[Experiences with a mobile monitoring system for fetal and neonatal cardiotachometry].

A monitoring system for fetal and neonatal cardiotachometry which set up in own instrumentation at the Department of obstetrics of the Regional Hospital of Rostock. With aid of a wireless telemetric system is it possible to estimate the fetal condition before and during of caesarean section as well as at transfer of high risk neonates.

[The significance of laparoscopic examination in acute adnexitis for bacteriological diagnosis].

Cultures of abdominal bioptic material from 102 cases of acute pelvic inflammatory disease diagnosed laparoscopically revealed that anaerobes were involved in 42.2% of the cases; 18.6% of the cultures were sterile. There were no signs that anaerobes predominate in cases, in which an abscess has already formed. The proportion of predominantly polymicrobial multiple infections was 54.2%, the remaining 45.8% of the cases involving mono-microbial infection. Scrutiny of the abdominal and cervical distribution of the germs in 42 of our female patients confirmed that cervical smears are inadequate for detecting the causal germ involved in a case of acute pelvic laparoscopy it seems advisable to use abdominal cultures of tissue taken from the locality of the inflammation in order to identify exactly the causal germ and to test these cultures also for anaerobes and gonococci.

[Economic aspects of intensive care in obstetrics].

An intensive obstetric care unit has been established at investment costs of 646,167.--Mark which broke down into 220,248.--Mark for prepartum attention, 268,010.--Mark for intrapartum attention, and 157,909.--Mark for neonatal attention. Operational expenditures on equipment and services were found to amount to 144,052.--Mark per annum.--Specific outlays resulted from intensive obstetrics, as compared to conventional procedures. They varied between 48.--Mark and 96.--Mark per delivery, depending on annual numbers of deliveries and on the use of equipment quantitatively adapted to requirements. Individual costs for intrapartum attention varied by bedside equipment combinations and turnover of patients (n/labour bed/d) and were between 12.--Mark and 36.10 Mark.--Something between 2000 and 3000 births per annum, with patient turnovers between 1.5 and 2.0 per bed and die, was considered optimum. The cost of intensive obstetrics went up under such optimum conditions to something between 48.--Mark and 58.--Mark, with something between 14.80 Mark (BMT 504 biomonitor, Lineomat, FTS 101 foetal-function recorder) and 21.60 Mark (BMT 9141 biomonitor, Lineomat) being required for intrapartum monitoring of one birth.--Reduction in perinatal mortality, as compared to figures of conventional obstetrics and to cost factor between 519,168.--Mark and 627,328.--Mark for equipment and other hardware, yielded a benefit for 10,816 births of something between 46.9 and 167,5 million Mark.

[Effects of acidosis on the adaptive behavior of full-term newborn infants].

59 full term, appropriate for gestational age newborn infants (20 delivered by forceps, 39 by cesarean section) with umbilical artery pH below 7.20 are studied in their adaptation period immediately after delivery up to 120 minutes by neonatal monitoring (cardiorespirogram). Control group consists of 50 newborn infants with normal pH (pH greater than or equal to 7.20). There are differences of baseline, of amplitude and frequency of long-term variability and of the initial tachycardytime. The interval to arrive the level of control group depends from the degree of acidosis. Results are demonstrated in 8 diagrams and discussed.

[Screening for prevention and early detection of cervical cancer in the German Democratic Republic-- an analysis (author's transl)].

The cytological mass screening to prevent invasive carcinomas by detection and therapy of intraepithelial neoplasia and to detect early stages of invasive carcinomas is the most important basis for cervical cancer control. An analysis of epidemiological effects caused by mass screening programs in two districts (Berlin, capital of the GDR; Rostock) demonstrates during the last 7 years a marked increase of detected ca in situ followed by a clear decrease, a decrease of morbidity of cervical cancer (49.4/100,000 :36.1/100,000 women) and an increase of earlier stages, especially stage Ia. The highest decrease of morbidity could be observed in women aged 40--49 years (111.6 : 61.0) followed by those aged 50--59 years (91.1 : 76.1). Preliminary results demonstrate the benefit of screening program and the practicability of organization form used in the capital Berlin. A further analysis demonstrates screening activities in the whole country. a continuous increase of cytological capacities in many districts is the remarkable result of the activities and gives the assurance for further success of the cytological screening program decided for the German Democratic Republic.

[Role of rheobase measurement in recording and controlling premature labour (author's transl)].

A rise in the rate of premature childbirth from six to eight per cent was recordable in some region of the GDR, in recent years. This figure added to the statistical record of perinatal mortality and accounts for something between 60 and 80 per cent. -- Aggravating neurovegetative irritability, next to foetal and maternal causes as well as to factors of the environment, has been quoted as the major cause of premature onset of labour. Rise in vegetative tension may be determined by means of rheobase measurement of neuromuscular excitability, on the basis of central nervous coupling of the striated muscle. -- Rheobase measurements were applied to pregnant women and statistically evaluated, between 1977 and 1979. -- The RS 12-1 electrostimulator and AP 202 arterial pulse indicator were used for rheobase measurement proper. Contractions were recorded by means of the DT-500 tocograph and Hewlett-Packard cardiotocograph. -- As a whole, 167 rheobase data were recorded from 101 patients. The rheobase data recorded from patients with premature onset of labour were significantly below those obtained from women with normal course of pregnancy. The rheobase threshold (critical value) may be defined as 3.5 mA. Statistical evaluation of follow-up rheobase measurements on pregnant women with premature labour showed that the values underwent significant change towards the normal in response to tocolytic therapy. Decline in rheobase in response to tocolytic therapy will be followed, with high probability, by termination of pregnancy. -- Rheobase is a parameter which was found to correlate properly with the risk of premature childbirth and, therefore, should be used in diagnosis (score to determine risk of premature childbirth) and therapy (supervision of tocolytic treatment).
