Quantitative bacteriology in wound care.

Quantitative bacteriology in the management of wound sepsis provides an objective way to monitor the success or failure of wound care. The clinical appearance of a wound requiring skin grafting may be misleading. Grafting wounds that appeared clinically ready was unsuccessful when analysis showed bacteriological counts of 10(5) or greater organisms per gram of tissue. A rapid and simple method for quantitative tissue culture is described. Three cases are described illustrating the value of the results of the method.

Semi-automatic data processing in clinical chemistry.

A semi-automatic system for data processing in the Clinical Chemistry Department of the School of Pathology, Johannesburg, is outlined. Careful planning of workflow in the laboratory can provide a better interface of laboratory results with a manual cumulative reporting system in which technologists and clerks co-ordinate their activities. The system used provides a less tedious and less error-prone process via automatic printing facilities. This system achieves improved patient care in terms of quality control (data checks) and cumulative reporting of results. Facility for accounts and administration purpose are also incorporated. In our experience, semi-automatic data processing can provide a solution to the clerical problems generated by the introduction of automatic testing equipment. Hardware costs are minimal in comparison to fully automated systems, which utilize on-line or off-line computers.

Bence Jones myelomatosis and intravenous pyelography.

The consequences of intravenous pyelography are described in 3 patients with unsuspected myelomatosis. One patient died, and 1 surviving patient showed unusual solubility of Bence Jones protein to acid pH in vitro. The urine of a third patient was negative for Bence Jones protein. Simple chemical screening of urine, to detect Bence Jones protein, is essential before patients undergo urography.

The value of quantitative protein electrophoresis.

Study of serum protein fractions separated by electrophoresis shows variation in subjects of different sex and race. Interpretation of results by clinicians is often difficult since the bands are composed of several different proteins, each responding to different physiological stimuli. Important information can be gained by examining the electrophoretogram and the qualitative appearance of the bands. This should be communicated to the clinician and, depending on the clinical and qualitative electrophoretic findings, an analysis of certain specific proteins should be performed.

Serum immunoglobulins in Black South African children.

Serum immunoglobulin levels in Black children were estimated and the results analysed in 13 age ranges, from birth up to 15 years of age. Samples from children whose serum immunoglobulins were unlikely to be affected by their clinical condition were chosen for the analysis. The values measured were much higher when compared with conventionally accepted reference ranges in White children within the same age groups. Therefore, it is most important to interpret the results of immunoglobulin measurement within the range for the individual concerned.

Factor VIII and factor-VIII-related antigen in multiple myelomatosis and related conditions.

Factor VIII and factor-VIII-related antigen (FVIIIRA) were estimated in 19 patients with paraproteinaemia. The FVIIIRA levels were above the normal range in 12 cases with II of these showing an abnormal FVIII/FVIIIRA ratio. The patients could be subdivied into three groups according to their FVIIIRA levels: normal levels, levels greater than normal but less than 200%, greater than 200%. This subdivision correlated with the degree of bone involvement as assessed by radiological skeletal survey.

Serum beta-lipoprotein and other specific protein concentrations in patients with immunocytoma.

Serum beta-lipoprotein and other specific protein concentrations were measured in 56 patients suffering from multiple myelomatosis, "benign" paraproteinaemia or Waldenström's macroglobulinaemia and in 56 control subjects. The mean level of B-lipoprotein in untreated patients with multiple myelomatosis and macroglobulinaemia was significantly lower than that of the controls. Patients who responded to chemotherapy showed a rapid return to normal of the beta-lipoprotein concentration, while the level remained unchanged in most of those who did not.

Alpha-chain disease in a non-Mediterranean climate. A case report.

The clinical, biochemical, immunological and histopathological features in a patient with alpha-chain disease are described. The patient, a 20-year-old Coloured man, presented with severe steatorrhoea, malabsorption, abdominal pain and progressive general deterioration. An heterogeneous abnormal band with IgA immunochemical specificity was detected on electrophoresis of the patient's serum and urine. This protein was identified as free alpha-chain and was present in serum, urine, saliva and jejunal juice. A jejunal mucosal biopsy specimen showed distinctive appearances associated with alpha-chain disease. Bone marrow involvement was found and abnormal lymphoid cells were seen in the circulation together with an increased B lymphocyte population derived from bone marrow. This is the third South African patient with alpha-chain disease to be diagnosed. The patient has shown a partial remission after 12 months' chemotherapy. There was rapid symptomatic response and normalisation of protein parameters which were not paralleled by an objectively discernible response as assessed by haematological examination, intestinal absorption studies and histology of the jejunal mucosa.

Solitary myeloma. A study of 10 black patients during an 8-year period.

Solitary myeloma of bone occurred in 10 Black patients during the 8-year period from 1967 to 1974. In 4 patients, the solitary myeloma involved the bones of the paranasal sinuses, in 3 patients the pelvis, and in 3 others the manubrium sterni. An IgG monoclonal gammopathy was present in the serum of 4 patients and Bence-Joanes proteinuria was found in 1 patient. Radiation therapy was the treatment of choice and the recommended tumour dosage is 4000-5000 rads. Serial measurements of serum and urine protein electrophoresis and immuno-electrophoresis were most helpful in determining when patients had achieved a complete remission. In 3 patients, 1 of whom died, the myeloma disseminated in periods varying from 26 months to 7 years. Nine patients are alive, of whom 7 are in complete remission, and 5 have lived for more than 5 years since the initial diagnosis of solitary myeloma.

Serum immunoglobulins G, A and M in White and Black adults on the Witwatersrand.

Blood donors and hospital inpatients of the same sex and race group showed similar concentrations of serum immunoglobulins G and M. Serum IgA levels were significantly higher in three of four hospital patient groups as compared with blood donor groups matched for sex and race. Age was shown to be responsible for this difference, since on average the blood donors were younger and analysis of all subjects in age groups showed a progressive rise in IgA with increasing age. Lower IgA levels occurred in women than in men of comparable age and race group. A sex difference with higher IgM levels in women was confirmed. All values in Black adults were much higher than in Whites. It is therefore of importance to consider the ranges of values for any individual's age, sex and race when interpreting results of serum immunoglobulin assay.

Extramedullary plasmacytoma of the breast.

The clinical and histopathological features of a case of solitary extramedullary plasmacytoma of the breast are described. After 46 mth of follow-up, there has been no recurrence of the tumour, and no evidence of further extramedullary plasmacytomas, multiple myeloma or diffuse myelomatosis either clinically, radiologically or on biochemical and haematological investigations. She has developed diabetes mellitus. Distinction is made between the true extramedullary plasmacytoma and those which are a manifestation of multiple myeloma. The unpredictable behaviour and prognosis of extramedullary plasmacytomas is indicated.
