[Hemorrhage of esophageal varices: conservative or emergency treatment?].

The emergency treatment of bleeding esophageal varices still remain a very difficult problem. In this report results obtained with conservative and operative procedures will be compared. In particular 32 patients underwent transhepatic occlusion of the gastric vessels feeding varices, and 8 patients (total experience 27 cases) to emergency esophageal transection with the EEA stapler instrument. The conclusion is that both methods represent useful alternatives, in selected cases, rather than being antagonists.

Kidney 15-hydroxy-prostaglandin-dehydrogenase activity during the development of experimental hypertension in the rat.

15-hydroxy-prostaglandin-dehydrogenase (PGDH) activity was studied in rat kidney homogenates during the development of hypertension, within 20 days after left renal artery constriction by a solid silver clip. In the ischemic kidney PGDH activity increased at day 6, reached maximum at day 10, then progressively at day 15 and returned to normal levels at day 20. No difference was found between contralateral kidneys and kidneys of normotensive control rats. Variations of PGDH activity did not seem to be related to either renal perfusion pressure or renin production. Increased PGDH activity may be a consequence of an enzyme induction following increased PG-synthetase activity, or it could be viewed as a defence mechanism, according to the hypothesis of a prohypertensive effect of PG in the rat.

[Splenoportography: still important technic. New prospects (author's transl)].

Splenoportography still plays today an important role among angiographic examinations used in the study of portal hypertension. The authors arrived at these conclusions after studying the results obtained in more than 500 examinations carried out. In the text, they give a few details of the technic used to improve the photographic result and to reduce the risk of the examination.

Effect of cimetidine on steroid experimental peptic ulcers.

Cimetidine, a H2-receptor inhibitor was able to prevent the development of gastric ulcers induced in rats by chronic administration of prednisolone and hydrocortisone associated to fasting. The results suggest that cimetidine may be used in patients under long term steroid treatment.

[Present status of breast cancer surgery in Italy (author's transl)].

Radical mastectomy is the most frequent surgical procedure in breast surgery in Italy. It is indicated in all stages associated or not to complementary therapies. Extended radical mastectomies are performed less frequently than previously. In initial stages of the disease partial mastectomies are performed more frequently. Generally, castration is performed as therapeutic means, not any more for prophylactic reasons. Adrenalectomy and hypophysectomy are seldom associated procedures. As postoperative treatment, Roentgen-Cobalt-Betatrone therapy is still widely used. However, chemotherapy (CMF) is progressively replacing radiation therapy, mostly in the advanced stages.

Urinary and kidney kallikrein in hypertensive rats.

Urinary and kidney kallikrein were studied in rats with renal clip hypertension. The effect of protease inhibitors on urinary and kidney BAEE esterase activity was similar. Both hypertensive and not hypertensive operated rats excrete significantly less kallikrein than controls; in the ischaemic kidney kallikrein is diminished whereas is not increased in contralateral. Kallikrein is therefore related to renal functional mass but does not seem responsible for a natriuretic effect.
