Differences in sister chromatid exchange (SCE)-induction in vivo by cyclophosphamide in murine strains.

The susceptibility of sister chromatid exchange (SCE)-induction in bone marrow cells by cyclophosphamide (CPA) was tested in DBA/2, AKR, C57BL/6J, C3Hf and BALB/c mice in vivo. Mice were treated with one i.p. injection of 0.4, 2.0 and 10.0 mg/kg body weight of CPA or of saline. The base-line level of SCE was similar in all the strains with about 3 SCE/cell. Increasing concentrations of CPA caused an increased level of SCE. At any dose level, differences in the SCE frequency were observed among the five strains. The greatest difference was seen at the dose of 10 mg/kg, when the DBA/2 strain reached the frequency of 25.5 SCE/cell, whereas 19.3, 12.9, 11.4 and 10.1 SCE/cell were seen respectively in AKR, C57BL/6J, C3Hf and BALB/c mice.

[Single vessel coronary disease. I. Differences in clinical and prognostic aspects and in surgical results in isolated lesions of the anterior descending branch and of other coronary branches].

This study includes 290 patients having a 75% or greater stenosis of a sinlge coronary vessel, divided into two groups: 205 cases with isolated lesions of the left anterior descending coronary artery (LAD) and 85 with a stenosis of the circumflex (CF) or of the right (RCA) coronary artery. The following data have been compared in the two groups: -- characters of angina; -- results of stress testing; -- extent of left ventricular contraction impairment; -- natural history of unoperated patients; -- surgical risk; -- long term survival of operated patients; -- effect of medical or surgical treatment on symptoms. Results were as follows: -- LAD patients had slightly more severe symptoms and lower exercise tolerance than CF and RCA patients; -- no significant differences were noted as regards left ventricular contraction; -- five year survival rates were only slightly different both regarding unoperated patients (80 +/- 5% survival in LAD disease group; 86 +/- 5% in CF and RCA disease) and operated cases (83 +/- 5% in LAD lesions, 86 +/- 7% in CF and RCA disease); -- surgical risk was relatively low in both groups; -- progress of symptoms after bypass surgery was very favourable. Based on these results, indications for surgery in single coronary vessel disease are discussed.

[The significance of ventricular arrhythmias during muscular work: correlations with coronary heart disease (author's transl)].

The incidence of ventricular arrhythmias during muscular work in 400 patients hospitalized for clear or suspected coronary artery disease who underwent coronaroangiography and exercise test was studied. The correlations between the arrhythmias and some hemodynamic parameters and the coronaroangiogrphy patterns and left ventricle cineangiography were investigated. None of the factors that were supposed to be significant in the mechanism of the ventricular arrhythmias, such as high left ventricular end dyastolic pressure, modified myocardial contractility, previous myocardial infarct, or higher lesions of coronary arteries, gave significant correlations. In the present study, the ventricular arrhythmias during muscular work do not seem to be of diagnostic significance.

[The effects of verapamil on angina threshold during exercise (author's transl)].

The efficacy and duration of the effects of verapamil on angina threshold during exercise were examined in five patients with proved angina pectoris, with the experimental model of a latin square 5 X 5. The effects of verapamil (0,1 mg/kg i.v.) were compared with those of a placebo (20 ml saline i.v.), of isosorbide dinitrate (5 mg, sublingual), of propranolol (0,1 mg/kg i.v.), and of dipyridamol (20 mg i.v.). All the treatments were studied within 20 minutes after administration; the efficacy of the "drugs" was checked against placebo. Verapamil and isosorbide dinitrate significantly prolonged the duration of exercise and total amount of work done before angina and EKG alteration appearance. Similar changes were observed after propranolol, but not significantly different from the effects of placebo. Dipyridamol reduced thw work performance, without significant difference from placebo. The effects of verapamil, though less marked than its peak effects, were still present 70 minutes after administration. The possible mechanism of work performance improvements induced by verapamil in angina pectoris is briefly discussed on the basis of the Triple Product changes observed during exercise.

[Use of atrial pacing in diagnosis of coronary insufficiency. Comparison between coronarography and exercise test (author's transl)].

--123 atrial pacings (AP) performed as diagnostic investigations in patients with chest pains were re-examined. By using floating catheter without fluoroscopic control, this technique is very simple to perform and free from relevant risks. The diagnostic sensibility and specificity of AP were examined in 93 patients in which a coronary arteriography was performed; these figures were compared with the corresponding values observed in 65 patients in which an adequate diagnostic exercise test (ET) was also available. The diagnostic sensibility of AP examined in 63 patients with significant coronary artery disease was 90%; the corresponding value of ET was 79%. In particular, in patients with single vessel disease, the sensibility of AP (90%) was much higher than that observed in ET (40%). The specificity of AP examined in 30 patients free from significant stenosis of the coronary arterial tree was 43%. This value was largely lower than that observed in ET (82%) in the same patients, and appears to be inadequate for validation AP as a diagnostic tool in coronary heart disease. Therefore, AP must be limited to functional, and not diagnostic, evaluation of patients in which the diagnosis of coronary heart disease can be made by other means.

Sister-chromatid exchanges in lymphocytes and mutagenicity in urine of nurses handling cytostatic drugs.

Sister-chromatid exchanges (SCE) in peripheral blood lymphocytes and mutagenicity of urine (Ames test) were measured in a group of 21 nurses professionally handling antineoplastic drugs and in a group of 21 unexposed controls. No differences in SCE frequencies and in urinary mutagenic activity between exposed and unexposed groups were detected. A clear positive increase in urinary mutagenicity in the TA98 Salmonella strain was observed with increasing number of cigarettes smoked, whereas no evident influence of smoking on SCE was seen. Age, coffee and alcohol consumption did not show any detectable effect in the two tests.

Limited role of TP53 and TP53-related genes in myxoid liposarcoma.

AIMS: Circumstantial evidence suggests that genetic changes may lead to tumor progression within the myxoid liposarcoma tumors (MLTs) carrying non-random chromosomal translocation t(12;16). METHODS: To address this subject an immunophenotypic analysis, applying antibodies against proteins encoded by TP53, MDM2 and CDK4 genes, complemented by molecular analysis of eight suitable cases, was performed on 104 consecutive cases. Chromosomal translocations were assessed either by cytogenetic analysis or by RT-PCR in 9 suitable cases and chimeric transcripts were found in all cases but two pleomorphic liposarcomas. RESULTS: Based on immunophenotyping and tumor site, the case material consisted of three groups. The first one was made up of 92 non-retroperitoneal cases carrying a null p53, mdm2, cdk4 immunophenotype, which remained unchanged over the time of recurrences and along the gamut of histologic subtypes. The second group was represented by five p53+, mdm2-, cdk4- non-retroperitoneal cases, 4 of which were further analysed by PCR-SSCP for p53 mutation. The immunophenotypic profile of these cases, complemented by the molecular findings, supported a role of TP53 in tumor progression in three high-grade MLTs. The third group, consisting of 7 retroperitoneal cases, showed a heterogeneous immunophenotype, sharing immunophenotypic and molecular features with the well-differentiated/evoluted (dedifferentiated) liposarcoma group. CONCLUSIONS: TP53 mutations seem to play a role in tumor progression in a few cases of MLTs (2.8%) showing more aggressive histologic characteristics. The unexpected finding that a number of retroperitoneal LMTs display the immunophenotypic profile of the well differentiated/evoluted (dedifferentiated) liposarcomas, deserves further investigation.
