8/21 translocation in acute myeloid leukaemia.

8;21 translocation was found in 10 AML patients. These patients exhibited a distinct clinical and haematological picture, characterized by M2 bone marrow, with rather good maturation, a high count of mature granulocytes, splenomegaly, and the absence of DIC. Complete remission as easily obtained. It was reported that the median survival is better than for other AML patients with abnormal karyotypes, but this could not be substantiated in our small series. The loss of a sex chromosome was found to be frequent and of poor prognostic significance.

Chemotherapy-radiotherapy versus chemotherapy in Hodgkin's disease with bone marrow involvement. Retrospective analysis of 32 cases.

A retrospective analysis was performed on 32 patients suffering from Hodgkin's disease with bone marrow involvement treated between 1972 and 1976. After 6 courses of chemotherapy (MOPP for 25 patients), we observed 20 complete remissions (62.5%), 8 lymph node partial remissions (bone marrow sterilization but persistence of pathological lymph nodes) and 4 failures. among the 28 complete or partial responders, 17 received maintenance chemotherapy alone, 9 were submitted to 40 Gy and 2 to 20 Gy irradiations. 15 patients died and 8 relapses occurred - all in previously involved lymph node areas. Survival of complete and partial responders is significantly different (p = 0.01) according to whether they received maintenance chemotherapy alone (34.5%) or irradiation (75.7%). Disease-free duration is also significantly different (p = 0.05) according to whether the patients received maintenance chemotherapy alone (42.1%) or 40 Gy irradiation (100%).

[Pseudo-lymphomas of the ocular adnexa: their relations to Sjögren's syndrome (author's transl)].

Studies over the last fifteen years, of histological, immunological and hematological parameters has, led to the description of a non-malignant lymphoproliferation: a pseudo-lymphoma which is in the middle of the spectrum between glandular localisation of Sjögren's syndrome and frank neoplasia. This type of proliferation has been also observed in the conjunctiva, without antecedents of Sjögren syndrome. Histologically and immunologically, it is dysimmunitary process which has the potential to regress with appropriate immunosuppressive therapy, or to progress to malignant lymphoma.

[Malignant lymphoplasmocytic lymphoma of the orbit and conjunctiva (author's transl)].

New functional classifications of malignant lymphomas have demonstrated the relative frequency of malignant lymphoplasmocytic lymphoma of the orbit. The tumors, which show the particular characteristic of secreting monoclonal immunoglobulins, and which may follow a Sjögren's syndrome or a pseudolymphoma, could represent a localized form of Waldenström's disease. They have a relatively favorable prognosis, however, if recognized at a sufficiently early stage.

[Multiple chemotherapy (MOPP) followed by either focal or selective radiotherapy in clinical stages IA and II2A of Hodgkin's disease: results after four years of the use of prospective schedule (H 7701) in 79 patients (author's transl)].

Seventy-nine patients with clinical stages IA, or II2A of Hodgkin's disease were treated from January 1977 to April 1980 by a multiple therapy schedule, H 7701. Three courses of MOPP chemotherapy were first given to all patients. They were then randomly allocated to two groups: group 7701 S (38 patients) was treated by mantle irradiation, excluding the mediastinum when this was not initially involved, or inverted Y radiotherapy; group 7701 F (41 patients) was treated by focal irradiation only. After follow-up for 9-48 months (median: 26 months), overall survival was 98,6 p. cent and relapse-free duration 94,9 p cent. No statistical difference exists between the two randomized groups. Four patients relapsed; three are now free of disease after further treatment, while one patient has since died. With this chemotherapy-radiotherapy selective of focal sequence, staging laparotomy is not indicated. Results and side effects of this treatment schedule are compared with those of other treatment strategies.

[Orbital immunoblastosarcoma (author's transl)].

The new classifications adopted for lymphoid tumors now include a particular type of proliferative process: immunoblastosarcoma. This tumor is not a rare occurrence in the orbit, as shown by the two cases reported, and possesses sufficiently specific clinical and histologicas characteristics to justify its appearance in a separate class. The cytological and histological characteristics of the immunoblasts from which it develops are described. Tumors of this type were previously mostly included in the group of reticulosarcomas, now become obsolete because of poorly defined characteristics of the reticular cell. The prognosis is poor in spite of the chemotherapeutic and radiotherapeutic treatments proposed.

Angiofollicular and plasmacytic polyadenopathy: a pseudotumourous syndrome with dysimmunity.

Two cases are reported of an apparently distinct type of immune disorder. Beginning with mild anaemia and widespread massive lymphadenopathy, the disease progressed to a fatal autoimmune type haemolytic anaemia. Serum investigation showed polyclonal hypergamma-globulinaemia and some autoantibodies. Repeat lymph node biopsies in each case showed hyperplasia within B lymphocyte territory (follicular hyperplasia and polyclonal plasmacytosis with IgG predominance) and atrophy of T dependent areas. Dilatation of lymph sinuses, vascular proliferation, and sclerosis were striking features. This appears to be a new entity, and reasons are given for separating this disease from other pseudotumourous lymph node disorders associated with dysimmunity.

[Visceral Kaposi sarcoma with angioimmunoblastic lymphadenopathy and latent sicca syndrome (author's transl)].

A case clinically and morphologically typical angio-immunoblastic lymphadenopathy in a 64-year old man is reported. A sicca syndrome was discovered with histological localization in the labial accessory salivary glands. Death occurred after a course of a few months. The autopsy showed an association of angio-immunoblastic lymphadenopathy lesions of the bone marrow and the lymph node and of typical multivisceral localizations of Kaposi's sarcoma (lymph node, liver, thyroïdd gland) without skin involvement. This association of two diseases characterized by severe dysimmunity with B lymphocytes hyperplasia is discussed, and compared to the others diseases which could be associated with Kaposi's sarcoma.

[Hairy cell leukemia in the spleen (author's transl)].

Fifteen spleens with hairy cell leukemia are studied with immunofluorescent (7 cases), electron microscopic (5 cases), and usual histologic methods. The findings are: enlargment of the spleen (weight always superior to 400 g), diffuse red pulp infiltration by ambiguous cells with regular repartition of nuclei and clear spaces between them, hairy aspects of the cytoplasmic membrane which are especially observed on semi-thin and ultra-thin sections, presence of particular cytoplasmic inclusion bodies (polysome lamellae complex). The cellular infiltration is accompanied by some vascular modifications: pseudo angiomatosis lesions, nodular formations ressembling splenomas. An important hypertrophy of the splenic macrophages with erythrophagocytosis and siderosis is also observed. These two phenomena partly explain the anemia.

[Morphologic manifestations of Waldenström's disease].

The authors describe the various cytological and pathological lesions found in lymph node, spleen, bone marrow and liver in Waldenströms macroglobulinemia. The significance of the discovery of PAS positive intranuclear vacuoles is discussed. The findings are completed by a description of the results obtained in immunofluorescence and under the electron microscope. Unusual forms and the differential pathological diagnosis are then discussed.
