Surgical imaging systems.

Imaging i.n.surgery. is used for diagnosis, planning, intraoperative navigation and post-operative evaluation, DIgItal medical imaging modalities mclude computed tomography (CT), magnetic resonance Imaging (MRI), MR therapy (MRT), fluoroscopy and ultrasound.t These modalities are applied singly or jointly (multimodality) Surgical requirements differ according to the nature of intervention, and real-time guidance? is sometimes needed such that a sequence of images is generated and displayed as acquired, Soft copy display on CRTscreens is satisfactory for intraoperative use, while hardcopy film images or physical replica modeling may be needed in other cases. Computed tomography/ developed more than 20 years ago, remains important in craniofacial'v" and orthopedic surgery." Newer imaging systems, especially ultrasound, magnetic resonance imaging,'6 and digital fluoroscopy are used for neurosurgery, oncology, cardiothoracic and abdominal surgery. Each modality offers specific qualities that subserve specific needs in diagnosis, planning, intraoperative navigation and evaluation (Table 1).

[Role of nitric oxide in immunological liver injury in mice].

OBJECTIVE: To study the role of nitric oxide (NO) in mouse immunological liver injury. METHODS: Injection of either bacille calmette guréin (BCG) or lipopolysaccharide (LPS) alone in mice was used to induce moderate increase of plasma NO level and liver damage were seen after the RESULTS: Administration of LPS following BCG injection resulted in remarkable elevation of plasma NO level and severe liver damage. The elevation of NO level and liver damage induced by BCG or BCG + LPS were not affected by administration of L-arginine, and substrate of NO synthase. Inhibition of NO synthase by NG-monomethyl-arginine (NMMA) decreased the elevated plasma NO without effect on the elevated plasma GPT and GOT levels induced by BCG injection. The BCG + LPS induced elevation of plasma GPT and GOT levels were more pronounced after NO production was inhibited by NMMA treatment. The action of NMMA mentioned above was partially reversed by simultaneous administration of L-arginine. CONCLUSION: NO plays a protective action against liver injury induced by BCG + LPS in mice.

[Studies on multiple factor intervention in stroke of ten areas in northeast, north China and Shanghai].

In order to explore the ways to prevent from the disease, 4,793 subjects aged over 40 at high risk of stroke from a total population of 250,000 in communities with high incidence of it were studied with multiple factor intervention, by oral administration of "Nao An" capsules as a major measure in 10 areas of north and northeast China, as well as Shanghai, during 1990 to 1993. Three years after intervention, incidence of stroke decreased by 49.17% in the population, with reduction of systolic and diastolic blood pressure, levels of blood sugar, cholesterol and triglyceride. Lowering of stroke incidence correlated positively with that of blood pressure in the population. It suggests stroke can be prevented effectively by concentrated efforts in comprehensive intervention in individuals at high risk.

[Comprehensive cost-benefit evaluation for the improvement of rural water supply in Hunan province].

Comprehensive cost-benefit evaluation for the improvement of rural water supply was conducted according to the national standard methods for analysis of water quality. Results showed total bacteria count and coliform group count in the water declined by 51% after the improvement, and synthetic index of water quality declined to 19% of that before the Improvement. Environmental epidemiological studies showed yearly incidence of hepatitis A, typhoid fever, diarrhea and enteritis decreased by 2.20/1,000, 0.39/1,000, 2.70/1,000 and 14.30/1,000, respectively, as compared with those before the improvement, and with the manpower capital method, economic loss caused by the above-mentioned diseases lowered by 37,238 yuan, economic benefit gained by saving labor-time for water-taking reached 198,644 yuan, and family income increased 164,188 yuan per year. It suggests the above indicators all can be used in comprehensive cost-benefit evaluation of the improvement of rural water supply.

[Screening of high-risk population susceptible to stroke and their characteristics].

High-risk population susceptible to stroke were screened with self-designed scoring method for quantitatively evaluating the risk of stroke in 550,000 frame population in 22 areas with different incidence of stroke during 1990-1993 and its related factors were analyzed by a principal component analysis method. Results showed scoring value correlated positively with probabilities of stroke, which suggested its objectivity and feasibility. The first principal component correlated positively with age, blood pressure, blood sugar, blood cholesterol, body mass index, ect., which reflected the characteristics of high risk population susceptible to stroke, and 71.43% of those with stroke were in the first principal component class. The proportion of the population with the first principal component correlated positively with the incidence of stroke in the area, therefore, stroke prevention should be focused on such population.

