Mechanism of action of Clostridium difficile toxin B: role of external medium and cytoskeletal organization in intoxicated cells.

Toxin B, an exotoxin produced by Clostridium difficile, induces the rounding-up and arborization of cultured mammalian cells, a typical effect which resembles that provoked by cytochalasins. In this study, the effect of toxin B was examined on astroglial cells grown in primary culture. A specific antiserum to toxin B was used to investigate its mechanisms of action. We found that the toxin exerts its effects on cell morphology after its incorporation into cells. The internalization of toxin B requires the presence of calcium ions in the extracellular medium. Replacement of NaCl with sucrose or with potassium glutamate prevents the internalization of the toxin. The direct introduction of calcium ions into cells by the calcium ionophore A23187 stimulates toxin-induced morphological changes. In contrast, toxin-induced morphological transformations were prevented in cells treated with tumor-promoting phorbol. esters or with dibutyryl-cAMP, although such treatment did not abolish the internalization of the toxin. As in the other cell types, the earliest effect of toxin B on astrocyte cytoskeleton is the disruption of actin filaments, without no visible alteration of intermediate filament nor microtubule networks. As astrocytes with toxin-induced stellate morphology survive toxin treatment, the progression of cell morphology and cytoskeleton organization were followed for several weeks. Twenty-six days after exposure to toxin B, stellate astrocytes have processes which were markedly longer and much more branched than those of cells freshly exposed to toxin. At that time, cells are still devoid of F-actin as assessed with rhodamine-conjugated phalloidin and only 70% contain vimentin while all astrocytes present in control cultures express vimentin. Some flat epithelioid astrocytes with prominent bundles of microfilaments reappear during the second week after toxin treatment. Our results show that Clostridium difficile toxin B is internalized into brain astrocytes in culture where it acts by modifying cytoskeletal elements. Its cytopathic effects are reversible. Although actin-related components of the cytoskeleton are the major target of toxin B, other cytoskeletal elements also seem to be affected.

[Quantifying functional deficits in patients with multiple sclerosis using a computer-assisted visuomotor tracking procedure].

23 Patients with multiple sclerosis were subjected to a visuomotor tracking test, consisting of subtests of different task difficulty, while being in a state of acute exacerbation and four weeks later, while exhibiting a more or less pronounced clinical remission (Kurtzke DSS 3.9 (+/- 1.4) vs. 3.3 (+/- 1.5); p less than 0.001). Corresponding to this the tracking test performances improved, the difference being significant only with the easiest test run. The tracking performances correlated with the Kurtzke scores, both for the acute state and for the remitted state, the highest correlation showing up for the easiest test run. Hence, the visuomotor tracking test seems to be suited to measure functional deficits and to evaluate the efficiency of therapeutic procedures.

Antibody-mediated enhancement of calcium permeability in cardiac myocytes.

Our study shows that antibodies, specific to the ADP/ATP carrier of the inner mitochondrial membrane, crossreact with the cell surface of cardiac myocytes, where the calcium channel seems to be the antigenic determinant. The antibodies enhanced the calcium current and suppressed its inactivation. Affinity-purified antibodies (IgG) exhibit an acute cytotoxic effect, which required extracellular calcium and was prevented by calcium channel blockers. Our findings suggest that antibody-mediated cytotoxicity results secondary to calcium overload caused by enhanced cellular calcium permeability, requiring no complement-dependent process.

Intermittently occurring right-posterior slow waves (IRP) in psychiatric patients. An electroencephalographic indicator of cerebral dysfunction.

Intermittent right-posterior accentuated slow waves (irregular theta and/or delta waves: IRP phenomenon) in the EEG showed an incidence of about 5% in the total in-patients of a psychiatric university hospital. The IRP phenomenon was found in 7.5% of patients with schizophrenic psychoses, 2% of patients with affective psychoses and in 2.4% of patients with neurotic and personality disorders. The IRP groups differed from control groups in a number of sociobiographical and clinical characteristics. Having repeatedly confirmed these relationships with different samples, we regard IRP patients, irrespective of the diagnosis given, as having a maturational deficit of brain function.

