Therapy of Aujeszky's disease (pseudorabies) in naturally infected and artificially inoculated piglets using BW B759U (9-[1,3-dihydroxy-2-propoxymethyl] guanine).

Two experimental porcine models of Aujeszky's disease (AD) were compared for assessing the efficacy of potential antiviral compounds. While signs were the same following intranasal inoculation and in-contact transmission of the causal herpesvirus, SHV-1 (Suid herpesvirus), the time-course of the disease was different. There was less variation in clinical signs between pigs following artificial infection, but the disease was more severe, making this a consistent but also more stringent test system. A nucleoside analogue, BW B759U (9-[1,3-dihydroxy-2-propoxy-methyl] guanine), was administered intramuscularly in divided twice daily doses at 50 mg kg-1 to three-week-old piglets in these two SHV-1 disease models. In artificially infected animals, in which treatment was begun 1.5 hours preinfection and continued for six days, there was a delay in the onset of clinical signs (4.8 compared with 3.2 days), a 1 log10 reduction in virus shedding, and a 1 to 2 log10 reduction in virus recovered from tissues, but all the treated piglets died from AD. In contrast, none of the BW B759U-treated piglets in the in-contact model system died during the eight-day medication period, although two piglets died subsequently. Mean serum concentrations of BW B759U two hours after intramuscular dosing were about 45 microM, at nine hours 15 to 20 microM and at 17 hours 3.5 microM. The in vitro IC50 of BW B759U against SHV-1 varies from 1.5 to 80 microM depending on the cell line in which the assay is carried out. There were no overt signs of BW B759U toxicity in the treated piglets.

Prognostic significance of oestrogen and progestogen receptor activities in breast cancer.

The prognostic significance of the tumour activities of 2 steroid receptors, those for oestrogen (ER) and for progestogen (PgR), has been studied in 372 patients with breast cancer, in whom follow-up was available for 2-6 years (median 41 months). Of 252 patients with operable disease, 75.8 per cent had ER-positive tumours and 46.4 per cent had PgR-positive tumours, though a small additional fraction (6.3 per cent) had an equivocal PgR assay result. For the 236 patients with unequivocal receptor status, the relationships between disease-free interval or overall survival and receptor activity and other factors were evaluated by univariate and multivariate analyses. The latter revealed that only tumour size, node status, menstrual status and ER status related significantly to both disease-free interval and survival, though adjuvant therapy also related to disease-free interval, and tumour grade related to survival. It is concluded that measurements of PgR activity do not add to the prognostic significance of ER status.

Regulation of sterol synthesis and of 3-hydroxy-3-methylglutaryl coenzyme A reductase by lipoproteins in glial cells in primary culture.

Although plasma lipoproteins have been demonstrated to have a major role in regulating cholesterol biosynthesis in extraneural cells, no data concerning such regulation are available for developing brain, when cholesterol synthesis is especially active. Glial primary cultures derived from neonatal rat brain and by morphological and biochemical criteria essentially exclusively composed of astrocytes were utilized to examine such regulation. When the primary cultures, which had been maintained in 10% fetal calf serum, were placed in 10% lipoprotein-poor serum on day 7 of culture, an induction of sterol synthesis (1.6-2.2-fold) and of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase-specific activity (1.5-2-fold) resulted after 24 hr. Addition of low-density lipoprotein (LDL) to the 10% lipoprotein-poor serum prevented the induction of both sterol synthesis and HMG-CoA reductase. However, addition of high-density lipoprotein (HDL) to the 10% lipoprotein-poor serum caused a 1.5-2-fold further induction of sterol synthesis relative to that in cultures containing 10% lipoprotein-poor serum alone. In contrast to the glial primary cultures, cultures of C-6 glioma cells responded to replacement of 10% fetal calf serum with 10% lipoprotein-poor serum with much more marked increases of sterol synthesis and HMG-CoA reductase. Although, as with the primary cultures, addition of LDL to the C-6 glioma cell cultures prevented the increases in sterol synthesis and reductase activity, addition of HDL had no effect. Thus, these results indicate that in developing glial cells in primary culture, cholesterol synthesis and HMG-CoA reductase are capable of responsiveness to both LDL and HDL.(ABSTRACT TRUNCATED AT 250 WORDS)

Large vessel sealing with the argon laser.

