Survivin and Cyclooxygenase-2 are co-expressed in human and mouse colon carcinoma and in terminally differentiated colonocytes.

In the evolution of colon rectal cancer (CRC) the imbalance between cell proliferation and apoptosis is considered one of the prominent causes of tumor induction and/or progression. In order to establish the role of anti apoptotic proteins in colon cancer development, we studied with immunohistochemical techniques the expression of Survivin in a mouse model of colon carcinogenesis induced by 1,2-dimethyl-hydrazine treatment. In this mouse model Survivin was over-expressed during tumor development, showing a distribution mimicking that described in the correspondent human malignancies. We also correlated Survivin distribution with COX-2 and beta-Catenin expression patterns. The co-localization of COX-2/beta-Catenin/Survivin in the same epithelial cells in tumor samples lends credence to possible in vivo regulatory effects of COX-2 and beta-Catenin on the intracellular Survivin levels in mouse and human colon cancer.

The feasibility of urethral color ultrasound imaging in the diagnosis of female intrinsic sphincter deficiency: preliminary results.

STUDY DESIGN: Prospective study. OBJECTIVES: To evaluate the feasibility of color ultrasound imaging of the urethra in association with UPP to diagnose intrinsic sphincter deficiency (ISD). SETTING: Italy. METHODS: We studied a group of 13 normal female volunteers (mean age 29 years) during the estrogenic phase and a group of 15 patients (mean age 63.9 years) with ISD. All patients and normal volunteers underwent color ultrasound imaging using a 4-7 MHz convex broad band transducer. Translabial sagittal scans of the urethra were obtained. The color ultrasound parameters were optimized for detection of parenchymal slow flows. A subjective score of the degree of vascularization along the entire urethra was established as follows: (a) minimal or absent (1), (b) poor (2), (c) moderate (3), (d) good (4). Statistical analysis, using the non-parametric Mann-Whitney rank sum test, was carried out to determine differences of ultrasound scores between volunteers and patients. RESULTS: The statistical evaluation showed that the differences between the ultrasound scores in the two groups was statistically significant (P<0.001). CONCLUSION: We affirm that color ultrasound imaging of the urethra seems to be feasible and useful in association with UPP in the diagnosis of ISD even if this echographic investigation needs further observations.

Colour Doppler ultrasonography of female urethral vascularization in normal young volunteers: a preliminary report.

OBJECTIVE: To assess urethral vascularization in healthy young women, using colour Doppler ultrasonography. SUBJECTS AND METHODS: Eleven healthy young women volunteers (mean age 33.6 years, range 24-46) with no pelvic floor disorders and no history of incontinence were assessed. The subjects underwent colour Doppler ultrasonography using a 4-7 MHz convex broadband transducer. Translabial sagittal scans of the urethra were taken when the subjects had a full bladder, both during the oestrogenic and progestogenic phases. The colour Doppler ultrasonography parameters were optimized to detect slow flows in the anterior and posterior distal, middle and proximal urethra. A rank-sum nonparametric test was used to assess differences between the resistive indices. RESULTS: The statistical evaluation showed a significant difference in the resistive index only in the anterior urethra, between the distal and middle plus proximal urethra, in both the progestogenic (P = 0.002) and oestrogenic (P = 0.0127) phases. CONCLUSIONS: This study confirmed that the vascularization of the urethra plays an important role in the 'seal' effect, which is considered one of the most important factors in urethral closure. There was a significantly greater resistive index in the anterior proximal urethra than in the middle and distal urethra. These findings suggest that the seal effect is related to the existence of a rich venous urethral vascularization, involved in the mechanism of urethral closure. Colour Doppler ultrasonography of the urethra seems to be feasible and useful for understanding the mechanism of the vascular component in female continence.

[Termino-terminal urethroplasty using microsurgical technique in the treatment of bulbar urethral stenosis: evaluation of long-term results and preservation of potency].

The Authors report their experience, at long term, on the use of overlap anastomosis sec. Turner Warwick's technique for the treatment of posterior urethral strictures. 12 patients, from 17 to 58 years, were observed. Minimum follow-up was 43.3 months. All patients performed, pre and post-operatively, a retrograde cystogram, an uroflowmetry, a questionnaire to evaluate sexual potency by IIEF and AUA score, and some cases underwent also a cystoscopy post-operatively. Results demonstrated the effectiveness of technique five years later too, which is the period with the greater incidence of recurrences.

Primary cardiac leiomyosarcoma: seven-year survival with combined surgical and adjuvant therapy.

Primary cardiac sarcomas constitute a rare entity that have been uniformly associated with poor long-term survival. A case of left atrial leiomyosarcoma involving the interatrial septum and the right atrial free wall and presenting with syncope and atrial fibrillation, is described. Two extensive surgical excisions followed by adjuvant radiation and chemotherapy improved survival with a good quality of life. This approach of combined surgical, medical and radiation therapy may offer better longterm outcome, since our patient is the longest survivor thus far reported.