Prediction of neuroleptic on-drug response in schizophrenic in-patients by EEG.

The subjects were 34 acutely ill in-patients who met the RDC criteria of schizophrenic psychosis, and 4 EEGs were recorded from each patient before, 2 h and 24 h after oral intake of a single dose of 150 mg perazine, and on the 28th day of the neuroleptic treatment period. As a criterion of clinical response a decrease of at least 66% in the schizophrenia-specific sum score of the Brief Psychiatric Rating Scale on day 28 relative to the baseline value was decided upon. The EEGs were assessed using a newly developed procedure which takes into consideration 4 derivations simultaneously. As we tried to search out EEG variables with predictive value the statistical data analysis underlying our findings should largely by regarded as exploratory. Independent of day, responders (R) showed a tendency towards more low voltage desynchronized epochs (non-A stage) than non-responders (NR). Thus, R exhibited a higher degree of dynamic variability or a broader range of control of the spontaneous vigilance fluctuation (dynamic lability) than NR (dynamic rigidity). Furthermore, R and NR differed with respect to their time-dependent changes of non-A epoch frequencies before medication. While R showed a monotonous increase which is typical for normals, NR did not. Because of considerable inter-individual variability these group differences could not be used for individual prediction of the therapy response. By means of a qualitative data analysis R could be distinguished from NR with regard to various test dose-induced changes of the topographical distribution of absolute alpha power. All the group differentiating variables showed a time course of the same kind: R showed a prompt and ample deflection and the same recovery of baseline; NR, in contrast, showed no significant deflections at all. These findings are in line with the results concerning the dynamics of vigilance and certain claims of earlier authors according to which EEG changeability should be decisive for therapeutic outcome.(ABSTRACT TRUNCATED AT 400 WORDS)

Acquisition of vimentin in astrocytes cultured from postnatal rat brain.

Vimentin and glial fibrillary acidic protein (GFAP) represent the principal constituents of intermediate filaments found in astrocytes. In contrast to vimentin-GFAP transition which occurs during glial development in situ, vimentin coexists with GFAP in cortical astrocytes allowed to differentiate in culture. To examine whether culture conditions or proliferative activity of the cells is responsible for the expression of vimentin, we generated cultures of GFAP-positive, vimentin-negative astrocytes isolated from 26-day postnatal rat brain cortices. Isolated astrocytes are characterized by a very thin rim of perinuclear cytoplasm and by numerous processes. Antiserum to GFAP labelled major processes and cell somata of some astrocytes, especially those with relatively short and large processes. Within 3 days in culture, all astrocytes accumulated GFAP in hypertrophic cell bodies and many began to express vimentin. Vimentin appeared primarily close to nuclei, and filaments of vimentin extended into proximal segments of the cell processes. In some astrocytes, however, vimentin was always absent. Combined double immunolabelling and histoautoradiography experiments demonstrated that the acquisition of vimentin was independent of the ability of astrocytes to incorporate tritiated thymidine. The results indicate that astrocytes isolated from 26-day postnatal rat brain are heterogeneous with respect to their ability to express vimentin and that vimentin synthesis is not correlated with the growth state of the cells as had been previously suspected.

Auditory evoked potentials as possible predictors of outcome in schizophrenic outpatients.