This study compared the histology, biochemistry, and tensile strength of laser-welded and sutured canine venotomies, arteriotomies, and arteriovenous fistulas. Twelve animals had bilateral femoral vessels studied, with one repair (control) closed with interrupted 6-0 polypropylene sutures, and the contralateral repair (experimental) welded with the argon laser. Specimens were examined at weekly intervals from 1 to 4 weeks (four animals for each type of repair), and were evaluated histologically by hematoxylin and eosin, elastin, and trichrome stains; biochemically by the formation of [3H]hydroxyproline as an index of collagen synthesis; and mechanically by tensile strength determinations. At removal, all experimental closures were patent without hematomas, aneurysms, or luminal dilatation. Histologic and biochemical examination and tensile strength determinations suggest that laser welding may be an alternative to sutures for repair of large-diameter venotomies, arteriotomies, and arteriovenous fistulas, as healing is comparable to that seen with suture repairs up to 4 weeks postoperatively.

Valvulotomy in in situ vein bypasses performed by angioscope-assisted laser probe.

The objective of this study was to evaluate the use of an angioscope-assisted laser hot-tip probe for performing valvulotomies in in situ nonreversed canine veins used for arterial bypasses. In Group 1, 9 valvulotomies were performed in 7 bypass veins with a 2-mm probe activated by 12 W power, resulting in 4 vein perforations, 1 vein thrombosis, and 4 in situ grafts patent at removal after 2-28 days. In Group 2, 10 valvulotomies in 9 bypass veins were performed with a 1.5-mm probe at 14 W with no vein perforations, 3 vein thromboses, and 6 in situ grafts patent at removal after an interval of 1 to 6 weeks. We conclude that valvulotomy within in situ vein bypasses can be successfully performed using a 1.5-mm laser hot-tip probe at 14 W power, with patency of the grafts demonstrated at 6 weeks. Angioscope-assisted laser valvulotomy using a laser probe may significantly reduce the length of incisions and the time required for in situ vein bypasses and improve the technical accuracy of the procedure.

Laser vascular welding--how does it work?

This study evaluated the histology and electron microscopy of four samples of 2 cm long venotomies and artery-vein anastomoses formed in canine femoral arteries and veins using the argon laser (0.5 W power, 1 800 J/cm2, 4 min exposure/1 cm length of anastomosis). Welds were continuously irrigated with saline during the procedure to limit maximal temperatures to 44.2 +/- 1.6 degrees C (mean +/- SD), and the specimens were removed immediately following fusion and preserved for examination. Histologic and electron microscopic examination of different areas of the welds revealed various mechanisms of fusion including a) apposition of denatured collagen and elastin in the media and adventitia; b) bonding of vein medial collagen and elastin to the internal elastic membrane of the artery; and c) fusion consisting of a coagulum of platelets and fibrin depending on the alignment and apposition of the vessel edges. This study demonstrates that vascular tissue fusion by the argon laser occurs by various mechanisms. Future experiments should delineate which types of seal produce the optimal strength at the time of fusion, and enhance long-term healing.

Aortitis presenting as Buerger's disease.

Buerger's disease is frequently diagnosed in young smokers, based on clinical presentation rather than histologic criteria. This report describes a young male with idiopathic aortitis who had been managed for 10 years as Buerger's disease solely on the basis of clinical manifestations. This case emphasizes the need for establishing histopathologic, and perhaps biochemical, criteria to identify the underlying arteriopathy.

Mammary cancer chemoprevention by inorganic and organic selenium: single agent treatment or in combination with vitamin E and their effects on in vitro immune functions.

