic without medication after 1 years of operation. This case suggests that emergency myectomy may be a choice to relieve refractory congestive heart failure in patient with HOCM.
