[Antimicrobial activity exerted by sodium dichloroisocyanurate].

Sodium dichloroisocyanurate is a chlorinated cleaner. It was used for swimming pool sanitation and for the sterilisation of linen. Not recently ago sodium dichloroisocyanurate has substituted hypochlorite for the sterilisation of infant feeding bottles and teats. Sodium dichloroisocyanurate is soluble in water; this condition causes the hydrolysis of sodium dichloroisocyanurate in hypochlorous acid, that is the active agent, isocyanurate and isocyanurate chlorine. These compounds form a chlorine protein that carry out microbicidal activity. In a toxicology study has been shown that no severe changes in the normal metabolic function occurred, furthermore sodium dichloroisocyanurate has not shown teratogenic effects at the concentration of 200 mg/kg. The antimicrobial activity of sodium dichloroisocyanurate was evaluated against Gram negative bacteria such as E. coli or Salmonella typhimurium and against some fungi. This study illustrates a rapid antimicrobial activity using concentrations. Our study concentrated on the antimicrobial activity of sodium dichloroisocyanurate in some experimental conditions. We tested 66 strains of fungi, 28 Gram positive bacteria and 29 Gram negative bacteria. We also evaluated the antimicrobial activity of sodium dichloroisocyanurate against protozoa such as Trichomonas vaginalis. The antimicrobial activity was evaluated in cultural conditions and non cultural conditions; in these experiments we observed similar action in both the commercial product and pure substance. In cultural conditions sodium dichloroisocyanurate shows a good activity against fungi and bacteria, moreover it can be observed that the serum didn't interfere with its activity. In a non cultural condition the Candida was killed rapidly by the sodium dichloroisocyanurate but this activity is influenced by the growth phase of the yeast. Against mycelial form such as Penicillium and Aspergillus the sodium dichloroisocyanurate needs a longer contact time than yeast form for its activity. It is interesting to note that well known bacteria, that are resistant to the common antimicrobial agents, such as Pseudomonas aeruginosa, were inhibited by sodium dichloroisocyanurate in a rapid bactericidal action. Our data demonstrates that no significant adverse influence on the activity of sodium dichloroisocyanurate was shown by pH and by temperature even if in some experimental conditions increased activity was noticed at pH = 6.6. The sodium dichloroisocyanurate has demonstrated good activity against Trichomonas vaginalis. This fact extends the broad-spectrum activity of sodium dichloroisocyanurate to the protozoa. In conclusion, sodium dichloroisocyanurate has demonstrated a good activity against all tested strains, furthermore its activity did not decrease in the presence of 1% of organic substance (serum etc.).(ABSTRACT TRUNCATED AT 400 WORDS)

[Transhepatic emergency obliteration of bleeding gastro-esophageal varices (author's transl)].

The hemorrage following gastro-esophageal varices cause a high degree of mortality. Personal experience is presented consisting in 15 patients suffering from cirrosis with inoperable hemorrages. These patients underwent transhepatic emergency obliteration of the varices. The hemorrage stopped in 10 patients, 6 of whom later on underwent surgical treatment with porto-systemical anastomosis. In 5 patients the hemorrage reappeared at various intervals from the procedure. Owing to the possible recanalization of the obliterated varices the procedure should be considered essentially as a preoperative management.

[Complications of wide decompressive craniectomy. Clinical case].

A patient with acute subdural hematoma and multiple cerebral centres of infection, has been surgically treated with an hemicraniectomy. Three months after the operation, the appearance of both a counter-lateral stenic deficit and of a deterioration of the conditions of awareness, has been observed; these problems have been solved thanks to the operation of cranioplasty. The supposed mechanism appears to be that of the alteration of the pressory gradient existing between the environment and the cranial cavity.

[Cerebral abscess as a result of a ventriculo-peritoneal shunt].

The authors relate a rare case of cerebral abscess, complication of a ventriculo-peritoneal derivation. They discuss its etiopathogenesis and underline the importance of cerebral TAC for both diagnostical and post-operatory checking purposes.

[New simplified technic for cranioplasty. Technical note].

The authors report a new simplified technique of cranioplasty in order to repair wide losses of bony substance, particularly useful after wide craniectomies or decompressive craniectomies. They also underline the aesthetic result of such technique which is analogous to the same repair carried out with the patient's bony strip.

[Angio-CT in the staging of renal carcinoma].

The authors report the results about the staging of renal carcinoma using "angio-CT". The diagnostic accuracy, the sensibility, the specificity, in 42 patients, have been evaluated and compared with the subsequent operative or authoptic diagnosis. The meaning of this study is the comparison of the results obtained by this method in front of other examinations, especially artheriography.

