Intraocular magnet of Parel.

The intraocular magnet (IOM) is a new device based on permanent magnetism providing controlled energy for removal of magnetic intraocular foreign bodies. Its use is reported in 11 cases.

Gas gangrene infection of the eyes and orbits.

The literature on Clostridium perfringens infections is reviewed up to 1983. An additional case is reported with bilateral clostridial infections of the eye and orbit. One eye followed the classical course of relentless panophthalmitis, amaurosis, and orbital cellulitis ending in enucleation. The second eye contained intracameral mud and gas bubbles that were removed by vitrectomy instrumentation. Subsequent removal of the toxic cataract resulted in a final aided visual acuity of 6/18, N8. This is the third report of a retained globe, and we believe the only known case where the patient was left with useful vision.

Intraocular diathermy coagulation.

Previously used transvitreal diathermy systems were found to be less than adequate. A new, safer 1.25 MHz medium frequency bipolar instrument is presented, which obviates most of the earlier drawbacks. Coagulation at the retinal surface can be obtained with as little as 0.15 J.

Ophthalmic findings among one thousand inhabitants of Rarotonga, Cook Islands.

A survey of 986 Polynesians was conducted in Rarotonga, Cook Islands, for the complications of diabetes. The ophthalmologists in the team made a general assessment of eye health. Trauma was a major cause of blindness. Diabetic retinopathy, trachoma, cataract and macular degeneration were not common findings. The initial criteria of nuclear cataract were pigmentation or opacification with loss of fetal suture detail. This was invalidated by the finding in young adults of enhanced pigmentation may augment the intrinsic filter of near ultraviolet light. Of the 118 people with chorioretinal scars, 25 shared features with cases reported from the Pacific region attributed to filariasis. The majority, however, were typical of toxoplasmosis. Cases of pseudoexfoliation were rare. This is surprising in view of the high prevalence reported among Australian Aborigines at a comparable latitude.

The Intra Ocular Cutter.

Inappropriate infusion, either suprachoroidal or subretinal massive intraocular haemorrhage and inadvertent tearing or nibbling of the retina, are problems which may be induced iatrogenically during vitrectomy. Familiarity with the mechanical principles of each type of vitrectomy instrument will serve to minimise such hazards. This paper presents the special features of our newest generation intra Ocular Cutter (I.O.C.) in its two modes: (1) the fully integrated, electronically controlled, automatic unit with surgical stand and tray; (2) the Standby I.O.C., which is battery operated with simple hand suction. Particular stress is laid on the economy of ocular perfusion which may be achieved with informed use of these instruments.

Diabetic retinopathy in pregnancy: a 12-year prospective survey.

A prospective study extending over 12 years has surveyed 107 latent diabetics and 172 clinical diabetics during pregnancy. Among the clinical diabetics background retinopathy was evident or developed during pregnancy in 40 cases. Eleven cases showed proliferative changes, only 1 such case appearing de novo during gestation. There was a higher incidence of fetal loss in the diabetic than the latent diabetic group, and this loss was highest in those with proliferative retinal disease. Pregnancy was to associated with any increased risk to the mother of progression of retinal changes and visual loss. The only known direct relationship, namely, that between duration of diabetes and ocular complications, is confirmed.

A scanning electron microscopic examination of retinoblastoma in tissue culture.

Tissue cultures were established from 6 retinoblastomas and the cultured cells examined by scanning electron microscopy. Cells from cultures of normal fetal retinal and glial tissue were also examined by SEM. Though a number of different cell types were found to coexist in the retinoblastoma cultures, most had the features of cells of either glial or neuronal origin. Evidence of further differentiation of cells in each series was seen. These findings indicate that the cell of origin in retinoblastoma is a multipotential stem cell which, though neoplastic, is still capable of differentiation into both glial and neuronal series.

A new system of microsurgery for human and experimental corneal grafting. II. Clinical and experimental applications.

After preliminary testing with pig, dog, rabbit, monkey, cat, and human cadaver eyes the CLCC system has been used for penetrating and lamellar keratoplasty in human subjects. Clinical experience covers a broad range of corneal pathology. Detailed operative instructions and observations are given. In the experimental situation the CLCC is being used to achieve deep lamellar corneal dissection for a continuing study of non-penetrating keratoplasty.

Urokinase in the management of vitreous hemorrhage.

Urokinase is a plasminogen activator of human origin which breaks up the fibrin base of blood clots. When given as an intravitreal injection it produces hypopyon and glaucoma, both of which transient. In a series of 27 patients (34 eyes) with unresolved vitreous haemorrhage, this simple and relatively atraumatic treatment has produced marked objective improvement in 10, and greatly improved the life styles of a further 9. This series brings the total of reported cases to 93. When compared with recent American reports of surgical vitrectomy for vitreous haemorrhage, intravitreal urokinase appears to have a higher success rate, with a lower complication rate both in the short and long term. This study suggests that, despite the high cost of the purified enzyme, urokinase should be come the first line of attack in vitreous haemorrhage, vitrectomy being reserved for those patients who fail to respond.
