The rise of serum creatinine levels during labor.

Serum creatinine, blood urea nitrogen and creatine phosphokinase were measured in 32 women during the last 3 weeks of pregnancy and, in a further 39 women, during and after labor. The serum creatinine increased from 61.9 +/- 0.9 to 69.8 +/- 1.8 mumol/l (mean +/- SEM) (P less than 0.05) in the third stage of labor and returned to normal by 72 h after delivery. The muscle creatine phosphokinase increased from 54 +/- 7 to 77 +/- 9.9 units (P less than 0.05) during the third stage and remained high (87 +/- 13.3 units) 72 h later. We conclude that these changes are due to muscle contraction and injury during delivery.

Muscarinic acetylcholine receptor-mediated phosphoinositide turnover in cultured human retinal pigment epithelium cells.

Cultured retinal pigment epithelium cells prepared from post-mortem adult human eyes are shown to contain muscarinic receptors associated with phosphoinositide turnover. Carbachol at a concentration of 100 microM induced a four-fold increase in 3H-inositol phosphates (more than 76% is in the form of 3H-inositol-1-phosphate) accumulation within 45 min in cells prelabelled with 3H-myoinositol and exposed to 5 mM LiCl. The EC50 of carbachol was approx. 70 microM and the saturation concentration was about 1 mM. The carbachol-induced response was blocked by both atropine and pirenzepine, the former being most effective. Pre-exposure of cells to carbachol resulted in desensitization and a drastic reduction in the subsequent carbachol-induced stimulation of 3H-inositol phosphates. The carbachol response could be attenuated by the biologically active phorbol ester, 4 beta-phorbol 12-myristate 13-acetate, and this was nullified by the protein kinase C inhibitor, staurosporine. The biologically inactive phorbol ester, 4 alpha-phorbol 12,13 dideconoate, did not attenuate the carbachol-induced stimulation of 3H-inositol phosphates. Pertussis toxin failed to influence the carbachol receptor-mediated phosphoinositide turnover. These studies provide clear evidence for the occurrence of muscarinic receptors coupled to phosphoinositide hydrolysis on human retinal pigment epithelium cells.

Bromocriptine in the treatment of alcohol dependence.

A double-blind controlled study of the effects of bromocriptine on drinking behavior and associated symptoms in ambulatory alcoholics showed a marked improvement in both treatment and control groups. However, significant differences in favor of the medication were observed in psychopathological measures, and trends in the same direction in most of the other efficacy parameters.

Serum and intracellular electrolytes in patients with and without pain.

The effect of acute stress, with and without pain, on serum and mononuclear cell cation content was studied in 205 healthy women in their last trimester of pregnancy or during normal labour, in patients with acute medical conditions in which pain was or was not present, in acute surgical conditions, and immediately prior to elective surgery. In all subjects there was a fall in serum sodium, potassium, magnesium and calcium concentrations during stress, with an apparent shift into the intracellular space. An inverse correlation was present between the severity of pain and the fall in serum cations.

Compound heterozygosity in nonphenylketonuria hyperphenylalanemia: the contribution of mutations for classical phenylketonuria.

Hyperphenylalaninemia (HPA) results from defective hydroxylation of phenylalanine in the liver, in most cases because of defective phenylalanine hydroxylase. HPA is highly variable, ranging from moderate elevation of plasma phenylalanine with no clinical consequences to a severe disease, classical phenylketonuria (PKU). Non-PKU HPA was found in excess of PKU in Israel, while the opposite is true in Europe. To study the genetic basis of non-PKU HPA, we performed haplotype analysis at the phenylalanine hydroxylase locus in 27 families with non-PKU HPA. All individuals with this condition were compound heterozygotes. In six of these families, in which both PKU and non-PKU HPA were segregating, haplotype analysis showed that non-PKU HPA resulted from compound heterozygosity for a PKU mutation and a second mutation, with milder effect, which is probably expressed only when it interacts with the severe mutation. The involvement of PKU mutations in non-PKU HPA was further demonstrated in Jewish Yemenite families with non-PKU HPA, in which the individuals with this condition were carriers of the single PKU allele which exists in this community. In addition, two previously known PKU point mutations (R261Q and R408W) were found in individuals with non-PKU HPA. These mutations are associated, in our population, with the same haplotypes as those with which it is associated in Europe. Based on the above-mentioned genetic model for non-PKU HPA, successful prenatal diagnosis of this condition was performed in one family.(ABSTRACT TRUNCATED AT 250 WORDS)

Amino Acid Metabolism, Inborn Errors↗

A routine method for the measurement of the sodium, potassium, magnesium and calcium content of human lymphocytes.