Design in Arabidopsis thaliana of a synchronous system of floral induction by one long day.

A system of one-shot induction of flowering in Arabidopsis thaliana, ecotype Columbia, is described. Plants from vernalized seeds are grown for 2 months in 8 h short days at an irradiance of 48 mumol m-2 sec-1 (fluorescent light only). At that age they can be induced to flower by exposure to either a single long day or a single displaced short day. Non-induced plants stay vegetative for at least a further month. Synchrony of induction among the individuals of the population exposed to one long day is of the same order as in the best classical model plants, that is, the fastest individuals are only 6 h ahead of the slowest ones. A further advantage of this system is the large size of plants at the time of induction, allowing easy analysis of changes in leaves, leaf exudate and shoot meristem. The design of such a synchronous system will allow the timings of gene activations and deactivations to be established in the different plant parts, before flowers are initiated.

[Isolated left ventricular myxoma discovered by two-dimensional echocardiography].

We describe the case of a young patient in whom two-dimensional echocardiography, performed because of mild palpitation, discovered the presence of a small mass into the left ventricle, attached to the interventricular septum. The patient was operated upon on cardiopulmonary by-pass, and the mass was removed through the aortic valve, without any complication during the postoperative period. On histology the mass appeared to be a typical myxoma. This case has been reported because the infrequency of ventricular location of the myxoma is unusual, and because the use of 2D-echocardiography, even in the absence of significant symptoms and/or clinical signs, avoided dangerous embolic complications, which often represent the first sign of this neoplastic pathology.

[Intra- and perioperative arrhythmia and ischemic signals in myocardial revascularization patients].

In order to evaluate perioperative electrical cardiac disturbances and ST segment changes, 42 patients (38 M, 4 F, aged 57 +/- 6 ys) were studied using 24-hour Holter monitoring before, during and after coronary bypass surgery. In the 4-6 hours before cardioplegic arrest, 38% of patients had ST segment changes. No patient and malignant arrhythmias. The injection of cold cardioplegic solution was followed by bradycardia, ventricular tachycardia, ventricular fibrillation and isoelectric line within 2-4 minutes. After aortic declamping, 30 patients were defibrillated. Impulse formation and conduction disturbances, found in 55% of patients, solved themselves in 1 to 60 minutes. Bundle branch block continued in just 4 cases. A total of 59% of patients had ST segment elevation for 14 +/- 14 minutes and 19% had ST segment depression for 19 +/- 20 minutes. Successive transient ST segment changes were detected in 38% of patients. Sustained ventricular tachycardia occurred during 2 ischemic episodes. Impulse formation and conduction disturbances were not related to the duration of cardiac arrest or ventricular fibrillation, but were more frequent and lasted longer in patients with incomplete revascularization. Transient ST segment depression far from aortic declamping correlated with preclamping ischemia. Transient ST segment elevation correlated with incomplete revascularization. We concluded that ECG signs of intraoperative damage were reversible. Moreover, perioperative transitory ischemia was frequent but could be prevented by coronary active drug administration. On the other hand incomplete revascularization was associated with electrical disturbances and ischemia.

[Late aortic dissection after aortic valve substitution. Considerations on 5 operative cases].

This study analyses the clinical course of five patients who developed acute ascending aortic dissection 45.8 months (17-87 months) after aortic valve replacement (AVR) and underwent reoperation in our Department. All except one had aortic insufficiency with mild dilatation of the ascending aorta. No evidence of aortic wall degeneration was present at the time of AVR. Prostheses implanted were: Hancock 2, St. Jude 2, Bjork-Shiley 1. Associated procedures (coartectomy, coronary artery by-pass) were carried out in two patients. The dilated aorta was managed at the first operation by longitudinal resection in 3 of them. Emergent aortic dissection repair was performed: in 3 cases with composite graft according to the Bentall procedure, in one case with non valved dacron conduit and in the last case with teflon patch. Histological examination of the dissected aortic wall showed elastic fiber degeneration like cystic medial necrosis. Three patients survived surgical repair and are doing well at 22.4 months of mean post-operative follow-up. Surgical strategy of this complication is discussed; particular emphasis is given to the method of treatment of ascending aorta dilatation during AVR.

[Long-term evaluation of kidney function in Schoenlein-Henoch syndrome].

The Authors studied 104 children (58 males and 46 female) with Schoenlein-Henoch syndrome, admitted to the Department of Pediatrics, Ospedale Civile di Pescara, in the period 1961-1985. Clinical and laboratory aspects were evaluated. Fifty-one subjects (25 males and 26 females) were re-evaluated, after a period of 17.0 +/- 4.2 years after first hospital admission: in particular, the mean blood laboratory parameters and renal function were assessed. None of these 51 subjects showed important abnormalities, especially evidence of renal impairment. This study suggests that the Schoenlein-Henoch syndrome can have a good long-term prognosis, without important renal abnormalities.
