Biochemical characteristics, phage patterns, and O1 factor analysis of Escherichia coli O1:K1:H7:F11 and O1:K1:H-:F9 strains isolated from patients with urinary tract infections.

Biochemical characteristics, O1 antigen factors and phage patterns were examined in 35 urinary O1:K1 isolates of Escherichia coli different in H and F antigen. Fermentation of dulcitol, decarboxylation of ornithine, requirement for nicotinamide, and determination of O1 factor d allowed maximum differentiation. On the basis of these tests the strains could be divided into two major groups which are obviously of different clonal origin. Members of clone 1 represented by serotypes O1:K1:H7:F11 (12 strains) and O1:K1:H-:F11 (5 strains) were characterized by positive biochemical reactions and absence of O1 antigen factor d. Negative biochemical tests and presence of O1 antigen factor d were shown by strains of clone 2 which were of serotypes O1:K1:H-:F9 (14 strains) and O1:K1:H-:F- (3 strains). Phage patterns are less well correlated with clonal assignment. However, strains of clone 2 were not susceptible to K1-specific phage D and were non-typable with another set of 13 phages.

Fimbrial antigens of Escherichia coli 01:K1:H7 and 01:K1:H- strains isolated from patients with urinary tract infections.

Thirty E. coli strains belonging to serotypes 01:K1:H7 and 01:K1:H- were examined for the presence of fimbrial (F) antigens by crossed immunoelectrophoresis. All strains exhibited a mannose-resistant haemagglutination of human and monkey erythrocytes. This ability was found to be highly correlated with the existence of F antigens. Another correlation was found between O:K:H serotype and F antigen. Eleven 01:K1:H7 strains were associated with fimbrial antigen F11 which was also demonstrated in five immotile variants of this serotype. In other fourteen strains of serotype 01:K1:H-, however, the F9 antigen could be detected. The F9 and F11 antigens belong to the group of P fimbriae. The 01:K1:H7:F11 and 01:K1:H-:F9 isolates were supposed to be of different clonal origin.

[Relation between the O:K:H serotype and the fimbria antigen in E. coli of urinary tract infections].

Following introductory remarks on the typing antigens of E. coli and the relation between O:K:H serotypes and urinary tract infections (UTI), the importance of fimbriae as adhesive factors for the pathogenesis of UTI is pointed out. Our own results on the haemagglutination (HA) patterns and the establishing of fimbrial (F) antigens in E. coli strains from patients with UTI are presented. 42 O1:K2 strains were examined for the presence of F-antigens using crossed immunoelectrophoresis. In 31 strains with mannose-resistant HA from human and monkey erythrocytes this characteristically correlated with the presence of F-antigen. A close relation between O:K:H serotypes and F-antigen was found. In twelve O1:K1:H7 strains and five immobile variants of this serotype we succeeded in identifying the F11 antigen. 14 strains of the serotype O1:K1:H- had the F9 antigen. It is assumed that O1:K1:H7:F11 (incl. O1:K1:H-:F11-) strains and O:1:K1:H-:F9 isolates are derived from different clones.

[Serum resistance of Escherichia coli in chronic pyelonephritis. 1. Serum resistance in the human serum pool].

123 patients of the kidney department of the Clinic for Inner Medicine of Rostock University suffering from chronic pyelonephritis were taken into microbiological observation for between one and four years. 170 E. coli strains were bred from 59 patients with significant bacteriuria in the course of the disease and their serum resistence was determined with pooled human serum using Taylor's method. 78.24% of the strains examined were serum-sensitive, 11.18% intermediate and 10.59% serum-resistent. All strains were O-, K- and H-typed. 57.06% were successfully O-typed and were distributed over 40 O-serogroups. 24.12% were not typable and 18.82% were rough colonies. 86.50% of the resistent and intermediate strains strains were O-typable, 13.50% could not be typed. The significance of E. coli antigens (O, K, H) and serum resistence for the maintenance of a chronic infection is discussed.

[Serum resistance of Escherichia coli and serum bactericidal activity in chronic pyelonephritis].

