[Experimental studies on the regeneration capability and prognosis of hydronephrosis].

There are conflicting opinions regarding the ability of kidneys to return to normal functioning following removal of an obstruction. Opinions also differ on the precision of radioactive iodine for the measurement of functional ability in obstructed kidneys. We tried to assess the value of nuclear medical methods in comparison to the classical physiological clearance methods. Furthermore, we tried to answer the following questions: how long can an obstruction be present before total recovery becomes impossible and when is it no longer worthwhile to perform conservative surgery?

[Organ-sparing operative therapy of kidney tumors].

In 24 (4.8%) of 503 patients with renal tumors, the growth was located in solitary or residual kidneys, and the overall renal function was diminished. This was the indication used for tumor enucleation or partial nephrectomy. Of the patients with renal cell carcinoma, 75% were free of tumor after a mean follow-up of 27 months (8-65). There were 18 patients with stage 1 (T1, T2, N0, M0) and 2 with stage 2 tumors. Three patients had a angiomyolipoma and one underwent nephrectomy because of a late postoperative complication. This outcome compares favorably with the results of radical nephrectomy in cases with normal kidney function. In conclusion, enucleation or partial nephrectomy is justified in patients with impaired renal function and may be considered in patients with T1, G1 tumors in normal functioning kidneys.

[Ureteral reimplantation using the psoas bladder-hitch. Experience based on 111 operations in 100 patients].

In a 12-year period we performed 111 ureteral reimplantations with vesico-psoas hitch in 100 patients. The indication was a stricture of the lower third of the ureter in 51 ureters, vesicoureteral reflux in 31 ureters, and intraoperative laceration of the ureter in 25 patients. A congenital megaureter was the indication for ureteral reimplantation with psoas hitch in 4 cases. The operative result was classified excellent or good in 72% of the cases, and only 16% had to be regarded as operative failures. Ureteral reimplantation with vesico-psoas hitch is a versatile procedure that suits a number of indications, it is invaluable for the replacement of the lower third of the ureter in difficult cases, and it leads to a good long-term result in a high percentage of patients.

Intravesical ultrasound tomography in staging bladder carcinoma.

The staging of bladder carcinoma is most important for treatment and prognosis. Transurethral ultrasound scanning was done with a rotating 6 MHz. transducer interchangeable with the optic of a 24F cystoscope. Intravesical ultrasound tomography and computerized tomography were used to treat 28 patients with bladder carcinoma. The correlation between ultrasound and histopathological findings showed that local staging of bladder carcinoma is possible by transurethral scanning.

[Application possibilities for Neodym-YAG lasers in urology].

Because of the still relatively small, inhomogeneous number of cases and the still too short period of observation it has not yet been possible to give a definitive evaluation of Neodym-Yag laser irradiation in the therapy of bladder tumours. However, results to date support the hope that laser coagulation will one day take its place among the methods of transurethral therapy, whereby the combination of transurethral resection and irradiation of the tumour base to destroy intramural nests of tumour cells appears particularly suitable. Our own first results with a selected group of patients are reported. Laser irradiation of benign tumours of the external genitals is a very simple and successful procedure. With carcinomas of the penis or urethra, irradiation is only indicated for very superficial tumours which have not yet metastasized or when partial amputation of the penis is refused by the patient. The successes achieved so far justify the assumption that also here the Neodym-Yag laser will take its place in the range of therapy.

Endoscopic application of the Nd: YAG laser in urology; theory, results, dosimetry.

We present the results of experimental and theoretical investigations on the interaction of Nd; YAG laser light with biological tissue in general, and with the bladder in particular. A comparison is made between the thermal effects achieved with this laser and those of the argon and CO2 types. A therapeutically safe yet effective dosage for the endoscopic treatment of bladder tumours with the Nd; YAG laser is given. The operating techniques are described along with the first clinical results.

[Angiographic and isotopic studies following pole resection of kidneys in dogs and hemostasis performed by infrared-contact-coagulation (author's transl)].

It has been shown in an animal experiment that alterations of the renal vasculature and parenchyma after hemostasis performed by Infrared-Contact-Coagulation are best shown by intravital magnification angiography (magnification factor 2.22). Following pole resection and hemostasis the final morphologic state is reached 6 weeks postoperatively. The resulting scar penetrates into the parenchyma up to 1 cm. The loss of function corresponds to the parenchymal defect. There was no additional loss of renal function or damage to the parenchyma on nuclear medicine or histologic examinations.

[External application of neodymium-YAG-laser in urology (author's transl)].

The preliminary results of Nd:YAG Laser Irradiation treatment of a small group of patients with tumors of the external genitalia are presented; the instruments and operating techniques are discussed. Results from treatment of benign tumors have been excellent, in malignant tumors the result have been encouraging so far. Special advantages of using the laser is the non-contact bloodless treatment and the avoidance of strictures of the urethra and external orifice. The longer healing period after the intensive Irradiation of urethra and penis carcinoma, can be as long as nine weeks depending on the size of the defect.

[Experience with the use of neodymium YAG laser in urology (author's transl)].

Hitherto a conclusive evaluation of neodymium YAG laser irradiation in the treatment of bladder tumors has not been possible because of the relatively few cases, the heterogeneity of the cases and the still too short observation period. However, the results justify the hope that laser coagulation will succeed in establishing a firm place in transurethral treatment, the combination of transurethral resection and radiation of the tumor bed seems to be particularly rational for the destruction of intramural nests of tumor cells. This is also true for the application in penile tumors. It is certain that condylomata acuminata can be successfully treated with neodymium YAG leser in both men and women with lasting results. Experience is still too short to come to a final evaluation in the assessment of the application in carcinoma of the penis and vulva.
