Bronchial hyperreactivity after infantile obstructive bronchitis.

A follow-up study was performed on 406 patients treated for infantile obstructive bronchitis during the period between 1964 and 1973. Their mean age was 12.6 years at the time of the study. The male: female ratio was 1.7. Forty-three patients (11%) became asthmatic within 10 years after onset of the wheezy episode of infancy. In one-third of the 363 non-asthmatic children, bronchial hyperreactivity was shown by acetylcholine and histamine provocation. There was a significant correlation between the number of recurrent obstructive episodes and the length of the period of recurrent wheezing on the one hand and bronchial hyperreactivity on the other hand.

[Postextrasystolic sinus responses after autonomic blockade in patients with sinus node dysfunction].

The interpretation and significance of postextrasystolic responses obtained in human electrophysiological examinations of patients with sinus node dysfunction has long been a matter of controversy. We carried out programmed atrial stimulation by the method of Strauss et al. in 54 patients with sinoatrial disorder, before and after pharmacologic autonomic blockade (with propranolol 0.2 mg/Kg and atropine sulfate 0.04 mg/Kg intravenously). There were two responses, as follows: computable sinoatrial conduction times and chaotic patterns. Patients were divided into groups on the basis of their intrinsic heart rate (IHR). If the total estimated sinoatrial conduction time over greater than or equal to 200 msec and greater than or equal to 147 msec after autonomic blockade and chaotic pattern were considered to be pathologic, so the ratio of abnormal parameters decreased from 73 to 44% in patients of normal IHR, and increased from 70 to 90% in patients of abnormal IHR. The latter 90% was mostly to the expense of the incalculable chaotic patterns. Interpreting a postextrasystolic curve, the existence of reset zone refers to the functional integrity of the sinoatrial node, to the organisation and synchronism of sinus potentials, which depends on the balance of autonomic nervous system and on the intrinsic electrophysiological integrity of the pacemaker cells.

Electrophysiological classification of normal and pathological sinus node function.

The sympathetic and parasympathetic neurological cardiac effects were blocked with atropine and propranolol. The intrinsic heart rate (IHR) was determined and rapid atrial pacing (RAP) was carried out before and after administration of the drugs. The primary and secondary postpacing parameters were examined in both circumstances. Normal (n = 31) and pathological (n = 20) cases were differentiated on the basis of the IHR. In the secondary phase (PPC 2-10) after autonomic blockade returned to the basic frequency was of exponential character in the great majority of normal IHR cases. Anomalies may appear in both the primary and secondary phases. They are of different types: one of them concerns the recovery time; another electrophysiological anomaly occurs when there is no return to the predrive normal cycle length after pharmacological neurotomy. In the secondary phase there may appear sinoauricular blocks. They may depend on or appear independently of the effect of the vegetative nervous system. The new test allows a physiopathological classification of normal and pathological sinus node function.

The secondary poststimulative phase after rapid atrial pacing: its importance in the diagnosis of sinus node dysfunction.

The effect of atropine and the beta-blocker Tobanum was studied on intrinsic heart rate (IHR) and on rapid atrial pacing carried out before and after administration of the drugs. The primary and secondary postpacing parameters were examined in both circumstances. In patients with normal IHR, return to the basic heart frequency after the drugs showed an exponential characteristic while in patients with abnormal IHR, the cycles of PPC 2-10 have lost this characteristic feature. The latter was a more characteristic electrophysiological sign of sinus node dysfunction than the recovery time of the sinus node. In patients with abnormal IHR, maximum CPPC1 is sometimes normal.

Application of His bundle recording to study disorders of atrioventricular and intraventricular conduction.

This paper briefly describes the techniques available for evaluation of conduction in our catheterization laboratory. The application of the technique for recording His-bundle potentials has greatly enhanced our understanding of rhythm and conduction disturbances and has provided useful and important information in the management of atrioventricular and intraventricular conduction disorders.

Adams-Stokes Syndrome↗

Application of His bundle recording in different rhythm disturbances of the heart.

