[Correlation of the uptake of technetium-99m methoxyisobutyl isonitrile with expression of multidrug resistance genes mdr-1 and MRP in human lung cancer].

OBJECTIVE: To investigate the relationship between the uptake of technetium-99m methoxyisobutyl isonitrile ((99m)Tc-MIBI) and the expression of multidrug resistance genes mdr-1 and MRP in lung cancer so as to guide chemotherapy. METHODS: Eighteen lung cancer patients were given 1 110 MBq(99m)Tc-MIBI intravenously. Chest single-photon emission-computed tomography was performed 60 and 120 min after the injection. The early and delay uptake rates and retain index of (99m)Tc-MIBI were calculated by the count ratio of the lung lesion to contralateral normal lung tissue. The expressions of mdr-1 and MRP genes in specimens of lung cancer and normal lung tissues near the cancers resected during operation among the 27 patients were detected with RT-PCR method. RESULTS: The positive rate of (99m)Tc-MIBI in the chest of lung cancer patients was 83.3% (15/18). The early uptake ratio of imaging in lung cancer patients was 1.99 +/- 0.64. After time decay correction, the rate of target to background and the retain index in 13 delay imagings were 2.06 +/- 0.69 and -45% approximately 33% respectively. In specimens of lung cancer, the positive expression rates of mdr-1 and MRP were 22.2%(6/27) and 63%(17/27) with the expression amount of 0.39 +/- 0.1 and 0.23 +/- 0.17 respectively. In specimens of normal lung tissues near lung cancer, the positive expression rates of mdr-1 and MRP genes were 35%(7/20) and 45.0%(9/20) with the expression amount of 0.44 +/- 0.14 and 0.17 +/- 0.18 respectively. In lung cancer tissues, the positive rate of MRP was higher than that of mdr-1 (P < 0.05), however, in the tissues near lung cancer, no significant difference was found between the positive rate of MRP and that of mdr-1 (P >< 0.05). There was no marked correlation (P >< 0.05) among the co-expression or expression of mdr-1 and/or MRP and the early uptake or retain index in lung cancer. CONCLUSION: The primary chemoresistance of lung cancer has not significant correlation to the expression of multidrug resistance genes mdr-1 and MRP. Other resistance mechanism may exist. (99m)Tc-MIBI imaging has not much clinical value in predicting the expression of mdr-1 and MRP genes.

Human motion planning based on recursive dynamics and optimal control techniques.

This paper presents an efficient optimal control and recursive dynamics-based computer animation system for simulating and controlling the motion of articulated figures. A quasi-Newton nonlinear programming technique (super-linear convergence) is implemented to solve minimum torque-based human motion-planning problems. The explicit analytical gradients needed in the dynamics are derived using a matrix exponential formulation and Lie algebra. Cubic spline functions are used to make the search space for an optimal solution finite. Based on our formulations, our method is well conditioned and robust, in addition to being computationally efficient. To better illustrate the efficiency of our method, we present results of natural looking and physically correct human motions for a variety of human motion tasks involving open and closed loop kinematic chains.

[The observation of local immune response in the lungs from antigen-sensitized and challenged mice].

OBJECTIVE: To evaluate the local immune response after allergic sensitization and challenge in the lungs of mice. METHODS: C57BL/6 mice were sensitized with keyhole limpet hemocyanin (KLH) by intratracheal instillation and challenged 2 approximately 4 weeks later. Bronchoalveolar lavage fluid (BALF) and plasma were collected and cells from the lung-associated lymph node (LALN), the lungs and the spleen were cultured and collected. Anti-KLH IgA, total IgA and albumin levels were measured by ELISA. RESULTS: Intratracheal instillation of KLH induced local BAL antigen-specific IgA response, and further challenge expanded this response. The ratio of anti-KLH IgA/albumin in BALF was significantly higher than that in serum after both sensitization and challenge by KLH. In vitro, cells from the LALN and the lungs released anti-KLH IgA after sensitization and challenge by KLH, but spleen cells released lower levels of anti-KLH IgA compared to the LALN and the lungs. CONCLUSIONS: Allergic sensitization by intratracheal instillation of KLH into the mouse lungs induced a local pulmonary response of specific IgA, and it was amplified by challenge with KLH. The accumulation of anti-KLH IgA in the respiratory lumen was the result of local production but not the result of simple transudation or leakage from the serum. The LALN and the lung lymphocytes were major sources of anti-specific IgA.

[Treatment of elbow joint ankylosis by repair of articular surface with periosteal autograft].

OBJECTIVE: To evaluate the clinical effect of periosteal autograft in repair of ankylosis of elbow joint. METHODS: From May 1985 to November 1999, 18 cases of elbow joints ankylosis (6 cases of osteo-ankylosis, 12 cases of fibroankylosis) were treated by repairing articular surface with periosteal autografting. Out of 18 cases, 13 were caused by old dislocation and fracture of elbow joints, 3 by late rheumatoid arthritis, and 2 by old total joint tuberculosis. In this surgical approach, periosteum from upper end of tibia was transplanted into articular surface after correction of the elbow joint from ankylosis deformity, and continuous passive or active movement of the operated joint was adopted with skeletal traction through olecranon of ulna for 4 weeks after operation. All of the cases were followed up for 1-9 years, 5.2 years on average, before clinical evaluation. RESULTS: The elbow joints in 11 cases were restored to normal, the joints in 4 cases obtained active movement in the range of 100 degrees-0 degree, and the joints in the other 3 cases could only have limited movement because of severe muscular atrophy. CONCLUSION: The articular surface in arthroplasty of elbow joint ankylosis could be effectively repaired by periosteal autograft, and the function of the joints could be obviously improved by continuous movement of the joints after operation with skeletal traction.
