Previous comparisons between presenile Alzheimer's disease (AD) and senile dementia of the Alzheimer type (SDAT) did not control for disease severity and duration. In the current study, 18 patients with each diagnosis were matched for disease duration, cognitive dysfunction, and behavioral symptoms (using the modified Mini-Mental Status [mMMS] examination and the Blessed Dementia Rating Scale [BDRS] ). Regional cerebral blood flow (rCBF) was quantified by the 133xenon inhalation technique, and several indices of tissue perfusion were examined. The two variables of primary interest were relative gray-matter relative weight (35% in presenile patients versus 39% in senile patients and healthy control subjects, p = 0.006), with neither perfusion nor disease severity differences between the two dementia samples. This loss of gray matter was significantly related to both severity and duration of disease in the patients with presenile AD, but not in patients with SDAT. These findings lend support ot previous suggestions of greater degenerative process in presenile AD and confirm the need to examine and control age of onset in future investigations of AD. Further, correlation analysis suggests greater proportion of common variance among clinical and physiological indices in presenile AD.
The protease inhibitor, leupeptin, has been previously shown to cause an accumulation of lysosomally associated intracytoplasmic dense bodies resembling lipofuscin when administered intraventricularly to the brains of young rats (Ivy et al., Science, 226:985, 1984). These findings support the idea that lipofuscin formation during normal aging involves perturbed proteolytic degradation. In the current study, we delineate more precisely the proteolytic mechanisms in question and examine the effects of protease inhibition on other tissues and species. Four sets of experiments were done. In the first, rats were administered leupeptin (L; an inhibitor of cysteine and some serine proteases), E-64C (a cysteine protease inhibitor), chloroquine (C; a lysosomal enzyme inhibitor), aprotinin (A; a serine protease inhibitor) or physiological saline (S) into the lateral ventricle of the brain at a constant rate for two weeks using an osmotic mini-pump. In the second set, beagle dogs were administered L or S intraventricularly in a similar fashion. In the third set, young rats received intraocular injections of L, E64C, C, A or S from 1 to 9 days at 24 hr intervals. In the fourth set, young rats and mice were administered L, E-64C or S intraperitoneally via an osmotic mini-pump for 2 or more weeks. The tissue of all animals was processed for light and electron microscopy as well as for fluorescence analysis. In rat brain cells (especially hippocampus and cerebellum), substances which by morphological, histochemical and fluorescence emission criteria resemble lipofuscin, accumulated following L, C or E64C, but not A or S treatment. A similar buildup of lipofuscin-like substance occurred in canine brain following L but not S infusion. In retinal pigment epithelial cells, undigested rod outer segment discs accumulated after L, C or E-64C and possibly A, but not after S injections. With increasing survival times, the accumulated substances became morphologically more similar to lipofuscin. Cells of liver (both hepatocytes and non-parenchymal cells) and kidney (mainly renal tubule cells) also accumulated substances resembling lipofuscin in response to L but not S infusion; other internal organs were differentially affected. These results demonstrate 1) that specific inhibition of cysteine proteases is sufficient to cause accumulation of lipofuscin-like substances in brain and retina and 2) that several species and organ systems are similarly affected by protease inhibition. Taken together, these findings are consistent with the idea that defective or decreased proteolysis by lysosomal cysteine proteases is responsible for lipofuscin accumulation during normal aging.

Four current models for the parasitic phase of the Ostertagia circumcincta life-cycle were evaluated with respect to their ability to represent the outcome of experimental infection studies and the population biology of the parasite in the field. Neither of the two discrete-time models was able to mimic the rise and fall in parasite numbers that characterize trickle infection experiments. When the infection rate is constant, both models predict that parasite numbers will eventually reach an asymptotic equilibrium value. This happens because both models are formulated such that parasite mortality is constant when the infection rate is constant. The two continuous-time models are able to mimic trickle infection experiments because both of them represent parasite mortality as an increasing function of the infection rate and the duration of infection. However, the continuous-time models do not adequately represent the demography of the parasitic phase in the field because neither of them takes any account of the effect of variations in infection rate from host to host on the overall mean parasite death rate.

