Histologic appearances of endoscopic gastric mucosal biopsies 10--20 years after partial gastrectomy.

Fifty-six symptomatic patients, who had had a partial gastrectomy 10--24 years previously for benign disease, were examined by endoscopy and multiple mucosal biopsies. No patient had a completely normal gastric mucosa. Varying degrees of gastritis, intestinal metaplasia, foveolar hyperplasia and dysplasia were seen. Seventeen patients (30%) had extensive areas of dysplasia associated with acute or chronic inflammation. Severe dysplasis, however, was detected in only one case. In five patients a "diffuse type" carcinoma of the gastric stump was found. While it is generally accepted that significant dysplasia (moderate or severe dysplasis) is premalignant, the degree of risk is unknown. This makes management of individual patients difficult.

Randomised open controlled trial of colloidal bismuth subcitrate tablets and cimetidine in the treatment of duodenal ulcer.

In a study of 28 outpatients with endoscopically proven duodenal ulcers, 14 patients (with a total of 15 ulcers) were treated with bismuth tablets (colloidal bismuth subcitrate, De-Nol, Gist-Brocades NV) and 14 patients (14 ulcers) were treated with cimetidine (Smith, Kline, and French). Clinical and endoscopic assessments were made after four and six weeks' therapy. After four weeks, 10 of the bismuth treated ulcers (67%) and eight of the cimetidine treated ulcers (57%) were completely healed. After six weeks of therapy, complete healing was seen in 86% of both the bismuth treated and the cimetidine treated ulcers. Twenty of the 24 completely healed ulcer patients (10 of each group) cooperated in a three month follow-up study. Pain recurred in three patients of the bismuth group and four of the cimetidine group and they were examined endoscopically. A recurrent ulcer was found in one of the bismuth treated patients and in three of the cimetidine treated patients. These observations indicate that colloidal bismuth subcitrate was at least as effective as cimetidine in the healing of duodenal ulcer.

Local disorganization of interdigestive migrating complex by perfusion of a Thiry-Vella loop.

In dogs, a 50-cm Thiry-Vella loop was perfused with different glucose solutions (10-60%) and the effect on the interdigestive migrating myoelectric complex was studied. The perfusions did not disrupt the interdigestive pattern outside the loop, but produced a progressive "fall-out" of phase III, related to the concentration of the glucose solution. Quantitative analysis of the spike activity in the loop showed that the interdigestive pattern was partially disrupted by the 10% glucose solutions and that the 50 and 60% glucose perfusions induced a typical fed pattern characterized by short periods of spike activity alternating with short periods of quiescence. It was concluded that 1) perfusion of a Thiry-Vella loop with glucose solutions changes the myoelectrical activity of the loop from an interdigestive to a digestive pattern without interfering with the migrating myopelectric complex outside the loop; and 2) this local disorganization must be mediated by factors interfering with the effector mechanism in the loop.

Bile acid output and the interdigestive migrating motor complex in normals and in cholecystectomy patients.

Bile acid output in the duodenum was studied in relation to the interdigestive migrating motor complex in normal subjects and in cholecystectomy patients. In normal volunteers the bile acid output preceding a duodenal activity front (217 +/- 89 mumol/30 min) was 374% of the output following a front (58 +/- 24 mumol/30 min) (P < 0.05). In cholecystectomy patients the prefront output (354 +/- 64 mumol/30 min) was 195% of the value obtained in the postfront period (234 +/- 59 mumol/30 min) (P < 0.05). These results indicate that the bile acid output into the duodenum during the interdigestive phase in fluctuating in accordance with the interdigestive motility complex.

Fasting plasma motilin levels are related to the interdigestive motility complex.

Fluctuations of plasma motilin and gastrin levels were analyzed in relation to the interdigestive motor complex in humans, taking the onset of a duodenal activity front (phase 3 of the complex) as a reference point and the time lapse between two fronts as a time base. Statistical analysis showed a close relation between motilin fluctuations and the occurrence of duodenal activity fronts.

Motilin and the interdigestive migrating motor complex in man.

In order to assess the possible role of the new candidate gut hormone, motilin, in controlling the interdigestive migrating motor complex (MMC) in man, 14 normal subjects were studied after an overnight fast by means of three pressure-recording catheters with orifices 25 cm apart in the upper small intestine. The typical aboral progressing bursts of pressure waves occurred at a mean interval of 137 minutes and were preceded by a peak motilin level 25 pmol/liter higher than the lowest level in the postactivity-front quiescent period. To study the effect of exogenous motilin, an infusion of pure porcine motilin at various dose levels was given to 16 normal volunteers shortly after the onset of the phase I quiescent period. Motilin infusion induced an activity front in 12 of the 16 subjects. The mean activity front interval was reduced to 46 min (P less than 0.001). This effect could be obtained even at the low dose level of 0.4 pmol/kg/min, which produced an increase in plasma motilin level of only 57 pmol/liter. These data suggest that a cyclic rise in plasma motilin levels is one of the factors involved in the production of the activity front of the migrating motor complex in man.

Leiomyosarcomatosis of the gastrointestinal tract.

A case report of a patient suffering from diffuse leiomyosarcomatosis of the gastrointestinal tract is presented. The endoscopic features of the lesions localized in the stomach, the small bowel and the colon are compared with the radiological data. A description of the pathology of these polypoid lesions is given. The origin of this widespread leiomyosarcomatosis of the gastrointestinal system is discussed.

Bile acid studies in patients with Crohn's colitis.

