Neutrophil chemotaxis: the kinetics of cellular locomotion in vitro.

The Boyden chamber system has been used to study the motile response of rabbit peritoneal neutrophils to partially purified casein cytotaxin. The results of these in vitro experiments indicate that neutrophils respond in two distinct ways to cytotaxin. Firstly, they increase their innate random mobility. Secondly, they respond to the information contained in a cytotaxin concentration gradient by exhibiting directional movement with a velocity vector directed towards the higher cytotaxin concentration. Since the two responses are kinetically distinct, one may infer that they are mechanistically distinct.

Double-blind trials of clemastine ('Tavegil') in allergic rhinitis.

Two double-blind randomised trials are reported comparing the effectiveness of the antihistamines, clemastine and chlorpheniramine in comparable doses, in relieving the symptoms of allergic rhinitis. In the first trial, treating 58 adults seen in a general practice, both drugs were prescribed in tablet form; l mg. clemastine b.d. and 4 mg. chlorpheniramine b.d. The second trial was carried out in 42 patients attending a children's E.N.T. out-patient department and the drugs were prescribed as clemastine elixir (0.5 mg. b.d.) or as chlorpheniramine syrup (2 mg. b.d.). Both drugs were effective in providing symptomatic relief in a significant number of patients, and the overall efficacy of clemastine was marginally better than that of chlorpheniramine, especially in the second trial in children. Side-effects were minimal and drowsiness was not a problem.

General versus epidural anesthesia for femoral-popliteal bypass surgery.

This study examines whether epidural anesthesia is more effective than general anesthesia using an inhalation agent in controlling cardiovascular responses during femoral-popliteal bypass surgery. Nineteen patients were randomized into two groups: general anesthesia (n = 10) and epidural anesthesia (n = 9). The patients who underwent general anesthesia received sodium pentothal and succinylcholine for induction of anesthesia and 60% N2O, 40% O2, and 1% to 1.5% isoflurane for maintenance. Fifteen minutes before extubation, the patients received morphine sulfate 0.05 mg/kg intravenously (IV). The group that underwent epidural anesthesia received anesthesia to T-10 (through a catheter placed in the L4-5 interspace using 3% 2-chloroprocaine). Thirty minutes after the last dose, 0.05 mg/kg IV was administered. Hemodynamic variables were recorded at selected intervals during the operation and for 60 minutes in the recovery room. In the general anesthesia group, mean arterial pressure (MAP) and rate pressure product (RPP) significantly decreased (p less than 0.05) during the operation as compared with preoperative values. Following intubation and skin incision, 5 minutes after extubation, and after 60 minutes in the recovery room, MAP, heart rate (HR), and RPP were significantly greater (p less than 0.05) as compared with intraoperative periods. In the epidural anesthesia group, there were clinically important decreases in MAP and RPP after reaching T-10 and skin incision. The general anesthesia patients showed higher MAP, HR, and RPP 5 minutes after extubation and after 60 minutes in the recovery room. Epidural anesthesia patients showed stable hemodynamic patterns throughout the study.(ABSTRACT TRUNCATED AT 250 WORDS)

Acute hemorrhagic edema of childhood (AHE)

Four patients with acute hemorrhagic edema (AHE) of childhood are presented. Two patients had associated pulmonary tuberculosis. AHE of childhood is a benign condition, characterized by inflammatory edema and ecchymotic purpura in a target or cockade pattern. Histology of the skin lesions shows typical leucocytoclastic vasculitis and the condition has therefore been considered a variant of Henoch-Schönlein purpura (HSP). However, because of its benign course, AHE should be considered as a separate entity.

Reflective practice and guided discovery: clinical supervision.

This article explores the parallels between reflective practice as a model for clinical supervision, and guided discovery as a skill in cognitive psychotherapy. A description outlining the historical development of clinical supervision in relationship to positional papers and policies is followed by an exposé of the difficulties in developing a clear, consistent model of clinical supervision with a coherent focus; reflective practice is proposed as a model of choice for clinical supervision in nursing. The article examines the parallels and processes of a model of reflection in an individual clinical supervision session, and the use of guided discovery through Socratic dialogue with a depressed patient in cognitive psychotherapy. Extracts from both sessions are used to illuminate the subsequent discussion.