The chemopreventive efficacies of selenate, selenite, selenium dioxide, selenomethionine and selenocystine were examined during the promotion phase of carcinogenesis in the 7,12-dimethylbenz[a]anthracene-induced mammary tumor model in rats. Each agent was added to the diet at a final concentration of 3 p.p.m. selenium. In general there was no significant difference in the potency of these five selenium compounds in inhibiting the development of mammary tumors. The interaction of vitamin E (500 p.p.m.) with either selenite or selenomethionine was further characterized in a second carcinogenesis study. Results of this experiment suggested that vitamin E enhanced the protective effect of selenite but not that of selenomethionine. In an attempt to explore the synergistic mechanism of selenium and vitamin E, the effects of these two agents on mitogen-induced blastogenesis and natural killer cytotoxic activity were also investigated. No consistent changes in these in vitro immune functions were detected resulting from supranutritional feeding of either selenite or vitamin E or both. The metabolism of inorganic versus organic selenium was discussed in relation to their role in the control of neoplastic growth as well as to their selective modulation by vitamin E.

Localization of occult breast lesions: practical solutions to problems of triangulation.

The preoperative triangulation and localization of some occult breast lesions can be complicated if the lesion is located deep in the breast. Based on the authors' experience of 1,400 localization procedures, standard breast positions were modified to help locate lesions that were difficult to see in two projections. Standard compression plates were also modified, allowing placement of fenestrations over deep lesions--especially those in the axillary tail of the breast--to facilitate safe, accurate localization.

Mitoxantrone versus doxorubicin in combination chemotherapy for advanced carcinoma of the breast.

One hundred fifteen patients with metastatic carcinoma of the breast were treated in a randomized trial of mitoxantrone (Novantrone, Lederle Laboratories, Pearl River, NY) combined with vincristine and prednisolone (VMP) or doxorubicin (Adriamycin, Adria Laboratories, Columbus, OH) combined with vincristine and prednisolone (VAP). In 100 evaluable patients, the objective response rates were 35% for VMP and 61% for VAP, the complete response rates being 6% and 13%, respectively. In responding patients, median time to progression was 6.2 months for VMP and 7.9 months for VAP. The median survival whether measured from primary diagnosis, first metastasis, or from the start of chemotherapy was similar for both regimens. Toxicity, particularly alopecia, was appreciably lower in the VMP treated patients, but subclinical cardiotoxicity was seen within the scheduled dosage for both combinations. We conclude that VAP is clearly more active, but clinically more toxic than VMP. There is no survival advantage conferred by the more toxic combination. Cardiac toxicity is a potential hazard with either drug combination.

Mammographic density and physical assessment of the breast.

We studied the relationship between the radiographic density of the breast as measured by parenchymal pattern vs age, breast size, thickness after compression, and compressibility. Two hundred consecutive women were evaluated. None of the three parameters or the patient's age correlated with the degree of radiographic density. Although more older women had lucent parenchyma, 37% of women more than 50 years of age had dense patterns. Thus, these factors cannot be used to predict radiographic density. Phototiming is required to maintain image quality, and only a test exposure can accurately predict breast density.

Localization of breast lesions identified on only one mammographic view.

Occasionally, a breast lesion is visible in only one mammographic projection. A simplified method of using parallax in a dedicated mammographic system has been devised to locate accurately lesions that are not visible in two orthogonal projections. In eight patients, a parallax approach from essentially a single mammographic position was used to place a hookwire preoperatively at nonpalpable suspicious abnormalities detected by mammography. Needles were inserted parallel to the chest wall while the breast was compressed in the mammographic unit. In each case, the tissue containing the lesion was pierced by a needle chosen to be longer than the distance from the skin to the abnormality. Simple geometric ratios were used to determine the position of the lesion along the needle shaft after slightly changing the mammographic projection and viewing the needle obliquely. In all cases, this parallax approach permitted accurate localization of the lesion.

The halo sign and malignant breast lesions.

