The influence of medical drugs and X-rays upon early pregnancy causes problems of great responsibility for the physician. He is supposed to grant optimum consultation to the anxious women who finally have to decide to carry on their pregnancy or not. This topic is difficult to value as controlled prospective studies are not practicable for ethical reasons, and the results of animal experiments are rarely transferable to human medicine. In this paper the facts and data of our consulting activities are summarized from April 1985 to May 1986 in the sense of a prospective study. Over all 64 consultations were performed in these 14 months, up to now 30 women have been delivered of a newborn. The procedure and the principles of our consultations and the postnatal results are presented below.

Hirsutism is a frequent anomaly of the female hair follicles, which produces psychological problems. The pathogenesis, clinical manifestations and metabolism of the androgens are discussed. A way of diagnosis which is practicable for clinical purpose as well as for practice is described. Finally two interesting cases are demonstrated.
In order to classify events of bleeding in early pregnancy (7th-16th week of pregnancy), an ultrasonic examination was carried out on 72 pregnant women, as well as tests establishing the HCG-secretion in the early morning urine and the progesterone level in the serum before commencing hormone therapy. The collected medical evidence was compared with standard curves. In order to test its prognostic validity, it was related to the termination of the pregnancy. In cases of miscarriage, the placenta was examined histologically and the progesterone level decrease registered over 48 hours. In 34 women, the pregnancy ended with a birth; in 26 (76.5%) of these cases the progesterone values were with the normal area. The HCG-secretion was normal in 25 (74.3%) patients. The sonographic classification indicated an intact pregnancy in every case. In 38 cases the pregnancy was disturbed. In this group of patients, the progesterone level in 36 cases (94.8%) and the HCG values in 27 patients (71.1%) were below the normal. Signs of life were absent in 37 pregnancies (97.4%) in the ultrasonic examination. By virtue of the statistical evaluation of this data, a high reliability can be attributed to the sonographic evidence with regard to the correct prognosis for bleeding in early pregnancy.

The purpose of this study was to assess the incidence of congenital heart disease (CHD) in fetuses with premature atrial contractions (PAC) and to check whether these fetuses should be referred for a special fetal echocardiogram. Out of 120 fetuses with PACs and 75 fetuses with CHD both conditions were present in only 9 fetuses (= 7.5%). Two pregnancies were terminated because of associated severe fetal anomalies. After birth PACs disappeared in all other cases spontaneously. Despite higher numbers, the incidence of fetal CHD was not statistically significant higher in fetuses with PACs. Thus, recognition of fetal PACs should lead to a referral for a special fetal echocardiographic examination.
In contrast to IVF, in ICSI the surrounding cumulus and corona cells must be removed completely, as only denuded oocytes can be successfully manipulated by the holding pipette. This ancillary effect of ICSI allows us to focus on the morphology of preimplantation development from the earliest stages. Early prognosis regarding the developmental fate of oocytes would help to limit a negative impact of culture conditions. However, little evidence is available that non-invasive selection at the oocyte stage (first polar body, granular cytoplasm) may be of prognostic value. Recently, certain patterns of pronuclei (number and the distribution of nucleoli) at the zygote stage were found to correlate with treatment outcome in IVF and ICSI cycles, offering an additional prognostic tool prior to cleavage. As there is evidence that embryo selection on day 2 or 3 based on morphological criteria (fragmentation, number of blastomeres, multinucleation, uneven cleavage) may be imprecise, patients might benefit from extended embryo culture to day 5. However, not all major chromosomal aberrations are incompatible with blastocyst formation, and prolonged culture in vitro does not exclusively select embryos with a normal chromosomal complement. Consequently, special care should be taken to minimize the presence of aneuploid concepti in culture. In addition, multiple selection at different stages of development will be required to filter out the correct 'candidate' embryo which will result in a healthy newborn.
Continuous forward flow to the fetal heart in the umbilical vein is a normal Doppler finding. Altered fetal hemodynamics can cause a pulsatile flow pattern in the umbilical vein. Pulsations in the umbilical vein were diagnosed in 14 pregnancies complicated by fetal hydrops, cardiac malformations, arrhythmia or severe intrauterine growth retardation. To document the fetal outcome, the results were analysed retrospectively. Compared with a normal Doppler group (N = 56), a significantly higher rate of perinatal death (Alpha < 1%), (64% vs 1.75%) was diagnosed, when pulsations in the umbilical vein were present. The Apgar-score was significantly lower (4.5 vs 8.4) (p < 0.0002) in the group with pulsations in the umbilical vein. There was no significant difference of pH between the newborns of the two groups. Eight fetuses developed hydrops. Thirteen fetuses had increased reverse flow in the inferior vena cava. The vena cava of the acardiac fetus could not be identified. The knowledge of the poor outcome and the pathophysiologic relationships of fetal hemodynamics may be useful in clinical management. Therefore Doppler examination of the umbilical vein should be performed in high-risk pregnancies.
The case report describes the pre- and postnatal clinical and sonographic findings of a fetus with spontaneous constriction of Ductus arteriosus at 39 weeks gestation. An abnormal four-chamber view due to a hypertrophied and enlarged right ventricle, increased peak systolic (2.39 m/sec), increased peak diastolic (0.88 m/sec) velocities and the decreased pulsatility index of 0.92 were the diagnostic criteria. The flow of the right pulmonary artery was retrograde. After birth estimated pulmonary pressure was 50 mmHg. It is concluded that constriction of the fetal ductus arteriosus may also occur spontaneously in the absence of maternal drugs.
An open comparative trial studied the action of celiprolol and metoprolol (oral) on hemodynamic parameters during parenteral long-term tocolysis with 0.1 microgram/min of hexoprenaline. The observation period covered the critical first phase of tocolysis of 48 hours. No statistically significant difference was found between the two treatment groups, but metoprolol showed a more beneficial influence on the undesirable hemodynamic effects of the tocolysis, as fall of the blood pressure and increase of the heart rate. The ISA and an additional vasodilative effect together with the beta 1-selectivity of celiprolol are supposed to be the cause for this statistically not significant difference. We think that beta-blockers with a clinically relevant ISA are less suitable for the antagonization of the cardiovascular effects of the beta 2-stimulation than such without an intrinsic activity.
