113CD-1H spin-spin couplings in homonuclear 1H correlated spectroscopy of metallothionein. Identification of the cysteine 1H spin systems.

Heteronuclear spin-spin couplings between 113Cd and C beta protons of the metal-bound cysteines were observed in phase-sensitive, double-quantum filtered, homonuclear two-dimensional correlated (COSY) 1H NMR spectra of 113Cd-metallothionein-2 from rabbit liver. Comparison of 113Cd- and 112Cd-metallothionein-2 spectra revealed that 19 1H spin systems show heteronuclear couplings to at least one 113Cd and were thus identified as 19 of the 20 cysteines in this protein. From a detailed analysis of the manifestations of heteronuclear couplings in the homonuclear 1H COSY spectra, two cysteines could be identified as 'bridging cysteines', with spin-spin couplings to two different 113Cd nuclei. The observed 113Cd-1H coupling constants vary between less than or equal to 5 Hz and 80 Hz.

[Treatment of breast cancer at the 1st Gynecology Clinic at the University of Vienna].

Since 1979 diagnosis, operative therapy and postoperative treatment and follow-up is performed at the Ist University-Clinic of Obstetrics and Gynecology in Vienna. This management brought a close cooperation with radiologists and other oncotherapists. A standard operation method, in our case a modified radical mastectomy with a complete dissection of the axilla after radioisotope marking of the axillary lymph nodes enables us to perform a randomised prospective study of adjuvant therapy. Though the observation time is relatively short, first results show, that metastases or local recurrences were observed only in the non-treated control group. That leads in our opinion to the cautious conclusion, that any form of adjuvant therapy is of therapeutic value. Which form of adjuvant therapy should be qualified best cannot be decided yet because of the too short follow-up period.

[Skin tests as immunologic parameters in cervical cancer].

Immunological skin-tests in 158 patients admitted at the 1st Department of Gynaecology and Obstetrics, University of Vienna, and in 25 healthy controls were done in a long-term prospective study. 123 examined patients were suffering from invasive cervical cancer stage I or Ii (group 1), 17 patients were suffering from preinvasive lesions of the cervix (group 2) and 18 patients were admitted for treatment of benign disorders (group 3). Recall-antigens--streptokinase-streptodornase (SKSD), purified protein derivate (PPD) and candida (CAND)--and a primary antigen (DNCB) were used testing cutaneous delayed hypersensitivity reactions. Preoperative total reactivity to recall-antigens calculated by a score was significantly reduced in cancer patients in comparison to other groups. Subdividing the cancer patients according to histological criteria of the malignancy we could find significantly differences in particular tests (SKSD and CAND) but not in total reactivity score. Comparing the preoperative reactivity of cancer patients with or without recurrence of malignancy within two years less pronounced differences were found. Reactivity to recall-antigens in all tests and in all patient groups were better three weeks after surgical treatment than preoperatively. The prior found differences between the groups remained significant (p less than 0,05). In contrast, cancer patients demonstrated severe anergy in postoperative DNCB sensitization test as only 1 of 33 patients exhibited normal hypersensitivity reaction. 113 out of 24 patients of group 2 and 3 (54%) demonstrated delayed hypersensitivity to DNCB in operative sensitization test.(ABSTRACT TRUNCATED AT 250 WORDS)

[Marked regression of melanoma metastases under combined chemotherapy].

We report a 60-year-old patient suffering from malignant melanoma and multiple cutaneous, lung, and intraabdominal metastases. After 6 series of combination chemotherapy ( BNCU , Hydroxyurea, Dacarbacin ), the metastases showed a remarkable regression. At present, the patient is in a nearly complete remission.

Tampon-induced changes in vaginal oxygen and carbon dioxide tensions.

Vaginal oxygen and carbon dioxide tensions were measured continuously in a group of normal young women on the second day of menstruation during a 90-minute period. PO2 averaged 3 mm Hg (+/- 11 SD) and PCO2 averaged 64 mm Hg (+/- 13 SD). The value rose to that of atmospheric air when a tampon was inserted and gradually fell, giving a mean value of 112 mm Hg (+/- 18 SD) during the following 90 minutes; preinsertion values were reached in about 8 hours. Carbon dioxide rose rapidly to almost preinsertion values (mean value of 50 mm Hg +/- 12 SD) during the 90-minute period and remained steady at this level during extended periods. As in vitro studies have indicated an oxygen-dependent production of a toxin-like protein from Staphylococcus aureus, it is suggested that intravaginal tampons may be a risk factor in the development of toxic shock syndrome by supplying oxygen, thus changing the vaginal microenvironment from anaerobic to aerobic.

Penile erection: possible role for vasoactive intestinal polypeptide as a neurotransmitter.