The new intracardiac electrophysiological method: His bundle electrocardiography, combined with the electrophysiological examination used in routine clinical practice, has considerably improved our knowledge of the cardiac dysrhythmias. It helps to localize the focus of the atypical junctional and other supraventricular premature beats complicated with either organic or functional intraventricular conduction disturbance. The various forms of paroxysmal tachycardias can be differentiated more exactly from each other. In pre-excitation syndrome, it has become possible to detect the abnormal pathway and to screen the patients susceptible to life-threatening arrhythmias. By means of this the diagnosis of sick sinus syndrome and estimation of its severity can be made more exactly. In addition to the diagnosis, the electrophysiological examinations connected with pharmacological tests give valuable information whether medical or pacemaker therapy should be applied. Six cases of various forms of cardiac dysrhythmias are presented to demonstrate the superiority of this method.

A Roentgen-anatomic study of the relationships between conduction defects and coronary disease.

Intraventricular conduction defects of various types were correlated to the extent of coronary disease and to the parameters reflecting the function of the left ventricle in 125 patients who had been subjected to coronary arteriography. In the patients with conduction disturbances, coronary disease was found to be more extensive and the proportion of abnormal parameters of left ventricular function higher than in those with normal conduction. The relationship of the various conduction defects with the lesions of the individual coronary branches and with the parameters of left ventricular function are discussed in detail.

Scleroderma-heart and conduction disturbances.

Disturbances of impulse formation and conduction may arise in the early stage of scleroderma, as a sign of cardiac involvement, termed "scleroderma heart". The incidence of various intraventricular conduction disturbances and their association with other disorders of cardiac rhythm were analysed in 193 scleroderma-patients by age. The presence of ventricular conduction abnormalities was found to carry an increased hazard of the production of disorders of atrioventricular conduction and of abnormal impulse formation. The patients were also followed up for deviation of the R axis in the frontal plane. Septal hypertrophy was found more frequently in patients with intraventricular conduction disturbances than in those free from disorders of this type.

Frequency of p53 codon 72 genotypes in human papillomavirus associated squamous intraepithelial lesions and cervical cancer.

BACKGROUND: A recent study suggested that the p53Arg (at residue 72) homozygous genotype could be a potential genetic risk factor for cervical cancer among white women. To confirm this result we examined the proportion of p53 genotypes in a larger number of patients with cervical cancer and in patients with squamous intraepithelial lesions (SIL) compared to a control population. MATERIALS AND METHODS: We used allele-specific primers to amplify the p53Arg and p53Pro sequences and we examined the proportion of genotypes in the study populations using chi 2-test. RESULTS: The distributions of p53Arg homozygous, heterozygous and p53Pro homozygous genotypes were 63%, 27% and 10% in cervical cancer patients, 53%, 36% and 8% in individuals with SIL, and 60%, 36% and 4% in control population. Using chi-square test, no significant difference was found between genotype frequencies in the study groups. CONCLUSION: Thus, the p53Arg homozygous genotype does not seem to increase the risk of cervical cancer in Hungarian women.

Integration status of virus DNA and p53 codon 72 polymorphism in human papillomavirus type 16 positive cervical cancers.

BACKGROUND: The aim of this study was to determine the physical state of HPV16 DNA and to reveal any association between the physical state of virus DNA and pathologic or prognostic factors in HPV16 positive cervical cancers. The other aim was to estimate the role of p53 codon 72 polymorphism in disease progression. MATERIALS AND METHODS: The presence and physical state of HPV16 DNA was analysed by Southern blot hybridisation and E1-E2 specific PCRs in the primary tumours and pelvic lymph nodes of 85 cervical carcinoma patients. Results Integrated HPV16 DNA was found in 32 out of 41 (78%) primary tumours and 2 out of 22 (95%) lymph nodes carrying HPV16 DNA. No significant association was found between integration of virus DNA and course of the disease. There was a trend towards an association between disease recurrence and the presence of the p53 codon 72 arginine homozygous genotype (OR = 3.41, p = 0.23). CONCLUSION: The physical state of HPV16 DNA does not seem to play a major role as a prognostic indicator in Hungarian cervical cancer patients, while the p53 codon 72 genotype may have an impact on the clinical outcome of the disease.
