Renal transplantation in 22 children with nephropathic cystinosis.

In 1989, 22 children (11 boys, 11 girls aged 8-23 years) with nephropathic cystinosis, who had received a total of 28 renal allografts over the previous 14 years, were reviewed. Nineteen were alive, of whom 17 had functioning grafts 5 months to 13 years after transplantation. The mean serum creatinine level in these 17 was 135 mumol/l. Patient and graft survival did not differ from non-cystinotic children. Persistent hypothyroidism was found in 3 patients, transient diabetes mellitus in 1, severely disturbed vision in 1 and brain atrophy in 11. Arterial hypertension was present in 16 patients. Growth retardation was universal, although in 4 patients on cyclosporin A post-transplant catch-up growth occurred. Five patients over 15 years completed puberty. Readjustment in terms of school performance was good but was less good for psychosocial development. None of the patients had ever been treated with cystine-depleting agents; the data will therefore provide a historical control group with which to compare the results from a group treated with these agents.

[Genetic counseling in prenatal diagnosis of cystic fibrosis].

In this paper we discuss the possible results of post- and prenatal DNA-diagnosis in cystic fibrosis with respect to the modification of the risk of having an affected child, depending on how closely the affected patient is related to those seeking advice. For parents with an affected child post- and prenatal DNA-diagnosis yields very reliable results and is an important factor in their decision process. In contrast to this, DNA-diagnosis for other relatives or individuals without an affected family member can lead to results which may intensify conflicts of decision making. This stresses the growing importance of genetic counseling before and after genetic diagnostic measures for cystic fibrosis.

The effect of non-ionic surfactants on the SOS-inducing potency of 4-nitroquinoline-1-oxide in Escherichia coli PQ37.

Various non-ionic surfactants affect the SOS-inducing potency (SOSIP) of the model genotoxin, 4-nitroquinoline-1-oxide, in Escherichia coli PQ37 to varying degrees, as measured by an automated version of the SOS chromotest. While there is little effect on the SOSIP value and other test parameters from Tween 20 and 80 and, with some reservation, Triton X305 and Tyloxapol, over the critical micelle concentration range, the SOSIP value increases in the presence of comparable concentrations of Triton X15, 45 and 100. A possible dependence of the tester strain's beta-galactosidase production and its growth inhibition on the HLB of the non-ionic surfactants added is discussed.

The structural analysis of the mitochondrial SSUrRNA implies a close phylogenetic relationship between mitochondria from plants and from the heterotrophic alga Prototheca wickerhamii.

The gene for the mitochondrial small subunit rRNA (SSUrRNA) from the heterotrophic alga Prototheca wickerhamii has been isolated from a gene library of extranuclear DNA. Sequence and structural analyses allow the determination of a secondary structure model for this rRNA. In addition, several sequence motifs are present which are typically found in SSUrRNAs of various mitochondrial origins. Unexpectedly, the Prototheca RNA sequence has more features in common with mitochondrial SSUrRNAs from plants than with that from the green alga Chlamydomonas reinhardtii. The phylogenetic relationship between mitochondria from plants and algae is discussed.

Down-regulation of androgen receptor by progestins and interference with estrogenic or androgenic stimulation of mammary carcinoma cell growth.

The regulatory influence of medroxyprogesterone acetate (MPA) on estrogen and androgen receptors of the human breast cancer cell lines MCF-7 and EFM-19 was explored in conjunction with the growth-promoting properties of these steroids. In the absence of steroidal stimulation, up to 1 microM MPA had no effect on the proliferation of the MCF-7 cell strain used and of EFM-19 cells. Under stimulation with 10 nM 17 beta-estradiol or 1 microM dihydrotestosterone, dose-dependent inhibition of the cell proliferation rates by 0.1-1 microM MPA was observed. Binding of MPA to the androgen receptor (Kd = 2.1 nM) but not to the estrogen receptor was demonstrable. During incubation of MCF-7 or EFM-19 cells with 1 microM MPA for 7 days, the estrogen and androgen receptor contents were down-regulated by approximately 50% and 60%, respectively. Likewise, the number of androgen-binding sites was reduced to 35% of the untreated controls after incubation of MCF-7 cells with 1 microM synthetic progestin R5020 for 7 days. The results indicate down-regulation of estrogen and androgen receptors by progestins in the absence of stimulatory effects on the proliferation of mammary carcinoma cells.

Neurofibromatosis 2: a clinically and genetically heterogeneous disease? Report on 10 sporadic cases.

Clinical and genetic data of 10 patients with neurofibromatosis 2 (NF-2) are presented. Interestingly, no family history of neurofibromatosis was detectable in any of them, which indicates that these are sporadic cases of NF-2, most likely due to a new mutational event. According to our own results and the data in the literature, sporadic cases of NF-2 are clinically characterized by a high incidence of multiple meningiomas and spinal tumors in addition to the bilateral occurrence of acoustic neurinomas. The clinical heterogeneity of NF-2 is pointed out and the possible existence of different forms of this disease is discussed.

Application of an alternative approach for using the SOS chromotest to screen complex indeterminate mixtures.

We report on the application of an alternative approach to screen two complex indeterminate mixtures for genotoxic activity with the SOS chromotest, a simple quantitative colorimetric short-term bacterial genotoxicity assay. Considering the mixtures as a complex matrix, we evaluated the shape of the dose response curve of the direct acting reference genotoxin 4-nitroquinoline-1-oxide (4-NQO) with and without the matrix and found for one mixture a previously unrecognized genotoxic potential.

[Spontaneous course of chronic gastritis in corpus and antrum--is there a regression?].

In 148 patients (medium of 59, 1 years) we investigated the natural history of chronic gastritis within 1-7 years (average of 5.9 years). We performed two biopsies, but in 35 cases 3 or 4 biopsies. In most patients the histological picture was not changed. In superficial gastritis of the body we observed a regression more often than a progression. Regression of atrophic gastritis was see in body and antrum. The rate of regression was higher in cases with 3 or 4 biopsies and with a longer interval of observation. The frequency of regression was higher than awaited according to simultaneous biopsies (criticism of method). There was no difference of rate of regression in various age groups and in various diagnosis (gastric or duodenal ulcer). In gastritis Type B the regression was more seldom than in gastritis Type a or type AB. For spontaneous regression no cause was found.
