[The inhibitory effect of recombinant fibronectin polypeptide CH50 on the growth of B16 melanoma].

OBJECTIVE: To study the inhibitory effect of CH50, a recombinant polypeptide consisting of 2 binding domains of human fibronectin, on the growth of B16/F1 melanoma in mice. METHODS: Mice inoculated with B16/F1 cells were treated with CH50. The growth inhibitory effect on i.p. inoculated tumor was assessed by the number and size of tumor nodules in the peritoneal cavity, and on s.c. inoculated tumor by tumor size. RESULTS: For s.c. inoculated B16/F1, CH50 injected at the tumor site led to 50% inhibition of tumor growth while injection at a distant site was much less effective. In mice bearing i.p. inoculated tumor, the growth inhibition by CH50 on tumor nodules < 1 mm in diameter was 80% and 50+% on those > 1 mm. Synergistic therapeutic effect was obtained when CH50 used in combination with hydroxycamptothecine (HCPT). CONCLUSION: CH50 is effective in the treatment of B16/F1 melanoma. The effect can be enhanced when used in combination with HCPT.

[Expression and significance of urokinase-type plasminogen activator in breast cancer].

OBJECTIVE: To study the expression and clinical significance of urokinase-type plasminogen activator (uPA) in breast cancer. METHODS: Applying streptavidin-biotin complex (SABC) immunohistochemical technique, expression of uPA was studied in 100 patients with primary breast cancer. RESULTS: There were 55 patients with high uPA expression, and 45 with low expression. There was significant correlation between uPA expression and TNM stage, lymph node status, tumor size. Neither age, menopausal status, nor ER status was significantly related with level of uPA expression Patients with high expression of uPA had significantly shorter disease-free survival period and overall survival than did those with low expression of uPA. Univariate analysis showed that uPA as a prognostic factor was of similar magnitude to lymph node status and TNM stage, but stronger than that of ER status and tumor size. uPA was an independent prognostic factor affecting disease-free survival and overall survival. CONCLUSION: uPA appears to be a strong and independent biologic marker for predicting prognosis of breast cancer.

[Treatment of class II furcation involvement: guided tissue regeneration with atelocollagen membrane].

OBJECTIVE: To evaluate the effectiveness of guided tissue regeneration (GTR) on class II furcation involvement with absorbable atelocollagen membranes made in China. METHODS: GTR was performed on 26 sites with class II furcation defect in 17 patients. Clinical measurements and digital subtraction radiography were used to evaluate the effect. RESULTS: Clinical measurements showed significant attachment gain, significant decrease of probing depth (PD) and horizontal probing depth (HPD) at 3, 6, and 12 month after surgery. The mean reduction of HPD were 1.8 mm, 2.2 mm, 2.5 mm at 3 month (23 sites), 6 month (12 sites), and 12 month (6 sites) after surgery respectively. PD decreased by 1.22 mm, 1.56 mm and 1.75 mm. Attachment gained by 1.17 mm, 2.11 mm and 1.75 mm. Three sites showed complete defect fill during 3 to 12 months after surgery. Only 1 mm or less could be probed horizontally in another 4 furcation lesions during 3 to 12 months post-operation. The HPD of pre-operation in these 7 cases were 2.0 mm to 4.0 mm, and the bone defects observed during surgery were smaller in these cases than in other cases. The other 19 furcation involvement improved significantly. The result of digital subtraction radiography showed that increase of bone density was found at 2 months after surgery. The amount of bone gain increased with time. The areas of increased bone density were 0.3-25.7 mm2 in 13 teeth. CONCLUSION: The class II furcation involvement can be treated by GTR with absorbable atelocollagen membranes with good results.

[Limited pleuro-pneumonectomy for bronchogenic carcinoma associated with cancerous pleural effusion].

OBJECTIVE: To evaluate the effects of limited pleuro-pneumonectomy for bronchogenic carcinoma associated with cancerous pleural effusion. METHODS: From January. 1994 to December 1997 we performed limited pulmonary lobectomy wedge resection of lung, resection of the metastases of the parietal pleura, visceral pleura, diaphragmatic pleura and pericardiac pleura, and electro incision of the micormetastases of the above position as well as cleaning the mediastinal lymph nodes in 14 cases. RESULTS: The 14 cases were followed-up to December 1998. Eight patients of them died, the other 6 patients survived with a survival period of 7 - 30 months (median survival period 16.5 months). No pleural effusions recurrence was seen. The longest survival period was 30 months. There were no operative mortality and no severe postoperative complication. Chest distress, dyspnea and pain of chest or abdomen were relieved. CONCLUSIONS: This simple method has such advantages as, less traumatic, rapid rehabilitation and effective control of pleural effusion. The postoperative survival period is longer than that of palliative operation and medical therapy. The pain caused by parietal pleural metastasis can be relieved.