Studies on the transfer techniques of three maize genes.

Maize transformation has been carried out through microprojectile bombardment, ultrasonication in a DNA buffer, and ovary-injection with a self-made microinjector. The plasmid pB48.415, which carries a 3'-truncated Bt-toxin protein gene and a hygromycin phosphotransferase (hpt) gene, was used in the transformation. Transgenic maize plants were obtained from immature embryos and embryogenic calli bombarded with a particle gun, embryogenic calli ultrasonicated under different conditions of ovaries injected 10-20 hours after pollination. The results of Dot blotting and Southern blotting analyses proved the integration of the Bt gene into maize genome.

Rapid freezing of mouse embryos in ethylene glycol at different preimplantation stages.

The objectives of this study were to examine the effects of a rapid freezing protocol on the survival and in vitro development of mouse embryos cryopreserved in ethylene glycol (EG) at different preimplantation stages, and secondly, to investigate the effect of exposure to 3.0 M EG with 0.25 M sucrose on the survival and in vitro development of mouse embryos without freezing at different developmental stages. To perform the rapid freezing procedure, embryos were equilibrated in Dulbecco's phosphate buffered saline (DPBS) containing 3.0 M EG and 0.25 M sucrose (freeze medium) for 20 min and loaded into 250 microliters straws in a single column of freeze medium. The straws were held in liquid nitrogen (LN2) vapour for 2 min and immersed into LN2. Embryos were thawed in a 37 degrees C water bath for 20 sec and transferred to DPBS supplemented with 0.5 M sucrose (rehydration medium) for 10 min and cultured for 24 to 96 h in HTF (Human Tubal Fluid) plus 4 mg/ml BSA (Bovine Serum Albumin). Significant differences were found in the survival and development of mouse embryos at different developmental stages rapid frozen in EG and sucrose: two cell 43/84 (51%), 4-8 cell 44/94 (47%), morula and early blastocyst 56/70 (80%), expanding and expanded blastocysts 10/59 (17% (p < 0.05). These data indicate that the developmental stage in which mouse embryos are subjected to this quick freeze protocol affects survival and development in vitro and the majority (80%) of morula and early blastocyst stage embryos survive the procedure. No significant differences were observed in the in vitro developmental capacity of embryos at different developmental stages after treatment with high concentrations (3.0 M) of EG solution without freezing. Further investigations are underway to better understand the reasons for different survival rates of embryos frozen at different developmental stages using the present procedure.

Gallium-67 scanning for detection of alveolitis in idiopathic pulmonary fibrosis and sarcoidosis.

OBJECTIVE: To investigate 67Gallium scanning for the detection of alveolitis in idiopathic pulmonary fibrosis and sarcoidosis. PATIENTS AND METHODS: 67Ga scintigraphy was performed in 14 patients with IPF (age: mean +/- s = 58.3 +/- 10.3 years), 13 patients with sarcoidosis (age: mean +/- s = 46.9 +/- 9.0 years) and 11 controls (age: mean +/- s = 44.0 +/- 10.5 years). RESULTS: Lung/thigh ratio was respectively 3.18 +/- 0.07 in IPF, 3.12 +/- 0.94 in sarcoidosis and 2.11 +/- 0.26 in controls. IPF and sarcoidosis groups had a significantly higher lung/thigh ratio comparing with the control group (P < 0.05). Three patients received 67Ga scanning examination after corticosteroid therapy and the 67Ga uptake decreased. CONCLUSIONS: The 67Ga scintigraphy is useful in monitoring the activity and extent of alveolitis.

Omniplane transesophageal echocardiography imaging planes exploration.

One hundred and twenty-four patients with heart disease were examined by omniplane TEE in order to systematically research every views of omniplane TEE, and further explore anatomy and image feature of each view. The result showed that omniplane TEE transducer can be rotated in probe from 0 degree to 180 degrees, obtain many views at various angles behind the heart and fully demonstrate the structure and pathology of the heart and great vessels. It was useful for clinical diagnosis because of getting more information about the heart and great vessels. As omniplane TEE probe was little rotated in esophagus, it lessened esophagus stimulation. Meanwhile, it was suitable for three-dimensional reconstruction of left ventriculum.

[Textual studies on shangzhou zhiqiao fructus Aurantii].