Social psychological interventions have been successful in preventing drug and tobacco use in adolescents, but target audience involvement has not been well documented. Examination of program acceptance is necessary to avoid ambiguity when interpreting findings within and across studies. This report presents results from a program acceptance study of a seventh-grade smoking/drug-use prevention and cessation field trial. The research design is an assessment of two experimentally manipulated variations in program delivery on the program acceptance of all provider and target groups involved. Experimental manipulations included classroom and mass broadcast television demonstrations of social resistance skills against pressures to smoke and use drugs. Three aspects of target audience program acceptance were assessed: participation, satisfaction, and perceived program efficacy. Respondents included the target audience (seventh-grade students and parents), instructors, classroom observers, and school administrative staff. Strong main effects of television delivery on student and parent participation were observed. The findings suggest the usefulness of student homework assignment to view television segments with parents at home as a strategy to achieve family involvement in school-based programs. Both television and classroom delivery separately demonstrated significant positive effects on overall program acceptance with moderate interactive effects on perceived program efficacy. While classroom teacher/observers and school administrators reported strong preferences for the experimental (social resistance) classroom curriculum, acceptance of experimental and comparison (information-based) curricula by the student/parent target audience was equivalent.
Any inhalation study that investigates environmental tobacco smoke, ETS, is considering a very complex entity. ETS contains numerous chemicals that are continuously changing both in their absolute concentration, in the ratio of concentration between one and another, and even in their particulate to vapour phase distribution. Moreover, when considering ETS in real-life situations, many of the chemical components of ETS will be present as a result of sources other than tobacco smoking. This paper emphasises that the chemical and physical nature of ETS must be considered in the design and interpretation of any inhalation study on ETS, and illustrates the difficulty in defining precisely what constitutes ETS in such studies.
The development and mortality of the eggs of Ancylostoma duodenale and Necator americanus in distilled water were monitored over a range of temperatures between 15 and 35 degrees C. Egg demography was examined within the context of a four-parameter mathematical model of development and mortality. Over the range of temperatures studied, egg mortality (mu) was an increasing exponential function of temperature (T) measured in degrees Celsius. A single model adequately described the mortality of both species (In [mu] = 0.41*T-6.87). The minimum time (tau) to hatching was consistently less for A. duodenale (In [1/tau] = -(0.79 + 53.05*[1/T]) than N. americanus (In [1/tau] = -(0.99 + 53.05*[1/T]]. The hatching rate (sigma when t greater than tau) was an increasing function of time and temperature in both cases, but the precise functional relationship was species specific.

The relation between refractive error and visual acuity has been measured by two very different methods. In one called "source methods," emmetropes or corrected ametropes view defocused stimuli presented on projection screens or photographs. In the type called "observer methods," focused stimuli are presented to the observers who are either uncorrected ametropes or emmetropes defocused by lenses placed (usually), in the spectacle plane. The study reported in this paper demonstrates for the first time that these two methods of defocusing retinal images and their effects on visual acuity can be correlated. Results show that the source method of producing defocus could be used interchangeably with the observer method in investigating the rates of change of visual acuity with defocus for young normal observers. The angular diameter of the defocused image of a point, the blur disc diameter in object space, allows the two methods to be compared. Although the results show that the two methods are highly correlated, they show that the source method gives a statistically but not clinically significant lower acuity. The results of both methods are used to derive an equation linking refractive error, visual acuity, and pupil diameter.
The defocus levels required for normal observers to notice the first perceptible blur of a clear test target (blur threshold) and the least perceptible change in the degree of blurriness of an already blurry target (threshold of perceived change in blur) were measured using both the source and observer methods. In the source method observers viewed defocused stimuli presented on a projection screen, whereas in the observer method focused stimuli were presented to observers who were defocused using lenses placed in the spectacle plane. Blur thresholds were found to be dependent on target size and when the Landolt ring targets were near threshold acuity size blur thresholds were as small as 0.10 D. For larger target sizes (0.6 log min arc or more above threshold acuity size) the blur thresholds remained relatively unchanged and were about 0.18 D. Thresholds of perceived change in blur were found to be independent of the initial defocus level. Measurements of the threshold of perceived change in blur were found to be 0.05 to 0.07 D, which is much smaller than the blur threshold values. Comparison of results from the two methods of producing defocus indicate that the source and observer methods can be used interchangeably. However, for the same angular blur disc diameter, the blur thresholds found with the source method were significantly lower than those found with the observer method.

The London Based Children's Research Unit (CRU) has investigated juvenile smoking initiation and advertising in 15 countries. Each country investigated differed with respect to tobacco advertising restrictions, ranging from Norway, which has had a long-term ban on all advertising of tobacco products, to Hong Kong and Japan, which had few advertising restrictions at the time of the research. Advertising was not found to significantly influence juvenile smoking initiation and it was also found to be a poor predictor of juvenile smoking prevalence. That is, countries with tobacco advertising did not have a higher level of juvenile smoking prevalence than countries without advertising.
The absorption of morphine sulphate in a controlled-release formulation was studied in 12 patients who had undergone unilateral inguinal hernia repair under a light general anaesthetic and ilio-inguinal block. Serum morphine concentrations were measured serially and gastric emptying was assessed by measurement of paracetamol absorption. Three patients had delayed gastric emptying and impaired morphine absorption in the immediate postoperative period. Four hours later, there was a significant reduction in gastric emptying in eight patients who had normal paracetamol and morphine absorption in the immediate postoperative period.
Various configurations have been suggested for the optical measurement of corneal thickness by slit beam doubling, and corresponding ray traces have been provided to relate actual thickness to apparent thickness. In this report, a set of equations is provided which apply to the general situation in which neither the angle of illumination nor the angle of observation are aligned with or symmetrical about the normal to the cornea. An explicit solution to these equations giving true corneal thickness in terms of apparent thickness cannot be established, and an iterative procedure is used to develop a relationship for a number of different pachometer configurations. The near linearity of these relationships suggests the existence of a simple equation to relate these entities. By making appropriate assumptions, a linear approximation relating the inverse of the true thickness to the inverse of the apparent thickness is developed. An equation for determining the location of measurements of peripheral corneal thickness is also derived. The equations presented here may be generally applied to all optical pachometer configurations, can be used to estimate the relative contributions of various sources of error in pachometry and provide a basis for obtaining uniform absolute derivations of corneal thickness.
