Intraoperative high-flow antegrade irrigation. A new bowel-cleansing system.

A new intraoperative method, employing a specially designed apparatus, is described by which the colon can be cleansed to a degree not obtainable by other methods. The method enables continuous antegrade irrigation with large volumes of irrigant at high rates of flow, prior to enterotomy. It has been tested in an experimental canine model and found to be safe and effective.

The effect of oral corticosteroid dosage on CT enhancing multiple sclerosis plaques.

Fifty high-dose delayed CT scans were performed on 36 patients with definite MS within 4 months of a clinical relapse. The number of enhancing lesions visualized was compared in patients being treated with high- or low-dose oral corticosteroids and untreated controls. High-dose oral corticosteroid treatment significantly but incompletely reduced enhancement of MS plaques. Lower doses were less effective, especially when the latter were given on alternate days.

Intracranial subdural sarcoid granuloma. Case report.

A patient is reported in whom a subdural sarcoid granuloma caused symptoms of an intracranial mass lesion, and disappeared following steroid therapy. Twenty-three previous cases with large intracranial sarcoid granulomas have been documented, a review of these 24 cases leads the authors to conclude that: 1) neither symptoms nor nonhistological diagnostic studies, including computerized tomography, differentiate sarcoid from cerebral neoplasms or other central nervous granulomas; and 2) because sarcoid mass lesions frequently respond well to corticosteroid therapy, surgery should probably be reserved for cases in which there is diagnostic uncertainty, a need for emergency decompression, or lack of response to steroids.

Multiple vertebral hemangiomas resembling metastases with spinal cord compression.

A 58-year-old woman with spastic paraparesis had bony hemangiomas involving the spine, sacrum, and skull. Multiple bony lesions, paravertebral mass, abnormal serum protein levels, and myelopathy suggested multiple myeloma or metastatic neoplasm. Decompressive laminectomy demonstrated a benign capillary hemangioma.

Histologic findings in motor neuron disease. Relation to clinically determined activity, duration, and severity of disease.

Correlation of 18 histologic variables with age and sex of 24 patients with motor neuron disease (MND), and the duration, severity, and activity of their disease, showed that high density of atrophic fibers correlated with degree of muscle weakness and the worst prognosis and that type I grouping correlated with the best prognosis. Although both type I and type II fibers are involved in the majority of patients with MND, the data suggest that involvement of type I fibers is more important in relation to activity of the disease.