As part of a follow-up study on long-term neuroleptic treatment, 36 schizophrenic out-patients under neuroleptic treatment were studied in an 'auditory oddball' event-related potential paradigm after a 3-month stabilization phase following clinical discharge. In the first cross-sectional analysis, we evaluated the influences of age and gender and tried to find auditory evoked potential (AEP) variables, which might be promising as potential predictors of the course of illness. Compared with healthy age-matched controls, the patients showed smaller N1/P3-amplitudes. Males and females showed only minor AEP-differences, the amplitude N1/P2 being slightly higher in females. Age correlated positively with the latency P3 and negatively with the latency P2R. The interpeak latency P3-P2R showed the highest correlation with age. Test/re-test reliability was measured and variables with r less than 0.60 were rejected. The amplitudes N1/P2 and N1/P3 showed the highest test/re-test correlations. The more severely disturbed patients (global assessment scale [GAS]-score less than 65) had shorter interpeak latencies P2F-N1F than the less disturbed patients (GAS greater than or equal to 65). Patients with a high rate of relapse tended to have shorter interpeak latencies P2F-N1F than patients with low rates of relapse. Our results indicate that the interpeak latency P2F-N1F has an acceptable test/re-test reliability (C3: r = 0.72; C4: r = 0.80) and is related to clinical variables characterizing the course and outcome of illness. This leads to the hypothesis that a short interpeak latency, P2F-N1F, might be a predictor of poor prognosis.

The importance of the concept of vigilance for psychophysiological research.

The variety of meanings nowadays attached to the notion of "vigilance" essentially conceals H. Head's object in view when introducing the term more than half a century ago. It may be supposed that a major reason for the existing confusion is that Head failed to distinguish explicitly between "vigilance" as a force, i.e., a reorganizing power ("vital activity") on the one hand and as a system's state ("state of the nervous system") established by the action of the reorganizing power, on the other. We try to explain that the original intention of "vigilance" is compatible with recently elaborated proposals according to which living systems have to be regarded as organizationally closed. Rather than regarding an organism only alternatively, i.e., either as organizational closed (alive) or open (dead), "vigilance" implies a graduated description according to the state of order which has actually been realized by "vigilance".

[Vigilance--a state of order or a force that produces order?].

The variety of meanings nowadays attached to the notion of "vigilance" essentially mask H. Head's original intentions when he introduced the term more than half a century ago. In an attempt to overcome the obviously existing disturbance in scientific communication, we contrast the new meanings, both with each other and with Head's original conception. We also discuss the importance of "vigilance" as a precursor of a current change of paradigms. We conclude that the original intention of "vigilance" is in line with recently elaborated proposals stressing the self-referential character of living systems. This revised notion of "vigilance" might serve as a conceptual base for empirical research taking self-reference into consideration, and hence for biological research in the true sense of the word.

Topographical distribution of absolute alpha-power in the EEG and psychopathology in schizophrenic outpatients.

In 36 schizophrenic outpatients (Research Diagnostic Criteria) under neuroleptic maintenance treatment the EEG was recorded under resting conditions. The absolute alpha-power (7.5-13 Hz) was estimated by Fast Fourier Transform as the mean of 300 2-second epochs for the leads F3/A1, F4/A2, 01/A2 and 02/A2. Two lateralization quotients (anterior, posterior) and two anteriorization quotients (left, right) were calculated from the absolute alpha-power. Clinical status was assessed cross-sectionally with BRPS, GAS and CGI. The main findings are: 1) A linear relationship between the left/right ratio of absolute alpha-power over posterior regions and anxiety/depression--the more the alpha-power is left-lateralized, the higher the anxiety/depression score; 2) a curvilinear, inverted U-shaped relationship between the left/right ratio of absolute alpha-power over posterior regions and the score of schizophrenia-specific symptomatology; 3) the amount of daily neuroleptic dose (mgCPZ) has no impact on EEG-parameters. Results are discussed with respect to hemisphere asymmetry models of psychopathology.

Antibodies against ADP-ATP carrier enhance Ca2+ current in isolated cardiac myocytes.

Antibodies previously described to inhibit specifically nucleotide transport (ADP-ATP carrier) of the inner mitochondrial membrane were found to bind specifically to the sarcolemma of the enzymatically isolated rat ventricular myocytes. In this communication, we report for the first time that a component of these antibodies enhanced the Ca2+ current in isolated cardiac myocytes and potentiated twitch tension in ventricular strips. Prolonged exposure of rat myocytes to large concentrations of antibodies caused spontaneous contractions, progressive cell deterioration, and death. Our results thus show that a component of antibodies against ADP-ATP carrier cross-reacts with cardiac sarcolemmal proteins enhancing the Ca2+ channel.

