Lower extremity amputees with peripheral vascular disease: graded exercise testing and results of prosthetic training.

Thirty-nine subjects, mean age 72 (range 52 to 89), with a leg amputation because of peripheral vascular disease performed graded exercise testing at the start of their prosthetic training program. Their walking performance at the end of the program was assessed and compared with the test findings. They had a history and rest electrocardiogram (ECG) examination which revealed cardiac problems in 75% of the patients. The exercise test was performed on a specially designed arm ergometer permitting coordinated exercise of arm and trunk muscles. Cardiac condition was judged by the achieved peak heart rate (mean 125 +/- 3.8 beats per minute) and observed ECG abnormalities. In only three patients exercise-induced ECG abnormalities were found at peak workload. The mean peak workload was 52 +/- 1.9W. It was concluded from these results that the average physical and cardiac condition is poor in vascular leg amputees. In 34 patients (87%) the prosthetic training was successful. Fourteen patients needed a walking frame and twenty could walk without a walking frame. The probability of achieving walking without a walking frame was 70% in patients with peak workload above 45W and 30% in those with peak workload lower than 45W.

A new gait analysis system for clinical use in a rehabilitation center.

A semi-portable system based on a microcomputer has been developed to measure the vertical reaction forces on both feet during walking. Eight capacitive force transducers are attached to each sole of the patients' shoes. This allowed the forces to be measured for several consecutive steps during a walk of 20 sec. This article describes the principles of operation of the force transducers and the associated electronic system. It also shows data processing procedures and gives a review of the available methods of data presentation. Recordings of the walking pattern of three patients with different diagnostics are presented and discussed to demonstrate the usefulness of the new gait analysis system in a rehabilitation center.

Electrode implantation and electrostimulation in rabbits.

The investigators studied functional electrostimulation by means of an implanted electrode placed around the peroneal nerve in rabbits. Current pulses were administered from a stimulating unit outside the body, via a connecting skin plug and subcutaneous leads to the electrode. It is concluded that if this form of stimulation is to be praticable without undesirable results, certain mechanical problems require further study.

A low-cost FES exercise bicycle for training paraplegics at home.

The success of FES exercise programmes for training paraplegic muscles at home depends highly upon the availability of reliable, easy-to-use and inexpensive training equipment. For endurance training, FES bicycle exercisers are well accepted. However, they are not suited for home use due to the high expense of commercially available equipment. This paper describes the development of a FES exercise bicycle for use at home. It consists of a standard bicycle ergometer with minimum modifications. The exerciser can be used by the patient sitting in the wheelchair, and may be used with any two-channel muscle stimulator.