Bile acid studies were performed in patients with Crohn's disease, radiologically confined to the colon. The bile acid pool size of 10 patients with isolated Crohn's colitis was significantly lower than that of 10 normal control subjects (P less than 0.001) and of 10 ulcerative colitis patients (P less than 0.005). Measurements of 14C-excretion in breath and in 24 hours stool collections after the administration of 5 muCi 14C-glycocholate showed a normal 14C-excretion in breath and usually a markedly increased loss of 14C in the stool (greater than 7% of the dose). The simultaneous administration of 5 muCi 3H-polyethylene glycol MW 4000 (3H-PEG) as a marker indicated that the 14C/3H ratio in the patients with Crohn's colitis was significantly greater than in a control series of patients with diarrhoea not due to bile acid malabsorption. Studies on the composition of duodenal bile showed a significantly decreased concentration of deoxycholic acid in duodenal bile. These observations suggest bile acid malabsorption in patients with Crohn's disease apparently confined to the colon.

Achalasia, diffuse esophageal spasm, and related motility disorders.

From 1972 to 1977, the authors observed 156 patients with primary esophageal motility disorders which caused such a severe degree of dysphagia that treatment by pneumatic dilatation was deemed necessary. Before dilatation, 24% of the patients presented with motility disorders that did not fit well into the two classical disease entities, diffuse esophageal spasm and achalasia (absence of peristalsis with presence of lower esophageal sphincter (LES) relaxations or presence of peristalsis with absence of LES relaxations). After treatment with pneumatic dilatation, these "intermediate" forms constituted 45% of the motor disorders. This was due mainly to the reappearance, on manometric tracings, of peristaltic contractions and of LES relaxations. Radiologic and manometric observations suggest that in many patients, this "return of peristalsis" may be an apparent change in pressure pattern rather than a real change in motility. In 6 of the 156 patients, a deterioration of the esophageal motility disorder was observed, which was characterized by the loss of peristalsis and of LES relaxations over a period of a few months or years. The frequent occurrence of intermediate types of motility disorders and the transition from diffuse spasm to achalasia suggest that achalasia and diffuse esophageal spasm are part of a spectrum of related motor disorders.

Evaluation of the discriminative power of lipidophoresis by discriminant analysis using a simple quantitative method of agarose lipidophoresis.

A simple, reproducible and quantitative method for separation of lipoproteins in agarose is presented. The validity of the results is shown by comparison with gravimetric analysis of fractions separated by ultracentrifugation in a discontinuous NaCl-KBr gradient. Results for a large group of normals and of patients with various lipid disorders are presented, and discriminant analysis is used to evaluate objectively the potential of lipidophoresis in the classification of hyperlipoproteinemias. It is shown that quantitative lipidophoresis gives as much information as the determination of cholesterol and triglycerides, for classification purposes.

Submucosal tumors of the colon: experience with twenty-five cases.

Submucosal tumors of the colon are rare and require alertness on the part of the physician for early diagnosis. These tumors may not cause symptoms before attaining large size. In most cases the clinical histories are atypical. Because of the possibility of malignancy, adequate diagnosis and treatment are necessary. This report deals with 25 cases of submucosal tumors of the colon. The tumor series included seven leiomyomas, one granular-cell myoblastoma, two leiomyosarcomas, seven carcinoid tumors, six lipomas, one lymphangioma, and one neurofibroma. Five of the tumors were asymptomatic. The endoscopic appearances of the tumors, although not diagnostic, may give some indications as to their natures. Endoscopic removal is possible, although it is indicated for high-risk patients only. Because submucosal tumors may be malignant, and because differentiation from secondary tumors (metastases) or tumor-like lesions is difficult without histologic examination, complete removal of the tumor is the treatment of choice.

Disruptive effect of test meals on interdigestive motor complex in dogs.

The disruptive effect of food and food components on the migrating myoelectric complex (MMC) was studied in conscious dogs. Three types of test meals were fed, and the interval between feeding and the reappearance of the first phase III of the MMC was determined. When commercial dog food was used a linear relation was observed btween the quantity of food (expressed as kcal/kg body wt) and the duration of the disruption of the MMC. Equicaloric amounts of the three major food components disrupted the MMC pattern for periods of time that were related to the nature of the food: the effect of arachis oil lasted longer than that of sucrose and much longer than that of milk protein. Medium chain triglycerides had a potent disruptive effect. Mixtures of these food components produced a disruptive effect that could not be calculated from the individual effect of each component. Phase III of the first and second MMC after feeding started at a lower level of the intestine than after a long fast. The duration of disruption of the MMC after a meal depends much more on the physicochemical composition of the food than on its volume or amount of calories.

Role of gastrin and insulin in postprandial disruption of migrating complex in dogs.

The duration of the disruption of the interdigestive migrating myoelectric complex (MMC) by various test meals in dogs was correlated with changes in serum gastrin and insulin levels. The test meals consisted of milk protein, sucrose, arachis oil and medium chain triglycerides (MCT). Intravenous infusions of glucose 20% were also used. Electrical activity of the small intestine was registered by means of electrodes implanted over the entire length of the gut. Hormones were assayed by radioimmunoassay techniques. The insulin level rose significantly after both the glucose infusion and the sucrose meal. The rise was small after the milk protein meal and nothing after arachis oil and MCT. Gastrin level was not changed by arachis oil or MCT and rose slightly after sucrose and milk protein. The MMC was not disrupted by glucose infusions, but was disrupted for 5--7 h by archis oil and for 6--12 h by MCT. We conclude that in dogs neither gastrin nor insulin have an important role in the mechanism of disruption of the MMC after feeding.
