Nineteen cases of plague in Arizona. A spectrum including ecthyma gangrenosum due to plague and plague in pregnancy.

We review the cases of 19 successfully treated plague patients, with emphasis on the clinical and epidemiologic features of the disease. Proper staining and culturing of bubo aspirates; prompt institution of streptomycin, chloramphenicol or tetracycline therapy in presumptive cases, and supportive care are the crucial factors in the treatment of plague. This disease should be considered in patients in a toxic condition who have lymphadenitis, pneumonia or septic shock and who have been in endemic areas within the past ten days.

Central retinal artery occlusion following transfemoral cerebral angiography.

Whereas most ocular complications following cerebral angiography are benign and transient, central retinal artery occlusion following cerebral angiography produces severe permanent visual deficits in nearly 40% of patients. Physicians ordering and performing cerebral angiography should be aware of this complication, since immediate attention to patients with visual disturbance from central retinal artery occlusion may save useful vision. All previously reported cases of central retinal artery occlusion following cerebral angiography have occurred after direct percutaneous carotid angiography. We report a case of central retinal artery occlusion following transfemoral cerebral angiography using a #5 French catheter and meglumine iothalamate (Conray-60) contrast.

[Diagnosis and surgery of an insulinoma using a glucose-controlled insulin infusion system].

A 71-year-old woman with insulinoma was studied. Preoperatively, using the glucose controlled insulin infusion system (GCIIS) for glucose clamping at various blood glucose levels, autonomous insulin production was demonstrated and intravenous glucose needs for maintenance of normoglycaemia were evaluated. The results of a somatostatin suppression test, guided by the GCIIS, supported the postulation of a well differentiated beta cell adenoma with reduced storage capacity. These assumptions were later confirmed by histochemical and ultrastructural investigations. Hypoglycaemia during surgery was avoided by means of the GCIIS. Upon clamping of the plasma glucose at 90 mg/dl, 15.5 g dextrose had to be given until resection of the tumour. Immediately thereafter, a sharp rise in plasma glucose to 140 mg/dl together with a need for 4.1 U insulin showed that the insulinoma tissue had been removed completely.

Splenic autotransplant and residual partial spleen: prevention of septicemia.

Postsplenectomy septicemia carries an ominous prognosis. Accompanying disseminated intravascular coagulation and adrenal hemorrhage result in a high mortality, despite aggressive treatment by antibiotics. The efficacy of prevention by in situ partial spleen and splenic auto-transplant were evaluated in Sprague-Dawley rats. All totally splenectomized rats died following intravenous challenge of live pneumococcus. Both partial spleens and autotransplants gave substantial protection. The rats which succumbed to pneumococcal sepsis demonstrated massive fibrin thrombi in renal glomeruli and frank adrenal hemorrhage, strikingly similar to clinical observations.

Interactions of caffeine and phenobarbital on sister chromatid exchanges in rat liver epithelial cell lines treated with 2-acetyl-aminofluorene and N-acetoxy-2-acetylaminofluorene.

Using "Fluorescence plus Giemsa" technique, sister chromatid exchanges (SCE) were determined on second-division metaphases in rat liver epithelial cell lines treated with 2-acetylaminofluorene (2-AAF), the procarcinogen, and N-acetoxy-2-AAF (N-OAc-2AAF), an ultimate carcinogen analog. The SCE frequency was found decreased after some conditions of 2-AAF treatment and increased with N-OAc-2-AAF. Phenobarbital (PB) decreased also the SCE frequency but cancelled the 2-AAF action when incubated after the procarcinogen. The addition of caffeine (Caf) cancelled the action of 2-AAF but not of phenobarbital. It is suggested that the mechanism of SCE may have several origins.

Poisonous plants contaminating edible ones and toxic substances in plant foods. Part 3. Pyrrolizidine alkaloids from Heliotropium digynum Forssk. (= H. luteum, Poir.).

Investigation of the alkaloidal constituents of Heliotropium digynum resulted in the isolation of four alkaloids viz. heliotrine, europine, lasiocarpine and 7-angelylheliotrine. Moreover, HPLC and GLC showed the probable presence of heliotridine and some other unidentified minor constituents. A summary of the pharmacotoxicity and biological activity of the ethanolic extract, total alkaloids with special reference to heliotrine is presented.

Diuretic radionuclide renography in upper urinary tract obstruction.

Diuretic renography using radioidine sodium hippurate and a frusemide stimulus was used to differentiate between obstructive and nonobstructive dilatation of the urinary tract. Information was recorded by a gamma camera on line to a computer. Sixty-seven uretero-renal units (URU) were studied in 33 patients with an average age of 91/2 years. The diuretic renogram correctly indicated obstructions in 18 URU but gave false positive results in another five. Of these five, four had severely impaired function. The renogram indicated nonobstruction in 49 URU, and gave false negative results in another two; of these two one patient had a duplex kidney with a poorly functioning ureteric segment and the other patient had gross reflux and megaureters. The diuretic renogram is useful in the investigation of equivocal upper urinary tract obstruction but may be unreliable in situations of inadequate renal function.

Aflatoxin M1 in manufactured dairy products produced in the United States in 1979.

In a 1979 survey of manufactured dairy products (992 samples of nonfat dry milk, vanilla ice cream, yogurt, Cheddar cheese, and cottage cheese) for aflatoxin M1 contamination, one sample, a cottage cheese, had detectable aflatoxin equivalent to .08 ng/ml in the milk from which the product was made. Samples were taken by Food and Drug District inspectors from randomly selected establishments at three times throughout the year. The distribution of sample quotas to each District was weighted to double the representation of establishments in the southern tier of states. The conclusion from this survey is that in a "normal" year aflatoxin M1 should not be in a manufactured dairy product in the United States at a level in excess of that from milk with .1 ng aflatoxin M1/ml.

Patient characteristics and clinician attitudes influencing the prescribing of benzodiazepines.

A questionnaire designed to evaluate the effect of patient characteristics and clinician attitudes on the prescribing of benzodiazepines was distributed to 86 members of the Departments of Psychiatry and Family Practice; the return rate was 87%. Of the respondents, 30% were consistently for, 20% were consistently against, and 50% were neither consistently for nor against the use of benzodiazepines. Prescribing decisions apparently depended on a complex interaction of patient's sex, employment, symptom severity and chronicity, and the respondent's attitude about the drugs. Plans are under way to revise the questionnaire, undertake in depth interviews, and compare questionnaire data with actual prescribing practices.

Interaction of plasma fibronectin with selected cryoglobulins.

Plasma fibronectin (PFN) is associated with cryoglobulins of the polyclonal type. Cryoglobulins from two patients with essential mixed cryoglobulinaemia were analysed immunoelectrophoretically to determine a possible association between PFN and immunoglobulin complexes in the formation of cryoprecipitates. In cryoglobulins from each patient, distinctive alterations in electrophoretic migration patterns of both PFN and immunoglobulin were associated with the formation of a cryoprecipitate. The observations indicate that PFN binds to some serum component in the process of cryoglobulin formation. That component may be immunoglobulin.
