Low dose cytosine arabinoside in refractory anemia with excess of blasts in transformation.

Myelodysplastic syndromes (MDS) are heterogeneous diseases. Patients with blast counts of more than 20% of nucleated bone marrow cells have a high risk of short survival. We treated six patients with refractory anemia with excess of blast in transformation (RAEBiT) with low dose cytosine arabinoside (LD Ara-C). We had one partial remission (PR), surviving 16 weeks and two complete remissions (CR), surviving 22 and 55+ months. Myelosuppression was dominant in all patients, but was not as serious as with conventional remission-induction treatments for leukemias. Bone marrow aplasia occurred in all responding patients, but a differentiation effect is possible too. Maintenance therapy with LD Ara-C may be important for the two long-lasting CR.

[Does bone metabolic activity determined scintigraphically correlate with calcium and phosphorus measurements by x-ray microanalysis?].

An animal experiment on bone healing of dental implants offered the opportunity of comparing scintigraphic images with histological findings, specially with electron microscope results and analysis of the calcium and phosphorus values in different stages. The clear correspondence between the scintigraphic results and the calcification level of the new bone parts must be seen with reservation because of possible methodical mistakes of the radio-micro-analysis from surfaces and of intraindividual differences.

[Intramyocardial block in carbamazepin poisoning].

A 37-year-old woman took 20 g carbamazepine with suicidal intent. After 16 hours the serum carbamazepine level was 260 mg/l. She died 29 hours after taking the tablets. Main cause of death was progressive intramural conduction disturbance. As carbamazepine is widely used, it should also be considered in intoxications associated with intraventricular and intramyocardial conduction disturbances. Haemoperfusion is suggested as a means of secondary poison elimination.

[Treatment of life-threatening digoxin poisoning with heterologous digoxin-specific antibody fragments].

A 72-year-old woman was admitted to hospital four hours after taking, with suicidal intent, 20 mg digoxin (100 tablets Novodigal) and 50 mg nitrazepam (10 tablets Mogadan). No digoxin tablet residues were found on gastroscopic gastric lavage. A pacemaker was placed transvenously into the apex of the right ventricle. Multifocal ventricular premature systoles and recurrent ventricular fibrillation, which was difficult to terminate, alternating with extreme bradycardia quickly responded to the administration of heterologous digoxin-specific antibodies. Five hours after treatment had been started (total dose 800 mg antibody fragments) the patient again had a stable sinus rhythm of normal rate.

An assessment of the fibrinolytic system in hemophilia A.

The fibrinolytic system was assessed in 28 hemophiliacs using the chromogenic substrate H-D-Val-Leu-Lys-pNA. To determine whether a state of hyperfibrinolysis might be associated with Factor VIII replacement therapy, 14 patients with severe disease who were intensively treated with Factor VIII concentrates were compared with 14 patients with mild disease who were receiving infrequent transfusions with cryoprecipitate or fresh frozen plasma. Seventeen normal males served as controls. With the exception of an elevated level of plasminogen activator and a decreased level of immediate antiplasmin in the mild group only, no evidence of enhanced fibrinolysis was found. Other components of the fibrinolytic system were either normal (plasmin) or increased (progressive antiplasmin containing both alpha 2PI and alpha 2M, and plasminogen). The elevated plasminogen levels were found only in the severe intensively transfused group. The elevated progressive antiplasmin levels were found in both groups of patients and did not appear to be related to transfusions. These findings do not support the concept of enhanced fibrinolysis associated with intensive Factor VIII replacement therapy.