[Clinical protocols for the treatment of severe acute pancreatitis].

OBJECTIVE: To set up principles for the treatment of severe acute pancreatitis (SAP). METHODS: The therapeutic principles and results of SAP in three periods were analyzed. Period I (1980 - 1990) was characterized by early surgical intervention supplemented with integrated traditional and western medical therapy, period II (1991 - 1993) by integrated traditional and western medical therapy supplemented with surgery for early complications, and period III (1994 - 1997) by integrated traditional and western medical therapy supplemented with surgery for local infectious complications only. RESULTS: Because of the improvement of intensive care, the operation rates kept declining in the three periods. They were 77.59%, 54.55% and 29.55% with a mortality rate of 40.52%, 17.17% and 11.36%, respectively. CONCLUSIONS: Integrated traditional and western medicine has been effective in most SAP patients, and surgery is indicated only for late complications such as necrotizing infection. Early surgery should be restricted.

[Effects of reperfusion on neuronal changes and macrophagic response after transient focal ischemia--reperfusion of brain in rats].

Male SD rats(n = 29) were subjected to 2 hours' middle cerebral artery occlusion and were killed at various times of reperfusion (0.5-48 hours). The histological features of neuronal changes, the macrophagic response and the relationship between them were investigated. The neurons underwent acute changes(shrinking or swollen), and the pyknotic apoptotic neurons were occasionally seen within 3 hours. The number of apoptotic neurons increased with recirculation time. The scattered red neurons and ghost cells were first found at 6 hours of reperfusion. Many apoptotic neurons, apoptotic bodies were detected in the penumbra of ischemic core at 48 hours. Lectin histochemistry with GSI-B4 was performed to observe the macrophage. The results showed the active microglia and macrophage from meninges presented at 12 hours. The macrophages from parenchyma vessels appeared at 24 hours. Reactive microglia and extrinsic macrophage aggregated into the band of macrophage which phagocytized the apoptotic neurons. The results indicate that, firstly, it is important to prevent the neuronal apoptosis and reduce the volume of infarct in stroke patients; secondly, irreversible neuronal lesions simultaneously stimulate microglial activation and the presence of extrinsic macrophages from meninges was earlier than the appearance of the macrophages from parenchyma blood-monocytes. The authors also suggest that the data on the neuronal changes over time after ischemia and reperfusion be useful reference materials for selecting the most timely medication to treat cerebral infarction.

[Construction of full-length cDNA clone and complete nucleotide sequence of RNA2 of cucumber mosaic virus SD strain].

The full-length cDNA clone of CMV-SD RNA2 was constructed from two cDNA clones, which were obtained by RT-PCR with primers synthesized based on terminal sequences determined using 5' RACE and 3' RACE, respectively; and the complete sequence was determined. It consists of 3048 nt and there are two partial overlapped ORFs. ORF1, which is located in the 5' portion (79-2652 nt), codes for 2a protein of 858aa, and GDD consensus sequence reported to be "polymerase site" was found in deduced 2a protein sequence. ORF2, which is located in the 3' portion (2414-2746 nt), codes for 2b protein of 111aa. The overall nucleotide sequence homologies with RNA2 of CMV Fny and Q strain, which are representatives of subgroups I and II, respectively, are 91.7% and 75.6%; the amino acid sequence homologies are 93.8% and 67.7% for 2a protein, and 83.0% and 51.3% for 2b protein, respectively. These results demonstrated that CMV-SD belongs to subgroup I.

[Anatomic variations of the ostiomeatal complex and their correlation with chronic sinusitis: CT evaluation].

OBJECTIVE: To explore the relationship between anatomic variations in the ostiomeatal complex (OMC) and chronic sinusitis. METHODS: Coronal plane CT scans of the paranasal sinuses showing the ostiomeatal complex including the middle turbinate, uncinate, ethmoid bulla, agger nasi and Haller cell were analyzed in 297 individuals (594 sides). Statistical analyses were carried out with SPSS 5.0. RESULTS: There were 81.14% of OMC noted to have at least one variation. The anatomic variations included paradoxical curvature of the middle turbinate (13.97%), the pneumatized middle turbinate (34.85%), uncinate hyperplasia(19.36%), deviation of uncinate (45.27%), large ethmoidal bulla (30.30%), large agger nasi (0.70%) and Haller cell (1.00%). The correlation between variations and chronic sinusitis was significant (P < 0.01). CONCLUSION: The occurrence of OMC variation is common. The variations may be one of the causes of chronic sinusitis. Therefore, properly handling the variations was important in the endoscopic sinus surgery.
