Sorbents used for removal of urea in the system of artificial kidney.

Three copolymers were made from the suspension polymerization of methyl acrylate, divinyl benzene and triallyl isocyanurate; acrylamide and N,N'-methylene-bis-acrylamide; and styrene and divinyl benzene. These copolymers were then converted into derivatives of hydrazides through several steps. They were used as polymeric sorbents. They sorbed in situ a little bit less than 40 mg. of urea per gram of sorbent at 37 degrees C, pH = 7.0-7.4 and the percentage of urea in the buffer solution being 130 mg%. The sorption of urea was related to the specific surface area of the sorbent, temperature of sorption and time of sorption, concentration of urea, pH value of solution etc. In order to find out whether the above-prepared derivatives of hydrazide can be used clinically as polymeric sorbents in artificial kidney system, the junior author carried out experiments with patient's dialysate as imitated to hemodialysis. From the results, it indicates the sorbents might be used in artificial kidney system because they sorbed the urea from hemodialysate as much as that of the pure urea in a buffer solution.

Optic disc edema and Lyme disease.

A seven-year-old child developed a febrile rash, migratory arthritis, and neurologic signs. Three days earlier, the child had been bitten by a tick. An eye examination two months later revealed bilateral papilledema, although the cerebrospinal fluid pressure was 170 mm. The IgM antibody titer for the Ixodes dammini spirochete was 1:256. I dammini is the tick vector for the Borrelia spirochete that causes Lyme disease. The papilledema receded with treatment over a month's time, and bilateral pigment epithelial mottling at the fovea was present after one year.

Spontaneous remission of nephrotic syndrome in IgA glomerular disease.

We report two cases of adult onset nephrotic syndrome with pathologic features of minimal change disease but a predominance of IgA in mesangial regions. Both cases underwent spontaneous remission despite an episode of recurrent nephrotic syndrome in one patient. The relationship of the glomerular disease to minimal change and IgA nephritis is discussed.

A carrier state of X-linked juvenile retinoschisis.

The carrier state of X-linked juvenile retinoschisis has not been known to be associated with any fundus abnormalities. We report a case where there are funduscopic changes and mild visual acuity changes in one eye of a carrier.

The reduction in the peritonitis rate among high-risk CAPD patients with the use of the Oreopoulos-Zellerman connector.

We have described a prospective controlled study of the Oreopoulos-Zellerman peritoneal dialysis connection system which is based on the principle of chemical sterilization of the connector between each dialysis exchange. This system, in a short-term study of 27 high-risk patients, has resulted in a fourfold difference in the peritonitis rate between the concurrent control group (1/8.8 patient months) and the O-Z group (1/35.7 patient months). The probability of developing peritonitis by 6 mos was 0.15 using the O-Z connector and 0.46 in the control group.

The efficacy and adequacy of continuous ambulatory peritoneal dialysis.

Since it was introduced in 1976, continuous ambulatory peritoneal dialysis (CAPD) has won acceptance in many centres and it is now regarded as an important alternative to haemodialysis. CAPD patients have comparable and, in some circumstances, better survival than those on chronic haemodialysis. It is indicated particularly in patients with diabetes mellitus, cardiovascular instability and at the extremes of life. The success of kidney transplantation is similar in those maintained on CAPD and on haemodialysis. CAPD also achieves satisfactory physical and psychological rehabilitation, and the quality of life, including the level of sexual function, is similar during CAPD and haemodialysis. Women on CAPD menstruate more often than those on haemodialysis. CAPD provides adequate clearance of metabolic wastes, maintains fluid balance and ameliorates neurotoxic cognitive dysfunction. CAPD gives control of hypertension and anaemia which is superior to that on haemodialysis. Neuropathy remains stable but osteitis fibrosa seems to progress. CAPD is the most economical of the various forms of dialysis. We conclude that CAPD is an adequate form of replacement and should be made available in every nephrology centre providing treatment for patients with end-stage renal disease.

Continuous ambulatory peritoneal dialysis: no longer experimental.

Many patients with end-stage renal disease have now been maintained for 5 years or more with continuous ambulatory peritoneal dialysis (CAPD). Viewed initially as an experimental alternative to be used only when hemodialysis was not feasible, CAPD is now seen as the treatment of choice in an increasing number of situations. CAPD is suitable for self-care. The main concern in the early years--peritonitis--is now less frightening and less frequent (one episode occurring every 18 patient-months as compared with every 8 initially), and this has allowed chronic complications of CAPD, such as malnutrition and loss of the peritoneum's capacity for ultrafiltration, to come to light. As would be expected, among patients of advanced age and those who have heart disease or diabetes, survival rates tend to be lower than among other CAPD patients. However, hypertension seems to be more easily controlled, pre-existing anemia can be significantly ameliorated, and young children grow more normally than they do with hemodialysis. Diabetes-related changes in vision stabilize in most CAPD patients, and control of the blood glucose level is good; insulin is administered intraperitoneally. CAPD is thus showing itself to be a feasible form of long-term treatment for end-stage renal disease.

Electron microscopic study of the peritoneal kinetics of iron dextran during peritoneal dialysis in the rabbit.

Iron dextran, an electron-dense tracer, was given intravenously (100 mg of iron/100 g of body weight) to 8 normal rabbits to study its movement from the plasma to the peritoneal cavity during peritoneal dialysis. The dialysate infused at 75 ml/kg contained 1.5 g/dl of glucose in 4 animals and 4.25 g/dl in the remainder. Peritoneal dialysis was discontinued and the peritoneum was fixed in vivo at various times (20-120 min) after the injection of the iron dextran. Large amounts of tracer were detected in the effluent after draining the peritoneal cavity. Electron microscopic examination of the mesentery showed particles of iron dextran in the endothelial cells of small vessels (capillaries and venules) the interstitium and the mesothelial cells adjacent to vessels irrespective of the dialysate concentration or duration of dialysis. Tracer was not clearly demonstrated in the interendothelial or intermesothelial spaces. In the mesothelial cells, the particles were found exclusively in (small, elongated or large) vesicles, while in the endothelial cells they were both in vesicles and free in the cytoplasm. Our findings suggest that during the 20- to 120-min period after intravenous administration, the intracellular transport of iron dextran, depends on either moving vesicles or the presence of pre-existing tunnels in the mesothelial cells of rabbit mesentery.

Perichondrial arthroplasty in the hand: a case report.

A two-stage arthroplasty technique was used to treat an isolated fibrous ankylosis that developed in the index metacarpophalangeal joint of a 14-year-old boy after a rattlesnake bite. The first stage consisted of a minimal joint resection combined with rib perichondrial grafts to resurface the joint. The second stage, performed 5 months later, consisted of a capsulotomy and removal of a silicone membrane left within the joint to separate the grafts. Biopsy of the joint surfaces revealed the presence of hyaline-like cartilage. Nearly normal pain-free motion was restored. Follow-up now exceeds 30 months.
