[Recommended algorithms of prescription in the diagnosis of iron deficiency and overload].

In view of the recent development of new tests of biochemistry and molecular biology the assessment of iron status should be reconsidered and updated. The French Society of Clinical Biology (SFBC) and the French Society of Hematology (Cellular Hematology Group) recommend algorithms in the diagnosis of iron deficiency and iron overload bearing in mind the best efficiency and health economy. These recommendations are based on the known sensibility and specificity of each test. The analytical requirements for the determination of the tests as well as the clinical and biological signs evoking an iron deficiency or overload are recalled.

[Congenital type II dyserythropoiesis].

The authors report a case of congenital dyserythropoieses, type II, the diagnosis of which was made in a young 15 year old girl with anemia, sub-clinical jaundice and splenomegaly. The following three criteria were found: erythroblastic hyperplasia with a high percentage of binuclear cells, a double peripheral red cell membrane under the electron microscope and a positive Ham-Dacle test. An isotopic study showed a double process or erythropoiesis which was inefficacious and peripheral hemolysis. In connection with this case, the authors then recall the main characteristics of the three types of congenital dyserythropoiesis. The origin and consequences of the peripheral membrane excess of the erythroblasts characteristic of type II are then discussed.

[Richter's syndrome and acute transformation: two terminal haematological complications of chronic lymphoid leukaemia (author's transl)].

Two cases of CLL terminating in Richter's syndrome and in blastic crisis raise the problem of the physiopathological signification of these unusual complications. Richter's syndrome is described as a "histiocytic" lymphoma or a Hodgkin's disease supervening on a CLL. It does not seem that these two pictures have actually to be distinguished since clinical and haematological aspects are not different, and histopathologic studies show always atypical aspects with difficulty of an accurate diagnosis. Cases of blastic crisis of CLL are very uncommon. They correspond to the transformation of a CLL in acute lymphoblastic leukaemia. These two terminal complications of CLL suggest they may be interpreted as two pictures of the transformation of the same initial B cellular clone. This hypothesis, highly probable for the Richter's syndrome, has been actually demonstrated for the acute transformation of CLL.