The halo sign, described as a complete or partial radiolucent ring surrounding the periphery of a breast mass, has long been considered a mammographic sign indicating a benign process. The phenomenon is most frequently seen with cysts and fibroadenomas. Over the past 8 years, 25 malignant breast lesions with a partial or complete halo on mammography have been identified in 19 women. These cases suggest that a halo sign does not guarantee a benign process.

Argon laser-welded arteriovenous anastomoses.

This study compared the healing of laser-welded and sutured canine femoral arteriovenous anastomoses. Arteriovenous fistulas 2 cm in length were created bilaterally in the femoral vessels of 10 dogs and were studied at 1 (n = 2), 2 (n = 2), 4 (n = 3), and 8 (n = 3) weeks. In each animal, one anastomosis (control) was closed with running 6-0 polypropylene sutures, and the contralateral anastomosis (experimental) was sealed with an argon laser (0.5 watt, 4 minutes of exposure, 1830 J/cm2/1 cm length of anastomosis). At removal all experimental anastomoses were patent without hematomas, aneurysms, or luminal narrowing. Histologic examination at 4 weeks revealed that laser-welded anastomoses had less inflammatory response and almost normal collagen and elastin reorientation. At 8 weeks sutured anastomoses had significant intimal hyperplasia whereas laser repairs had normal luminal architecture. Tensile strength and collagen production, measured by the synthesis of hydroxyproline and the steady-state levels of type I and type III procollagen messenger ribonucleic acids, at the anastomoses and in adjacent vein and artery specimens were similar in sutured and laser-welded repairs at 2, 4, and 8 weeks. We conclude that argon laser welding of anastomoses is an acceptable alternative to suture techniques, with the advantage of improved healing without foreign body response and possible diminished intimal hyperplasia at the anastomotic line.

BCG-modulated mammary carcinogenesis is dependent on the schedule of immunization but is not affected by dietary fat.

The present study was designed to study the effect of dietary fat intake on the modulation of dimethylbenz[a]anthracene (DMBA)-induced mammary carcinogenesis in rats injected with the methanol extract residue of Bacillus Calmette-Guerin (MER-BCG). Rats were maintained on either a 5% or a 20% corn oil diet for the entire duration of the experiment. When MER-BCG was administered 2 and 3 weeks before DMBA, mammary tumorigenesis was suppressed in the 2 dietary groups with different levels of fat intake. This was in contrast to when MER-BCG was administered 3 and 5 weeks after DMBA; in this case the development of mammary tumors was noticeably enhanced regardless of the fat intake of the host. The magnitude of inhibition or increase by MER-BCG was similar in animals fed either fat level, although a high fat diet consistently stimulated mammary tumorigenesis in the 2 experiments. In vitro assays on T cell mitogen-induced blastogenesis and natural killer cell activity in splenocytes isolated from the untreated rats showed that dietary fat failed to elicit any differential response in these immune functions.

Health education in paediatric ENT.

A multiple-choice questionnaire was completed by 100 parents of children attending ear, nose and throat out-patients to assess their ENT knowledge. This highlighted areas of poor understanding, and was followed by the production of information leaflets and a pictorial display.

Male adolescent sexual offenders: the offender and the offense.

A group of male adolescent sexual offenders were divided into three groups: Courtship Disorders (Exhibitionism, Toucherism and Obscene Phone Calls), Sexual Assaults, and Pedophilic Offenses. Group I offenders tended to come from a less disorganized family background, were better adjusted to school and in the community and were seen by clinicians as less seriously disturbed than the adolescents in the other two groups. In addition, these adolescents did not experience the offense as a sexual act. Group II offenders (Sexual Assaulters) came from a more disturbed family background characterized by a high rate of long-term parent-child separations, committed more violent offenses and had a higher frequency of intellectual functioning in the Borderline Range of Intelligence. Group III offenders (Pedophilic Offenses) were Canadian born, had witnessed physical violence between their parents, were described as having been infants who did not enjoy being cuddled and had siblings who were truant. This study suggests that classifying adolescent sexual offenders along the line suggested in adult literature seems to be justified.