[Technical problems in interventional angiography (author's transl)].

Interventional angiography has greatly developed recently, as a consequence of a better definition of clinical indications, the progress and the diffusion of the angiographic techniques and finally as a consequence of the technological improvements of angiographic material and accessories. A synthetic study of the actual technical knowledge and of the perspectives of further improvement, is presented, on the basis of a 10 years' experience, during which more than 500 therapeutical interventions have been performed by means of angiographic techniques.

[Impingement syndrome of the shoulder. Clinical data and radiologic findings].

Subcoracoid impingement syndrome pain is elicited by some positions of the upper limbs, i.e., adduction and inward rotation, whenever coracohumeral space reduces. Although acquired or congenital malformations of the humeral head and/or coracoid apophysis are the most common causes of painful syndromes, repeated flections and inward rotations of the upper limbs, typical of some sports, such as swimming and tennis, and of some sports, such as swimming and tennis, and of some kinds of work, are predisposing factors. The subcoracoid impingement syndrome exhibits on pathogenomonic signs at clinics and the specificity of diagnostic methods is low, which calls for reliable radiologic assessment of this condition. Fifteen patients with subcoracoid impingement syndrome underwent X-ray, US, CT and MR studies. Plain radiography detected no specific signs of this syndrome, but yielded useful information regarding other painful syndromes of the shoulder, such as anatomical variants of the acromion and degenerative changes. US yield was poor because of the acoustic window of the coracoid apophysis, but supraspinatus tendon changes were demonstrated in 2 cases. CT and MRI proved to be the most reliable and accurate diagnostic methods, the former thanks to its sensitivity to even slight bone changes and to its capabilities in measuring coracohumeral distance and acquiring dynamic scans and the latter because it detects tendon, bursa and rotator cuff changes. To conclude, in our opinion, when the subcoracoid impingement syndrome is clinically suspected, plain X-ray films should be performed first and followed by MR scans.

[Magnetic resonance angiography in liver transplant patients. Follow-up of vascular anastomoses].

Twenty liver transplant patients were examined with MRA and color-Doppler US 18 to 40 days after surgery to investigate the onset of vascular complications after surgical liver revascularization. Vascular anastomoses are the most frequent location for such complications as stenoses, occlusions, pseudoaneurysms and vessel ruptures. In liver transplant there are 4 vascular anastomoses, i.e. hepatic artery, portal vein and superior and inferior anastomoses of the inferior vena cava. MRA images were acquired with a superconductive unit operating at 1.5 T, using fast low angle shot (FLASH) 2D sequences on the coronal plane; all images were postprocessed with the MIP algorithm. The presence of a paramagnetic artifact, the "double black spot sign", caused by the suture wire used to make the vascular anastomoses, allowed us to precisely detect the site and the flow pattern alterations at this level. MRA images were studied by two independent observers and vascular anastomosis depiction was rated as "good", "fair" and "poor". The demonstration of portal vein anastomoses was good in the whole series (20/20 patients). The superior anastomosis of the inferior vena cava was clearly depicted in 19 cases and fairly depicted in only 1 patient. The inferior anastomosis of the inferior vena cava was clearly depicted in 19 patients and fairly depicted in 1 case. Hepatic artery anastomoses were far more difficult to demonstrate than the others, considering their caliber and flow pattern, but its depiction was nevertheless good in 12 cases, fair in 6 and poor in only 2 patients. In our series, only one portal vein stenosis was observed, which was clearly depicted on both MRA and US images. In conclusion, MRA is a useful and reliable noninvasive diagnostic tool to study vascular anastomoses in liver transplant patients.

[Magnetic resonance angiography in the study of neoplastic cerebral pathology].

To date, magnetic resonance angiography (MRA) has been used in neuroradiology mainly to study vascular malformations and atherosclerotic changes of the carotid bifurcation. Our study was aimed at investigating the role of MRA with the time-of-flight technique in the study of intracranial neoplasms; a superconductive 1.5 T magnet was used, and FLASH and FISP 2D and 3D pulse sequences were acquired before and after Gd-DTPA administration. Fifty-five MRA examinations were performed. Our series consists in 32 meningiomas, 14 glial tumors, 3 hypophysis adenomas, 2 metastases, 1 NF2, 2 craniopharyngiomas, 1 lymphoma and 1 rhinopharyngeal carcinoma with intracranial involvement. In 27 patients MRA results were compared with DSA findings. The results showed high agreement relative to indirect angiographic patterns (dislocations, encasement, dural sinuses involvement) and poor accuracy in the demonstration of tumor vascularization (inflow and outflow, vascular neoformation).
