[Multicystic dysplasia of the kidney in the adult].

Two cases of multicystic renal dysplasia in the adult are presented and the pathogenic theories of the condition are reviewed. The more severe defects of development are those which more often remain clinically silent, until are detected, usually in a casual manner, in the adult age. Palpable mass or, more often, traditional radiology, densito-tomography and echography allow differentiation from other conditions such as polycystic kidney, tumors, tuberculosis, hydronephrosis. Anyhow, in the Author's opinion, surgical exploration is mandatory for definite diagnosis as well as for exeresis of the dysplastic mass.

"In vitro" and "in vivo" leukocyte migration inhibitory factor production in acute infectious mononucleosis patients.

Since infectious mononucleosis (IM) mononuclear cells spontaneously release the leukocyte (migration) inhibitory factor (LIF) in culture and since previous experiments in our laboratory have demonstrated that one or more substances with LIF-like activity are detectable in the serum (serum LIF) of young patients with heterophile-positive IM, an investigation was carried out to determine both the in vitro and in vivo LIF production in a series of IM patients sampled during the acute phase of the infection. The observation than only the unstimulated lymphocytes from serum LIF-positive IM patients released LIF in culture suggests that a single active product is involved in the in vitro and in vivo LIF activities observed in Epstein-Barr virus (EBV) infected subjects. The biological implications of this finding with respect to the underlying immunopathology of the EBV IM syndrome are discussed.

[Sero-epidemiologic studies of acute hepatitis A in Umbria: behavior of IgM anti-HAV antibodies].

The diagnosis of acute Hepatitis A is made when antibodies anti-HAV, of the IgM class, are detected in the blood. The Authors have examined with RIA test (HAVAB-M, Abbott) 298 samples of serum from patients with acute non B hepatitis, non drug abusers, also they had not received blood transfusions; as control they examined 41 sera from patients with hepatitis B, 29 sera from patients with non B hepatitis and drug addiction, 25 sera from patients with transfusional non B hepatitis and 37 sera from healthy persons. From the first group (298 patients) 47% were positive for anti-HAV antibodies IgM class; the greatest incidence of positivity (80%) was detected in the class age 0-10 years. In the control groups only in one serum positivity was evidenced and the patient was 22 years old, drug addicted with non B acute hepatitis and with previous episodes of acute hepatitis.

Hydatidosis: a first approach for serological standardization.

Hydatid fluids were collected in three continents and used as antigens against both human E. granulosus and E. multilocularis hydatidosis-positive sera collected in the same countries, in ELISA. The sensitivity of the antigens was good for all sera examined and this confirms the existence of common antigens, which is particularly marked in the 0.8M fraction of Italian hydatid fluid.

[Urodynamic assessment in multiple sclerosis and Parkinson's disease].

Twenty five patients with C.N.S. diseases (parkinsonism and multiple sclerosis) have been studied by means of electrocystouretromanometry and electromyography. Cases with patent urological causes of vesico-sphincteric functional impairment have not been included. In multiple sclerosis urodynamic changes, chiefly detrusor hyper-reflexia, have been registered in 75% of cases, EMG sphincter activity disturbances in 85%. Respectively, 90% of cystomanometries and 54% of EMG sphincter activities showed changes in Parkinsonism. The precise assessment of the individual specific urodynamic change seems to allow a rationally better therapeutic perspective.

Serum migration-inhibitory activity in infants with respiratory syncytial virus bronchiolitis.

Using an unidirectional leucocyte migration-inhibition (LMI) assay, we investigated the migration-inhibitory activity (LIF) in the sera of 25 infants who developed bronchiolitis following respiratory syncytial virus (RSV) infection. Blood samples for serum-LIF activity detection were obtained from patients the day after admission and two weeks later. The LMI assay revealed serum-LIF activity in 17 (68%) blood samples taken during the acute phase of the disease. In contrast, no inhibition was found in the convalescent blood samples with the exception of 5 (20%). This difference was statistically significant (chi2 = 9.82; P less than 0.005). As positive serum-LIF test is a good in vitro correlate of cell-mediated immunity, our results suggest that cell-mediated hypersensitivity reactions occur during the clinical course of RSV bronchiolitis. These findings are consistent with the hypothesis that alterations of RSV-specific cell-mediated immune mechanisms may be involved in the pathogenesis of this illness.

[Uro-enteric prostheses: voiding activity and morphological changes of epithelium (author's transl)].

Ultrastructural findings in epithelium of human entero-urinary grafts are presented, five and ten years after intervention. Severity of changes seems to be in relationship with functional defects of grafts, chiefly as voiding is concerned, more than with the duration of implantation and the presence of a sphincteric control. As a matter of fact, as far as an active, regular, complete voiding is obtained, only slight late changes occur in ultrastructure, while flattening of macrovilli and cohalescence of cryptae are early findings. Anyhow, no favorable effect is to be expected from these epithelial changes in order to prevent absorption and/or secretion.

[Levels of specific antibodies against poliomyelitis virus and degree of immune protection in Umbria population].

The Authors studied neutralizing antibodies against poliomyelitis wild viruses in a sample of Umbria population. The number of seronegative results for each virus strain was very low (less than 1%). The geometric mean titres were 21.07, 26.72, 19.13 for poliovirus types 1, 2, 3, respectively. The results of this seroepidemiological study shows that the Umbria population is satisfactory protected against the poliomyelitis.

[Current trend in the medical treatment of oxalic calculosis].

On the basis of to-day knowledge about metabolism and excretion of oxalic acid, the rationale of therapy of stone pathology is revieved. The problems of both primary and secondary oxaluria and of inhibiting factors of cristalisation are particolarly discussed.