123 patients with chronic pyelonephritis of the renal dispensary of the clinic of internal medicine of the Department of Medicine Wilhelm Pieck University Rostock were controlled microbiologically for one to four years. Of 59 patients of this group with significant bacteriuria we cultivated 170 strains of E. coli, which were standardised O, K, and H. Of all causative organisms the serum resistance was determined in a human serum pool. All sera of patients were examined for their bactericidal activity to the homologous E. coli urine strain as well as to a serum-sensitive E. coli reference strain (E. coli O 22:K-:H+). In 11.8% we found in the sera investigated at the time of the excretion of causative organisms disturbances of the serum bactericidal action, which rendered possible the survival of the sensitive in the human serum pool causative organism. In 8.82% of the sera of patients at the time of bacteriuria there was a decreased bactericidal action against the E. coli reference strain, only two of these sera did also not kill the urine strain of the patients. In individual cases sera of patients were examined for bactericidal activity with the homologous germ after absorption. By this absorption the antibactericidal activity of the sera could be removed. The proof of a disturbance of a bactericidal action may be an important reference for the course of the chronic pyelonephritis.

[Immunoadsorption using Staphylococcus aureus, COWAN I strain].

Immunoadsorbents bind immunoglobulins, immunocomplexes or immunocytes. Their clinical use could become a new therapeutic principle for diseases in the pathogenesis of which antibodies or immunocomplexes play an important part. Immunoadsorbents are also being tested in the hope that they can at least partially replace the plasmapheresis-plasma exchange therapy at present practised. A report is given on in-vitro studies using protein-A-bearing staphylococci, strain COWAN I. The bacteria bind IgG selectively, IgM to a small extent, IgA, IgD and IgE hardly. The majority of the other plasma proteins tested (e.g. albumin, transferrin, ceruloplasmin, alpha-2-macroglobulin, beta-lipoprotein, alpha-2-glycoprotein, alpha-1-antitrypsin) are bound only non-specifically and in small quantities. The staphylococci can react with acid solutions (glycin-HCl buffer, acetic acid) in several ways. The activation of the complement system by IgG binding to protein A is one of the problems to be solved before immunoadsorbents an be used clinically.

[The effect of plasmids of various incompatibility groups on the serum sensitivity of Escherichia coli K 12].

Using the method of Henkel and Taylor (11, 18) 60 Escherichia coli K 12 J 53 strains carrying R-plasmids of 15 incompatibility groups (FI, FIme, FII, FIV, FV, FVI, I alpha, I gamma, I omega, I5, B, C, K, M, S) have been tested with respect to their serum sensitivity in pooled human serum. We found for 41 plasmid carrying strains a grade of resistance to serum bactericidal activity of 0, for 19 strains a grade of 1, 2 or 3 according to the method of Henkel. Applying the method the method of Taylor the results of the bactericidal tests of the same 60 strains could all graded in category 1. The results obtained show that there was not any change of sensitivity to serum bactericidal activity after the uptake of a plasmid. 29 strains carrying different plasmids of the same 15 incompatibility groups were selected for studies in a low concentration system of serum. Survivors' curves in 1, 2, 3, 5, 10 and 20% of serum after different times of incubation (10, 20, 30, 60, 90, 120 and 180 min) were established. In relation to the control strain E. coli K 12 J 53 R- a difference in the percentage of survivors of 18 strains carrying plasmids of 10 incompatibility groups (FI, FIme, FIV, FVI, I alpha, I omega, I5, C, K, and M) could be demonstrated. However, this could only be observed in concentrations of serum from 1 to 5% and most clearly after an incubation time of 10 min. The results were graded into 3 categories in dependence of the difference of the percentage of survivors to the control strain. The plasmid pIE 307 and pIE 403 (Inc FI and C) showed the strongest activity in increasing the serum resistance. We found the method used to be suitable for the estimation of small alterations of serum sensitivity. The results obtained in E. coli K 12 made us control their influence in other E. coli strains carrying a complete O-antigen.