The original plant of Shangzhou Zhiqiao, the best among the species of Zhiqino, is confirmed as Citrus aurantium rather than Poncirus trifolia in this paper based on the description and drawings of Zhiqiao in (Bencao Gangmu) by Li Shizhen in the 16th rentury, samples from Shangzhou(Shanxi Province), and the cold-warm period of historical climate in the local area, which is believed to be related to the distribution of C. aurantium in Shangzhou. However, P.trifolia has often been confused with C. aurantium by some herbalists.

[A comparison of the changes of the nasal deformities after cleft lip repair between rotation advancement and triangular flap repair].

126 patients with the secondary nasal deformities of cleft lip were chosen at random, whose primary correction of cleft lip were performed by the same surgon. Of them, 68 patients were treated by triangular flap method, other 58 by rotation advancement method. On their photographs, two methods were compared by measurement of the distance between alar bases and median saggittal plane, the width of nostril floor, the height of the columella, the sizes of nostrils on both sides, and tilt of columella. The nasal deformities were improved in the rotation advancement group, but this method trend to result in a small nostril to make correction of secondary nasal deformities more difficult. On the other hand, the nasal defects were little improved in triangular flap group, whereas there were much less tendency to result in a small nostril.

[The causes, diagnosis and treatment of aluminum toxicity in patients with chronic renal failure undergoing dialysis].

To study the causes, diagnosis and treatment of aluminum toxicity in patients with chronic renal failure, the serum aluminum concentration was determined in 27 normal subjects, 28 patients of various kinds of diseases with normal renal function and 81 patients of chronic renal failure with hemodialysis in 65 and without in 16, of whom 41 patients were determined the aluminum concentration in the bone tissue. Clinical symptoms were carefully observed in all patients and desferrioxamine (DFO) test was performed in 17 patients, of whom 10 were treated with DFO. The results showed that treatment with improperly processed water and administration of aluminum compound were the major causes of aluminum toxicity in uremic patients. Aluminum toxicity may induce anemia, encephalopathy and bone disease, but its clinical features were nonspecific and the diagnosis may require several serum aluminum determinations or DFO test. DFO can chelate aluminum in a variety of tissues so that the latter may be released into the blood circulation. The DFO test may be used to assess the actual aluminum load in the bone tissue. The changes in serum aluminum concentration after intravenous infusion of DFO correlated closely with bone aluminum level, suggesting that the DFO test may be useful for the diagnosis of aluminum toxicity. The DFO therapy may be indicated for, 1, patients with uremia having hyperaluminumnemia due to treatment with improperly processed water and intake of aluminum-containing agents. 2. those who had serum aluminum concentration of higher than 200 micrograms/L and positive DFO test and 3. patients whose aluminum concentration in the bone tissue was 10 times greater than normal values. In this study, DFO was given intravenously in a dose of 20-40 mg/kg, twice a week. Satisfactory results were obtained in 3 to 6 months and there were no severe side effects when the agent was administered slowly.

[Effects of oxidized low density lipoprotein and very low density lipoprotein on the expression of MCP-1 by monocytes].

Monocyte chemoattractant protein-1 (MCP-1) is a potent chemotatic factor for monocytes. This study aimed at examination of the effects of oxidized low density lipoprotein (OX-LDL) and oxidized very low density lipoprotein (OX-VLDL) on the expression of MCP-1 mRNA and protein by monocytes. The guanidinium isothiocyanate method was used to extract total RNA from monocytes which were exposed to LDL, OX-LDL, VLDL and OX-VLDL for 24 hours respectively and the monocyte derived conditioned media collected. MCP-1 mRNA expression in monocytes was examined by slot blot and Northern blot analysis using a gamma 32P-end-labelled 35 mer oligonucleotide probe of MCP-1. Meanwhile, MCP-1 protein in the conditioned media was examined by use of sandwich ELISA. It was found that monocytes from human peripheral blood can express MCP-1 mRNA and protein after 24 hours incubation at 37 degrees C and the expression of MCP-1 mRNA and protein was markedly increased by a 24 hour exposure to OX-LDL and OX-VLDL, whereas the expression of MCP-1 mRNA and protein was only sightly increased when exposed to LDL and VLDL. It is concluded that monocytes can express MCP-1 that induce additional monocyte migration into the subendothelial space, and that OX-LDL and OX-VLDL enhance this function by inducing stronger MCP-1 expression in monocytes.
