Ultrasonographic diagnosis and drainage in noncalculous acute cholecystitis following surgery.

Postoperative acute cholecystitis is not so rare complication especially in postgastrectomy patients. Ultrasonic examination is the most suitable means of identifying postoperative acute cholecystitis. 24 patients were diagnosed as postoperative acute chlecystitis mainly by ultrasonic examination. The majority of cases recovered only by conservative treatment. Clinically severe cases are particularly indicated for ultrasonically guided percutaneous transhepatic gallbladder drainage. Moreover, patient with postoperative acute cholecystitis are considered to require follow-up studies after recovery.

Causes of chromosome anomalies suggested by cytogenetic epidemiology of induced abortions.

Women who visited hospitals requesting induced abortions were asked about their obstetric and medical histories by attending physicians prior to the operation. The information was solely based on their reports at the interview. The period of coverage was nine weeks, comprising six weeks before and three weeks after the onset of the last menstruation. Both the occurrence of acute respiratory infectious diseases, which included the common cold, upper respiratory tract infection (U.R.T.I.), influenza, and pneumonia, and the use of analgesics and antipyretics were most frequently reported. Regardless of whether they had an acute respiratory illness, the women who took analgesics and antipyretics (including antihistamine, cough remedy, and nasal decongestant) had 13.8% (8/58) gross chromosome anomalies. The women without the treatment had 6.3% (71/1136) anomalies. The difference was statistically significant (P less than 0.05). Triploid formation was particularly related to drug consumption, thus there were 5.2% (3/58) in the treated group and 0.5% (6/1136) in the untreated group (P=0.0074). In contrast, trisomy was not considered to be related to drug consumption on account of 8.6% (5/58) and 4.1% (47/1136) in the corresponding groups (0.10 less than P less than 0.20). When the occurrence of trisomy was examined in connection with two variables, i.e., the presence of drug consumption and the age of mothers, association was not evident (X2 assoc. = 0.883, d.f. = 1 by Cochran's method). Besides these results, neither maternal X-irradiation nor irregularity of the menstrual period was found to be related to the frequency of chromosome anomalies in the progeny.

Treatment of intrahepatic lithiasis using the choledochofiberscope.

Our experience with choledochoscopic extraction of intrahepatic calculi in eleven cases is presented. In three cases, the intrahepatic bile duct containing multiple stones was punctured under the guidance of ultrasonography, and the stones were removed choledochoscopically through the percutaneous transhepatic cholangial drainage (PTCD) tract which was dilated with catheters of increasing diameter. In nine cases, the removal of intrahepatic stones was attempted choledochoscopically through the T-tube tract or the jejunostomy. This procedure was successful in six of the nine cases. The advantage of the choledochoscopic removal through the PTCD tract compared with that through the T-tube tract is discussed, and an illustrative case is presented.

Estimated prevalence of chromosome anomalies in the first trimester of the Japanese pregnant population.

Using the cytogenetic data we have collected in induced abortions, an estimation of the prevalence rate of chromosome anomalies in the first trimester of the general pregnant population was made. Since the maternal age distribution in induced abortions skewed toward the older age group in comparison with that of the general population in Japan in 1975, the maternal age adjustment was conducted. The projected rate was found to be 5.0 per cent. It is inferred that the present frequency may give the lowest limit value, for only the gross chromosome anomalies diagnosed by the conventional Giemsa staining method were counted in the present study.

Sonographic differentiation between the umbilical portion of the left portal vein and intrahepatic bile ducts.

The method of differentiating between the portal vein and the bile ducts by means of their intrahepatic portions was studied. The left branch of the portal vein courses horizontally as the transverse portion, then veers anteriorly at an acute angle to make the umbilical portion. This characteristic form was demonstrated in all normal controls and 95% of the jaundiced patients studied. The left hepatic duct does not curve anteriorly at an acute angle, but branches off to the lateral segment running superior to the umbilical portion. This anatomic relationship and characteristic form of the umbilical portion were useful in differentiating the portal vein and the bile ducts.

Ultrasonically guided percutaneous transhepatic bile drainage: a single-step procedure without cholangiography.

Percutaneous transhepatic bile drainage under real-time ultrasonic guidance was performed in 16 patients (21 procedures) without cholangiography. All bile ducts were successfully intubated. Merits of ultrasonically guided percutaneous transhepatic bile drainage are (a) the fact that only a single step is involved, without thin-needle cholangiography; (b) no contrast medium is injected; (c) the left-sided approach permits longer intubation; and (d) irradiation is not employed.

Inherited chromosomal translocation in two families (t(4q-;13q+) and t(5q-; 13q+)).

Two families with reciprocal translocations, t(4q-; 13q+) and t(5q-; 13q+), are reported. Each proband carried multiple malformations resulting from the unbalanced segregation in two mothers of balanced carriers. The karyotype-phenotype relationship is discussed with reference to other previously reported cases of B/D translocation.

[A further chromosome analysis in induced abortions (author's transl)].

The cytogenetic data of 500 unpublished cases of induced abortions are reported. The incidence of gross chromosome abnormalities was 7.4% (37/500). The frequencies of chromosome abnormalities after 6 gestational weeks decreased gradually with advancing the gestational weeks, while the frequency in the group of 5 gestational weeks remained considerably high as 25.0%.

Pericentric inversion of chromosome No. 8.

Chromosome studies of an infant with multiple malformations were made by means of the trypsin-Giemsa banding as well as conventional Giemsa staining methods. The propositus showed 46, XY +3, -C, and it was indicated that the abnormal metacentric chromosome was induced by the pericentric inversion of chromosome No. 8, in which chromosomal breakage had occurred most likely at the bands 8p23 and 8q23. The probable formula of the inversion is 46, XY, inv (8) (p23q23). Karyotypic analyses of the parents revealed no abnormalities, and the inversion therefore occurred spontaneously. Clinical features of the porpositus are postulated to be caused by a loss of very small portions of the chromosomal material with the occurrence of the pericentric inversion.

Chromosome studies in 500 induced abortions.

A survey of the chromosome constitution in 500 induced abortions (5-12 menstrual weeks) was undertaken over a period of 1 1/2 years. There were 34 cases (6.8%) of gross chromosome anomalies: 2 cases of trisomy A; 5 of trisomy C (including XXX and XXY); 1 of mosaic trisomy C; 4 of trisomy D; 2 of trisomy E; 2 of trisomy G; 1 of double trisomy E and G; 1 of XYY; 4 of monosmy C (including XO); 2 of mosaic monosomy C; 1 of mosaicism of ring D chromosome; 1 of extra small metacentric chromosome; 3 of triploidy (including triploidy with double trisomy C and G); and 5 of tetraploidy and its mosaicism. An increased risk for the occurrence of trisomic anomalies was found with advancing age of the mothers. In contrast, the production of monosomies was not age-related. Trisomies were the most common type of anomalies and were found almost at random, regardless of the characteristics of chromosomes. Neither satellited nor small chromosomes were predominantly involved in the formation of chromosome anomalies.