Concentrations of vasoactive intestinal polypeptide were measured in blood drawn from the cavernous spaces of corpus cavernosum of the human penis during tumescence and erection, and the effect of injecting the polypeptide into the cavernous spaces was studied. A significant release of the polypeptide was shown during tumescence and erection. Injection of exogenous vasoactive intestinal polypeptide induced erection. These findings support the concept of vasoactive intestinal polypeptide as a neurotransmitter in penile erection and suggest that it might have a clinical use in patients suffering from erectile dysfunction.

Controlled comparison of the pharmacodynamic effects of nicorandil (SG-75) and isosorbide dinitrate in man.

Nicorandil (SG-75) is a long acting mononitrate. A randomized, double-blind, crossover study in 10 healthy subjects has compared the haemodynamic actions of single sublingual doses of nicorandil 15, 30 and 60 mg with Isosorbide dinitrate (ISDN) 5 mg and placebo. Heart rate, blood pressure, systolic time intervals (STI) and left ventricular echocardiograms were used to assess haemodynamics over a 6 h period. Within 15 min of nicorandil administration, heart rate increased and peripheral resistance decreased (dose-dependent); the preejection period (corrected; PEPc) and the ratio of PEP to left ventricular ejection time (LVET) - PEP/LVET - were shortened by the 60 mg dose; the heart rate corrected electromechanical systole (QS2c) was almost unchanged; left ventricular end diastolic and systolic diameters (EDD and ESD) were diminished. The effects on certain parameters had not completely disappeared after 6 h. ISDN produced a similar pattern with somewhat less intense effects; although PEPc and PEP/LVET were prolonged. The effects of both drugs can be attributed to vasodilatation on both the arteriolar and venous sides. The predominant action of ISDN was to reduce preload and thereby to lengthen PEPc and PEP/LVET; in addition, it lessened afterload. Although nicorandil caused a distinct reduction in preload, as shown by the smaller EDD, the effects of afterload reduction were predominant at the high dose induced shortening of PEPc and PEP/LVET. The profile of action of nicorandil should encourage clinical testing to evaluate its ability to induce ventricular unloading.

An approach to oncological genetics.

Oncological genetics is defined as a branch of clinical cancer research dealing with the identification, description, analysis, and prevention of those human neoplasias and their related/associated syndromes in which a vertical (hereditary) transmission is assumed. The present clinical basis of this field is constituted by the 240 inherited preneoplasias and neoplasias that have been detected so far. The main hypothesis in oncological genetics might be the existence of an inherited cancer proneness phenotype (CPP), in whose determination the processes involved in the activation of cellular proto-oncogenes would presumably play a role. It is conjectured that CPP is a quantitative (random) variable with extreme values. Such indicators can be identified in small populations having a structure (i.e., tree) so that the collection of comprehensive pedigree data is indispensable.

Regulation of substrate oxidation in isolated myocardial cells by beta-hydroxybutyrate.

The role of ketone bodies in myocardial substrate oxidation was examined using freshly isolated Ca2+-tolerant heart myocytes, beta-hydroxybutyrate (beta OHB) inhibited lactate oxidation by the myocytes by 30-60%, and the inhibition was concentration dependent. Palmitate oxidation was also markedly decreased, whereas octanoate oxidation was only minimally affected by the presence of beta OHB. Lactate, octanoate, or palmitate had little, if any, effect on beta OHB oxidation. beta OHB oxidation was reduced by 22-28% in myocytes isolated from chronically diabetic rats, whereas the oxidation of palmitate remained similar to the controls. However, beta OHB still inhibited palmitate oxidation to the same extent as in the control cells. Our data support the role of beta OHB as a physiologic regulator of myocardial substrate metabolism.

[Complications of preoperative high-dose-rate iridium-192 irradiation in cervix uteri carcinoma].

A report is given about the experience in preoperative, nonfractionated high-dose iridium-192 irradiation in cases of carcinoma of the cervix uteri. A few days prior to radical hysterectomy a dose of 20 Gy (in 2 cm distance) was applied locally by means of a loop-applicator (afterloading technique, Buchler). In cases without postoperative irradiation severe vaginal necroses, in cases with postoperative irradiation severe local and peripheral lesions were observed. In 8 of 10 cases irradiated pre- and postoperatively, another surgical intervention was necessary because of complications in the urinary or intestinal tract. This publication is presented as a warning for preoperative high-dose irradiation with 20 Gy applied locally in cases of carcinoma of the cervix uteri.

[Synthetic inhibitors of serine proteinases. 31. The inhibition of trypsin, plasmin and thrombin by isomeric compounds of N alpha -arylsulfonylated omega-amidinophenyl-alpha-aminoalkylcarboxylic acid amides].

