[Sexual disorders in men with diabetes].

This research discusses the issues of erectile disfunction in diabetes, which constitute not only a medical but, what results from its spread, also a social problem. Diabetes as a social disease causes many metabolic disorders, dysfunction of many organs, including erectile dysfunction. Because of the fact that there are so many disorders in organs, people who suffer from diabetes should be especially carefully treated in view of metabolism. Erectile dysfunctions in diabetes have definitely an organic background: connected with vessels, neurologic, endocrinologic and psychological functions.

[Selected conditions influencing the frequency of prophylactic gynecologic visits].

The aim of the study was to estimate the frequency of prophylactic visits at gynaecologist among women and its correlation with: age, place of living, material status or kind of gynaecological illness. The study was carried out among 200 patients with different gynecological illnesses operated on in Department of Reproduction and Andrology University School of Medicine in Lublin. The study was carried out on the basis of author's questionnaire interview and analyses of medical documents. Participation in this study was voluntary and anonymous. The result showed that, 52% of women declared their regular (every half year and every year) visits at gynaecologist. Regularity of prophylactic visits depended on age, in favour of women under 35 year. The frequency of control visits was significantly higher among women living in the city. The majority (88.2%) of patients in the group of continuous medical therapy was the ones suffering from infertility. The education and material status did not influence the regularity of prophylactic visits at gynaecologist.

Comparison of the levels of copper, zinc and iron in cervical mucus and in blood serum of women of childbearing age.

The objective of this research study was a preliminary comparison of some chosen trace elements concentrations in cervical mucus and blood serum of women of childbearing age. The levels of iron (Fe), copper (Cu) and zinc (Zn) were determined in cervical mucus and serum of 10 patients. The obtained results do not point to any association between the levels of the examined trace elements in cervical mucus or any correlation between their contents in mucus and blood serum. As no correlation was found between the blood serum and cervical mucus, as far as trace elements are concerned, one might suggest that the uptake of trace elements by the mucus has an active form.

Forceps delivery after two previous Caesarean sections.

Caesarean section is one of the most frequently performed surgical procedures in current obstetric practice. There are several reasons for this upward tendency, including well developed hemotherapy, anesthesia, antibiotic treatment and skilled doctors. Despite such risks as uterine rupture, necessity of blood transfusion or the possibility of the necessity of ending the delivery by means of surgery, it is possible, in justified cases, to attempt spontaneous labor. A decision jointly made by the patient and her doctor, based on careful consideration of benefits and risks, as well as positive attitude on the woman's part may lead to positive outcome of labor. Taking the above factors into account we have decided to present a study of a difficult, operational labor after two Caesarean sections.

Evaluation of some parameters of human semen with positive chlamydial reaction.

The objective of the present research study was to compare some parameters of human semen of positive immunoenzymatic test for the Chlamydia trachomatis presence in blood serum with the results of those with the negative test results. Seventy (70) men with genitourinary system problems were examined: in 33 cases positive chlamydial tests were found as compared to 37 negative tests. The parameters of semen were compared in both groups. Statistically significant sperm reduced motility was found in the group with positive chlamydial test results.

Two-year studies of women with fertility problems following uterine septum hysteroscopic treatment.

The aim of the study was an evaluation of the effectiveness of uterine septum surgical treatment in women with fertility disturbances. Retrospective analysis of two-year observation of obstetric histories of women who were subjected to hysteroscopic treatment of the uterine abnormalities was performed. The analysis of the therapeutic outcomes of the endoscopic uterine septum correction supports the opinion that the operative procedure significantly increases the number of term deliveries.

Consumption of some selected trace elements (Fe, Zn and Cu) and reproductive health of women from the Podkarpackie province.

The objective of the present research study was to estimate the average daily consumption of some selected trace elements (Fe, Zn and Cu) by women of reproductive age and connect it to some aspects of health-oriented behaviour in the Podkarpackie province. We noticed that patients under 30 years of age consumed more examined trace elements (Fe, Zn i Cu) in their daily food rations than patients over 30 years of age.

