Treatment of Paget's disease with intermittent low-dose infusions of disodium pamidronate (APD).

Disodium pamidronate (APD) is a potent inhibitor of bone resorption, with less risk of defective mineralization than earlier bisphosphonates. We assessed the response to six spaced low-dose intravenous infusions of APD given at intervals of approximately 6 weeks followed by weekly infusions if bone turnover remained abnormal. Three groups of 10 patients were studied, each group receiving infusions of 15, 30, or 45 mg. Hydroxyproline excretion fell by 62% and alkaline phosphatase was reduced by 72%, with no difference between the dose levels. A total of 21 patients (70%) achieved normal levels of bone turnover, indicating that low-dose infusions of APD are a safe and effective treatment for Paget's disease.

An investigation of bulk tank milk selenium levels in the San Joaquin Valley of California.

We evaluated selenium determination of bulk milk tank samples as an alternative to testing blood selenium for evaluating herd selenium status in DHIA dairy herds in the San Joaquin Valley of California. A method of determining milk selenium levels using inductively coupled plasma spectrometry is described. Mean bulk tank milk selenium levels were 0.0224 mg/L (Range 0.0126-0.0418 mg/L). No statistically significant relationships were found between bulk tank milk selenium levels of a herd and calving interval, days open or log somatic cell counts. Mean herd blood and milk levels were directly proportional to bulk tank milk selenium levels. Within a herd milk selenium levels of a cow were directly proportional to the cow's blood selenium level. Herd selenium levels were not significantly related to soil selenium levels. Determination of bulk tank milk selenium levels has the potential to be a low cost, non-invasive means of evaluating herd selenium levels in order to determine selenium deficiency. Further studies with this technique in areas which are deficient in selenium may provide estimates of the sensitivity, specificity and predictive value of bulk milk tank selenium for determining selenium deficiency in dairy herds.

Ethical considerations in research with children.

The author discusses the nature of research in a therapeutic context, the documentation required by a review committee, the problem of proxy consent for children and other 'incompetent' individuals, the balance of risk-to-benefit, and the protection owed by society to its most vulnerable members. Opposing views are presented regarding the ethics of involving children in research. The difficulty of assessing risks and benefits is acknowledged while upholding the right of parents and guardians to receive full disclosure of information upon which to base consent. There is a role for serious ethical inquiry in the search for agreement on what is right. Life implies relationships and mutual obligations. Research involving children raises ethical questions for the physician-researcher, the parents, other hospital personnel and society at large.

Salivary gland dysfunction following radioactive iodine therapy.

Radioactive iodine is used extensively for the treatment of thyrotoxicosis and thyroid carcinoma. Iodine is actively taken up by the salivary glands and, following its use, salivary dysfunction may result as a consequence of radiation damage. The literature is reviewed and a case is reported in which a patient presented with a significant increase in caries rate attributed to salivary dysfunction following radioactive iodine therapy for a thyroid carcinoma.

Fatal and non-fatal deliberate self-harm in the Wellington region 1978: a social analysis.

Some characteristics of the people who deliberately harmed themselves in the Wellington Hospital Board region in 1978, and who were either admitted to hospital or died, are described. Records show 32 people killed themselves and 306 people were admitted to hospital. Twenty-six people were admitted twice. The rate of non-fatal events was nine persons per 10000 population. For fatal events, males and unemployed people were at greater risk. Drugs were used in nine out of 32 cases. The characteristics of people at greater risk in 1978 of non-fatal deliberate self-harm were female; aged 15 to 24 years if female and 15 to 29 years if male; unemployed, sickness beneficiary or in a service occupation; domiciled in the inner city; and separated from their spouse. Drugs were used in 96.5 percent of non-fatal events. The length of stay in hospital for 48.5 percent of admissions was one day.
