Respiratory care.

In summary, the field of respiratory care has witnessed a continuum of accelerating change in its first half century of existence as a health care profession. These changes were effected by internal change agents as well as external forces in the health care arena. Based upon a health care system undergoing a near cataclysmic pace of change in a time of multifocal and critical inspection, it is anticipated that the field will undergo an even more dramatic change in its scope and direction in the next 50 years. With the guidance and assistance of administrators, medical staff, and partner departments, the respiratory care department should continue to function as an integral part of the hospital organization.

[The radiologic diagnosis of inflammatory aneurysms].

An inflammatory aneurysm is defined as a nonbacterial special type of atherosclerotic aneurysm. The macroscopic characteristics are: a porcelaneous appearance, excessive thickening of the aortic wall, and perianeurysmal adhesions. Chronic inflammatory infiltrations, which are localized in the adventitia, can be found via microscopy. Six of forty-three patients with abdominal aortic aneurysms were found to have an inflammatory aneurysm at operation. In a retrospective study, we examined the sonographic, computed tomographic and angiographic appearance of inflammatory aneurysms. Only with computed tomography can thickening of the aneurysmal wall be demonstrated; angiography does not have this capacity and can only find an inflammatory aneurysm in particular cases.

[After-care problems and complications of percutaneous biliary drainage].

This is a follow-up study of 171 patients concerning the patients' care and complications following biliary drainage. Complications caused by puncture and drainage are considered separately. External biliary drainage resulted in 39%, internal in 26% with complications. The influence of advantages and side effects upon the indications of biliary drainage is discussed. Prognosis concerning the survival rate depends on the underlying pathology: In obstructive jaundice caused by metastases the patients only survived two months while the mean survival rate in carcinoma of the biliary ducts is 18 months.

[Dispensary control and rehabilitation of surgically treated patients with bronchial carcinoma in the region of Berlin (author's transl)].

Among 1813 persons with bronchial carcinoma in the years 1970-1974 475 were treated with surgery; more than 85% being older than 60 years. 299 of these patients had a survival time of at least 12 months. 64.9% of them received a cardiac treatment, 7.7% were only followed up. Bronchial infections were more common in these patients than in the average Berlin population. Among the 207 patients who had a survival time of more than 12 months at the end of 1974 56 were working; 41 from the 74 patients belonging to the age group younger than 65 years, 15 from the 133 patients belonging to the age group older than 65 years (pensioners). General recommendations concerning systematic controlled care should be given for persons resected for bronchial carcinoma. Systematic cardiac treatment should be provided.

Prediction of surface tension, viscosity, and thermal conductivity for common organic solvents using quantitative structure-property relationships.

Predictive models for the surface tension, viscosity, and thermal conductivity of 213 common organic solvents are reported. The models are derived from numerical descriptors which encode information about the topology, geometry, and electronics of each compound in the data set. Multiple linear regression and computational neural networks are used to train and evaluate models based on statistical indices and overall root-mean-square error. Eight-descriptor models were developed for both surface tension and viscosity, while a nine-descriptor model was developed for thermal conductivity. In addition, a single nine-descriptor model was developed for prediction of all three properties. The results of this study compare favorably to previously reported prediction methods for these three properties.

QSAR and k-nearest neighbor classification analysis of selective cyclooxygenase-2 inhibitors using topologically-based numerical descriptors.

Experimental IC(50) data for 314 selective cyclooxygenase-2 (COX-2) inhibitors are used to develop quantitation and classification models as a potential screening mechanism for larger libraries of target compounds. Experimental log(IC(50)) values ranged from 0.23 to > or = 5.00. Numerical descriptors encoding solely topological information are calculated for all structures and are used as inputs for linear regression, computational neural network, and classification analysis routines. Evolutionary optimization algorithms are then used to search the descriptor space for information-rich subsets which minimize the rms error of a diverse training set of compounds. An eight-descriptor model was identified as a robust predictor of experimental log(IC(50)) values, producing a root-mean-square error of 0.625 log units for an external prediction set of inhibitors which took no part in model development. A k-nearest neighbor classification study of the data set discriminating between active and inactive members produced a nine-descriptor model able to accurately classify 83.3% of the prediction set compounds correctly.

Prediction of inhibition of the sodium ion-proton antiporter by benzoylguanidine derivatives from molecular structure.

The use of quantitative structure-activity relationships to predict IC50 values of 113 potential Na+/H+ antiporter inhibitors is reported. Multiple linear regression and computational neural networks (CNNs) are used to develop models using a set of information-rich descriptors. The descriptors encode information about topology, geometry, electronics, and combination hybrids. A five-descriptor CNN model with root-mean-square (rms) errors of 0.278 log units for the training set and 0.377 log units for the prediction set was developed. Examination of data set subclasses showed that systematic structural variations were also well-encoded resulting in 100% accuracy of prediction trends. An experiment involving a committee of five CNNs was also performed to examine the effect of network output averaging. This showed improved results decreasing the training and cross-validation set rms error to 0.228 log units and the prediction set rms error to 0.296 log units.