Alterations in the expression of selected MHC antigens in premalignant lesions and squamous carcinomas of the uterine cervix.

BACKGROUND: The objective of the study was to determine whether there is a correlation between expression of selected major histocompatibility complex (MHC) antigens and the presence of carcinoma and premalignant lesions of the cervix. We also attempted to determine whether there is a correlation between expression of the selected MHC antigens in each grade of cervical intraepithelial neoplasia (CIN) and invasive carcinoma of the cervix. METHODS: Quantitative comparative analysis of MHC class I and class II expression between specimens of the uterine cervix was carried out. For detecting human leukocyte antigen (HLA) class I molecules, two primary antibodies were used, recognizing either epitopes of all free heavy chain class I molecules or epitopes of the HLA-Bw4 antigen. HLA class II antigens were detected by antibodies reacting with the alpha-chain of the HLA-DR molecule. An indirect immunoperoxidase technique was carried out using paraffin-embedded sections. The following groups of sections were formed: squamous metaplasia, CIN of different grades, and invasive carcinoma. The specimens were taken from women in whom routine histopathologic diagnosis of cervical lesions had been performed. RESULTS: A significant decrease in the mean number of stained cells showing expression of MHC class I antigens (HC 10 and Bw4) was found in the invasive cancer group of specimens. A decrease in Bw4 expression was also found in the CIN III group. The highest expression of the HLA-DR antigens was detected in the metaplastic epithelium compared to the other groups. In the dysplastic epithelium the expression of the HLA-DR antigens was increased in the highest grade of dysplasia (CIN III). Upregulation of HLA-DR expression was detected in invasive cancer specimens. CONCLUSIONS: Alterations in MHC expression are found in premalignant lesions and squamous carcinomas of the uterine cervix. Quantitative assessment with the use of an image analysis system is valuable in helping to discriminate staining patterns of HLA expression. Further studies are indicated to evaluate the prognostic value of the expression of selected MHC antigens.

[The influence of asymptomatic leukocytospermia on interleukin IL-2 and IL-6 levels in male semen plasma].

AIM: The study presents the results of the search for a correlation between leukocytospermia and the interleukin IL-2 and IL-6 levels in male semen plasma. MATERIAL AND METHODS: Interleukin levels were assessed by immunoenzymatic method (ELISA). Study covered 3 groups of males: I--fertile (No. = 16); II--with leukocytospermia (No = 16); III--with oligoasthenozoospermia (No = 20). RESULTS: The study revealed higher levels of IL-6 in a group of males with leukocytospermia (10.01 +/- 2.57 pg/ml) and with oligoasthenozoospermia (13.53 +/- 2.78 pg/ml) compared to a group of fertile males (7.68 +/- 1.4 pg/ml) (p < 0.01), however no statistically significant differences in IL-6 levels between group II and III have been observed. Higher level of IL-2 has been noted in III group (16.68 +/- 5.86 pg/ml) compared to I and II group (9.77 +/- 3.18 pg/ml and 6.05 +/- 2.16 pg/ml, respectively) (p < 0.01). No statistically significant difference in IL-2 semen plasma level between the fertile male group and the group with leukocytospermia has been revealed. CONCLUSION: Leukocytospermia is accompanied by an increase in IL-6 semen plasma level whereas IL-2 level remains statistically unchanged.

[Melatonin and its role in human reproduction].

