Acute cerebellitis.

Many recent reports of acute cerebellitis with a critical or potentially critical course have changed the conception of the disease from an acute cerebellar ataxia with a benign course to a notable disease entity with a heterogeneous pathogenesis. The most characteristic finding by magnetic resonance imaging is diffuse cortical swelling of the cerebellum, often complicated by hydrocephalus or tonsillar herniation. A timely assessment and appropriate choice of treatment, including surgical intervention when indicated, are key to improving the outcome of affected patients.

Left ventricular systolic and diastolic function during early neonatal period using transthoracic echocardiography.

To examine the effects of ductal closure on left ventricular (LV) systolic and diastolic function during the early neonatal periods, 45 normal term neonates delivered after uncomplicated pregnancies (mean 39 weeks) were studied using two-dimensional and Doppler echocardiography. We measured ductus arteriosus size, arterial blood pressures, ascending aortic size, LV dimensions, and transmitral flow velocity patterns and calculated LV output and rate-corrected fiber shortening fraction (mVcfc) at 2, 12, 24, and 120 hours after birth. The inner diameter of the ductus arteriosus was 4.3 +/- 0.7mm at 2 hours, 2.1 +/- 0.6 mm at 12 hours, and had closed in 42 of 45 neonates at 24 hours. LV output and LV end-diastolic dimension showed the highest level at 2 hours of age. However, the mVcfc did not change from 2 to 120 hours of age. The peak velocity during early diastole (peak E) was significantly greater at 2 hours than at 12 hours. The peak velocity during atrial contraction (peak A) remained unchanged during this period. The normalized peak filling rate at isovolumic relaxation time did not change over 120 hours. The present study demonstrated changes in LV systolic function and LV diastolic filling during the early neonatal period. LV systolic and diastolic function was preserved under the hemodynamic changes associated with the early neonatal period.

Distal pulmonary cell proliferation is associated with the expression of EIIIA+ fibronectin in the developing rat lung.

Fibronectin is known to regulate the development of the lung. The EIIIA segment of fibronectin is one of the major alternatively spliced segments and modulates the cell proliferative potential of fibronectin in vitro. But the specific role of the EIIIA-containing fibronectin isoform (referred to as EIIIA+ fibronectin) in pulmonary cell proliferation has not been explored in vivo as yet. In this study, to see whether EIIIA+ fibronectin is associated with pulmonary cell proliferation in vivo, the authors immunohistochemically examined the spatiotemporal relationship of the EIIIA+ fibronectin isoform protein and proliferating cell nuclear antigen (PCNA), a biochemical marker for the cell proliferation, during the alveolar formation in the developing rat lung. EIIIA+ fibronectin protein was localized in the epithelial cells and in the mesenchymal tissues. The expression of EIIIA+ fibronectin protein gradually decreased from the pseudoglandular stage to the saccular stage and then slightly increased from the saccular stage to the alveolar stage. This change in the EIIIA+ fibronectin expression seemed to be in accord with the change in the number of PCNA-positive cells in the distal pulmonary cells throughout the lung development. These results suggest that the expression of EIIIA+ fibronectin is associated with the distal pulmonary cell proliferation during the alveolar formation.

Elevated sphingomyelinase and hypercytokinemia in hemophagocytic lymphohistiocytosis.

PURPOSE: Ceramide generated from sphingomyelinase activation has been reported to play a role in cytokine-mediated events. Secretory sphingomyelinase (S-SMase), a product of the acid sphingomyelinase gene, has been found to be derived from many cell types and to exist in human serum. The purpose of the current study was to investigate the serum level of S-SMase. PATIENTS AND METHODS: Three patients with hypercytokinemia caused by hemophagocytic lymphohistiocytosis (HLH) were studied. Serum samples were collected from the three patients with HLH, patients with a deficiency of acid sphingomyelinase, or type B Niemann-Pick disease, and normal controls. The S-SMase activities were determined using 14C-sphingomyelin as a substrate. RESULTS: The serum S-SMase activities were increased 10-to 20-fold in the sera from the three patients with HLH (P < 0.0001). The high S-SMase activity was decreased to the normal level along with the clinical improvement of HLH. CONCLUSIONS: Increased release of S-SMase from cells into the circulation is observed in patients with active HLH. The authors suggest that S-SMase is related to the pathophysiology of hypercytokinemia.