Effect of nimodipine upon electroencephalographic vigilance in elderly persons with minor impairment of brain functions.

10 volunteers (61-78 years) with minor impairment of brain function were given placebo, 40 mg and 60 mg nimodipine (Nimotop) in single doses with intervals of 3 medication-free days between medications under double-blind conditions. Consecutive 2-s epochs of a 10-min EEG record (resting conditions) of F3/C3-F4/C4-P3/01-P4/02 were subjected to FFT (Fast Fourier Transform). Using the information given by the 4 leads for each minute of recording the number of epochs which correspond to a stage A or non-A were determined. In order to evaluate subtle effects on electroencephalographic vigilance within stage A, also the anterior-posterior and left-right relations of absolute alpha-power were calculated. Since previous findings suggested that a nimodipine effect can only be demonstrated when individual differences in the baseline-EEG are considered, the subjects were subdivided in equal subgroups according to the number of non-A epochs under placebo. The rationale for this is the finding that in geriatric patients two electroencephalographically distinguishable forms of dissolution can be observed. Under both 40 mg and 60 mg nimodipine the subgroup with a prevalence of non-A epochs (poor alpha-activity) showed a decrease of non-A epochs; the subgroup with a prevalence of A-epochs (abundant alpha-activity), an increase. Opposite results for the two subgroups were also obtained for the left-right relations of alpha-power over the anterior regions. Under 40 mg nimodipine the subgroup with poor alpha-activity showed a shift to the left; the subgroup with abundant alpha-activity to the right. Under 60 mg nimodipine both subgroups showed a shift to the left. The results confirm findings that nimodipine stabilizes electroencephalographic vigilance at a medium level. Moreover they suggest a dose dependence of the effect, i.e., at higher doses nimodipine seems to act as a sedative.

Visuomotor performance of schizophrenic patients and normal controls in a picture viewing task.

The eye movements of 20 partially remitted schizophrenic outpatients (ICD-9) under neuroleptic maintenance medication and those of 20 normal controls were recorded using corneal reflection pupil-center measurement. The visuomotor performance during a 1-min picture viewing task was studied on the basis of several eye movement parameters. Clinical evaluation comprised self-ratings (Frankfort Complaint Questionnaire) and observer ratings (BPRS, CGI, GAS), as well as recording of current daily neuroleptic dosage (mg CPZ). The main results are that schizophrenics differ from normals in their correlational pattern of fixation- and movement-related parameters, reflecting two opposite viewing styles in schizophrenics: staring and extensive scanning. Both styles are differently related to clinical symptomatology. There was no strongly marked relationship with neuroleptic dosage.

Interindividual variability of lithium-induced EEG changes in healthy volunteers.

A placebo-controlled, double-blind study on the effects of lithium on the electroencephalogram (EEG) was performed in 20 healthy volunteers. A reanalysis of the data which previously had been evaluated by factor analysis and other techniques is presented. The aim of this reanalysis is a quantitative reconstruction of visually defined vigilance stages. In 8 of the 10 healthy subjects who received lithium, a "stabilization" of vigilance on a slightly lowered level (i.e., increased continuity of alpha activity and a tendency in the direction of increased alpha anteriorization) could be demonstrated after a 2-week medication period (mean lithium plasma level: 0.65 mmol/l), indicating that in these subjects lithium counteracts the physiological decline of vigilance to be expected under resting conditions. The interpretation of this finding as a reduction of physiological variability, or as a "dynamic restriction," corresponds to findings in other areas of lithium research which can be interpreted in an analogous manner. In two subjects, however, a small decline of vigilance was observed. These two subjects showed an especially pronounced alpha rhythm in their predrug EEG compared with the other volunteers. The authors discuss whether the delineation of different EEG reaction types contributes to the responder problem in lithium research.