Melatonin may be a key factor in regulation of seasonal variation in gonadal activity. The circadian disturbances related to reproduction are probably subsequent to the seasonal change. Moreover, melatonin might also be considered essential to both spermatogenesis and folliculogenesis. Exposure to bright light, suppressing the concentration of melatonin in circulation, is hypothesized to be useful in treatment of both male and female infertility in couples with abnormal melatonin metabolism. In women, seasonal variation observed in the concentrations of gonadotropins and gonadal steroids is associated with melatonin synthesis. This hormone may exert an inhibitory effect on the GnRH pulse generator to decrease gonadotropin secretion. Consequently, it has been implicated in the control of the pubertal onset and timing of ovulation. The number of irregular menstrual cycles with anovulation in creases in winter compared to summer. In addition, there is circadian variation in the time of ovulation, occurring usually in the morning during summer and in the evening during winter. Hormonal interactions and phases of the cycle are important variables contributing to the fluctuations in melatonin levels during the menstrual cycle.

Fertility following laparoscopic uterine myomectomy in an infertile patient treated for 10 years. Case report.

The presence of myomas in the uterus is a relevant factor in infertility cases. Myomas may reduce contractility of the uterus, hinder migration of sperm and vascular changes within myomas themselves interfere with embryo implantation. The paper presents the case of a patient with over 10-year-history of infertility treatment diagnosed with numerous myomas of the uterus. The medical records revealed that the patient had undergone laparoscopy and laparotomy due to the left ovarian cyst and had been subjected to hysteroscopic removal of the uterine partition twice. The patient took part in the IVF programme twice--without success. At our Department the patient underwent laparoscopic myomectomy during which 5 myomas were removed: 4 subserous ones, 1.5-2 cm in diameter and an intramural one, 6 cm in diameter. The sites of the biggest myoma were laparoscopically sewn putting in 3 single sutures. In her next cycle the patient spontaneously conceited. The pregnancy was without serious complications and at 38 weeks was terminated by Caesarian section. The infant was delivered in good general condition (10 Apgar points) and weighted 3,360 g. On Caesarian section no uterine dehiscence following laparoscopic myomectomy was observed.

Activity of superoxide dismutase and glutathione peroxidase in human semen in normozoospermia and spermatopathy.

The aim of the study was to assess the levels of antioxidative enzymes--superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in the whole semen of men treated for infertility. The first group consisted of the patients with normozoospermia; the second--included the men with oligozoospermia. According to sperm motility, the patients were divided into the third group--motility higher than 50% and the fourth group--motility lower than 50%. The study showed that the levels of both enzymes were higher in men with oligozoospermia than those with normozoospermia. It was also found that the activities of SOD and GSH-Px were higher in men with low motility(< 50%) compared to high motility patients. The results confirm that the activity of superoxide dismutase and glutathione peroxidase is related to the parameters of human semen assessed according to the WHO standards.

[Androgen deficiency in women].

Androgens are defined as the steroids having a binding affinity of the androgen receptor. In the reproduction age a daily production of testosterone is equally divided between the ovaries and adrenal and local tissue conversion of androstenedione and DHEA. After menopause the 80% of testosterone is produced in ovaries, but majority of precursors for peripheral conversion is adrenal origin. Androgen receptors are present throughout in the body; over 200 cellular actions of androgens have been described. Androgenic action is determined by quantitative level of the androgen present in the circulation, its degree of binding to proteins, the degree of interconversion to other androgens and estrogens, and the biological potency and androgen receptor binding affinity of the androgen. The most common clinical symptoms of androgen deficiency are the reduction of sex motivation, sex fantasy, sex enjoyment, sex arousal, vaginal vasocongestion, but also reduction of pubic hair, bone mass, muscle mass, worsening of quality of life (mood, affect, energy), more frequent vasomotors symptoms, insomnia, depression, headache. All these signs and symptoms can be multifactorial. Most common conditions associated with hypoandrogenism in women are hypothalamic-pituitary abnormalities, lack or insufficiency of ovaries, adrenal insufficiency, glucocorticoid therapy, exogenous estrogen administration. Besides the clinical picture the free testosterone measuring is important for diagnosis. The method of choice of this measure is equilibrium dialysis assay. Despite of clinical importance of androgen insufficiency in women, none of methods of androgen substitution is approved by FDA.