Isomeric compounds of N alpha-arylsulfonylated amides of omega-amidinophenyl-alpha-aminoalkylcarboxylic acids--N alpha-amidinophenylsulfonylated amides or N alpha-arylsulfonylated amidino anilides of omega-phenyl-alpha-aminoalkylcarboxylic acids, respectively--possess weak antithrombin activity as compared to the derivatives of the basic structure. Thus, the assumption is corroborated that specific enzyme-inhibitor interactions account for the high antithrombin activity of certain derivatives of omega-amidinophenyl-alpha-aminoalkylcarboxylic acids. In contrast, amidinoanilides of N alpha-substituted omega-phenyl-alpha-aminoalkylcarboxylic acids are potent inhibitors of trypsin and plasmin, their inhibitory activity approaches that of primary amides of N alpha-substituted omega-amidinophenyl-alpha-aminoalkylcarboxylic acids.

Distribution of immunoglobulin-bearing cells in the gut-associated lymphoid tissues of the turkey: effect of antibiotics.

Distribution of immunoglobulin (Ig)-bearing cells in the gut-associated lymphoid tissues of antibiotic treated and untreated control turkeys (Meleagris gallopavo) was compared. Antibiotic treatment was similar to a regimen used in commercial turkey production, which included preincubation dipping of fertile eggs in gentamicin solution, injection of turkeys with gentamicin at hatching, and inclusion of chlortetracycline in the diet. Tissues were examined from turkeys at 3, 7, 14, and 21 days of age with a direct immunofluorescence procedure. Cell distribution in control turkeys was as follows: In the bursa of Fabricius, IgA-carrying cells predominated at 3 days of age, but at later intervals, the 3 classes of Ig-bearing cells were in equal numbers. In the cecal tonsils, IgM- and IgA-bearing cells were in larger numbers at 3 days of age, whereas, the IgG-bearing cells were sparsely distributed. By 7 days of age, IgM cells became more numerous in the cecal tonsils and remained numerous until 21 days of age. At 3 days of age, IgA cells predominated in the small intestines and IgM cells predominated in the large intestine. At 7 and 14 days of age, IgM cells were more numerous in the small and large intestines, but by 21 days of age, IgA cell population equaled that of IgM. The IgG cells were generally sparse in the intestines. Antibiotic treatment often resulted in lower numbers of Ig-positive cells, especially those bearing IgM and IgA. Normal development of the bursa of Fabricius was also retarded in this group.

[Synthesis of N alpha-(tosyl-beta-alanyl)- and N alpha-(tosyl-epsilon-aminocapronyl)amidinophenylalanineamides as strongly effective inhibitors of thrombin].

The headline compounds were prepared for further determination of structure-activity-relationships. By conversion of cyanophenylalanines with the acidic chloride of tosyl-beta-alanine resp. tosyl-epsilon-aminocapronic acid and aminolysis of the following prepared p-nitrophenylic esters the cyano compounds would get the desired structure, which were converted over the thioamides and thioimidic esters in the amidines by the common way. At the preparation of epsilon-aminocapronic acid derivatives occurred difficulties because of simultaneous formation of the N-tosyl-epsilon-caprolactame. The preparation of corresponding aminobutyric acid derivatives wasn't possible. The prolongation of the C-chain of the branchless amino acid decreased the antithrombine effect.

[Synthesis of N alpha-(arylsulfonylglycylglycyl)-4-amidinophenylalanine amides as thrombin inhibitors].

The headline compounds were prepared using N-arylsulfonylglycylglycine-4-nitrophenyl esters. Firstly these compounds were aminolized with 4-cyanphenyl alanine and thereby the N alpha-(arylsulfonylglycylglycyl)-4-cyanphenyl alanines got were transferred into the 4-nitrophenyl esters After aminolysis the desired hydroiodides of the headline compounds starting from the cyano compounds were prepared via the carbamoyl compounds and the thiomidic acid methyl ester hydroiodides. The arylsulfonylglycylglycine rest caused a decreasing of the inhibitor activity.

Affinity sorbents containing benzamidine derivatives for biomedical application.

Affinity sorbents for trypsin have been obtained by immobilization of 4-amidinophenylalkylcarboxylic acids on macroporous silica gel. Frontal chromatography was used to measure the dissociation constants of the trypsin-immobilized inhibitor complexes (Kd). The Kd value has been found to increase 50-100 fold on immobilization. A commercial trypsin preparation was purified on the obtained biospecific sorbent. Applicability of the affinity sorbent containing 4-amidinophenylacetic acid was demonstrated by hemoperfusion in dogs with acute pancreatitis. The sorbent with immobilized low molecular mass trypsin inhibitor has the capacity to greatly reduce the level of proteases in blood without altering the concentration of protease inhibitors: this results in efficient treatment.