We describe a rapid, single-step procedure for the isolation of human lymphocytes from whole blood, suitable for a routine clinical laboratory. Lymphocyte content of sodium, potassium, magnesium and calcium were measured simultaneously in a group of controls and found to fall within expected ranges. Expression of results per mg protein produced less inter-individual variation than per unit cell. In order to examine another, physiologically different but normal population, women during pregnancy were also studied. The cation content of lymphocytes expressed per mg protein was significantly lower than for controls due to a 44% increase in protein content per cell.

A single origin of phenylketonuria in Yemenite Jews.

Phenylketonuria (PKU) is a metabolic disease caused by recessive mutations of the gene encoding the hepatic enzyme phenylalanine hydroxylase (PAH). The incidence of PKU varies widely across different geographic areas, and is highest (about 1 in 5,000 live births) in Ireland and western Scotland, and among Yemenite Jews. A limited number of point mutations account for most of the PKU cases in the European population. Here we report that a single molecular defect--a deletion spanning the third exon of the PAH gene--is responsible for all the PKU cases among the Yemenite Jews. Examination of a random sample of Yemenite Jews using a molecular probe that detects the carriers of this deletion indicated a high frequency of the defective gene in this community. Although the deleted PAH gene was traced to 25 different locations throughout Yemen, family histories and official documents of the Yemenite Jewish community showed that the common ancestor of all the carriers of this genetic defect lived in San'a, the capital of Yemen, before the eighteenth century.

The relationship between beta-endorphin levels and uterine muscle contractions during labor.

Plasma beta-endorphin (BE) levels (8.6 +/- 0.8 pmol/l) (mean +/- SE) were lower in the third trimester than in non-pregnant controls (14.8 +/- 1.1 pmol/l) (P less than 0.001), increased during labor, to 29.3 +/- 4.4 pmol/l (P less than 0.005) and decreased, 72 h after delivery, to 3.5 +/- 0.4 pmol/l (P less than 0.001). BE levels were found to correlate significantly with uterine muscle contraction (r = 0.966, P less than 0.05) and with cervical effacement (r = 0.974, P less than 0.05) during labor.

Late clinical testing of cognition enhancers: demonstration of efficacy.

The major difference between clinical trials for efficacy and Phase I trials is their duration; clinical trials of cognition enhancers, for example in senile dementia, require at least 6 months. Because no widely accepted treatment is available, the methodology for these trials is still in a the process of refinement. Double blind placebo-controlled trials are considered to be mandatory particularly by the FDA. Parallel group designs seem preferable and the so-called "enriched design" seems to possess advantages over more classical procedures. Outcome measures should include ratings by clinicians and family members. Self-reports are considered to be unreliable in Alzheimer patients but may be less problematic in AAMI. Psychometric tests of cognition and motor skill should also be included. An essential and pragmatic outcome criterion is, however, the quality of daily life.

Factors in the development of severe forms of tardive dyskinesia.

The authors evaluated 558 patients for tardive dyskinesia. They found that the prevalence of tardive dyskinesia was 34%. There were no differences between men and women in prevalence of tardive dyskinesia. However, severe tardive dyskinesia was found to occur more in male patients 40 years old or younger and in female patients 71 years old or older. Patients with mild tardive dyskinesia received more neuroleptics than did patients with moderate and severe forms. However, patients with moderate tardive dyskinesia had significantly more drug-free periods in their drug histories than did patients with mild tardive dyskinesia.

A dose-range finding study of zopiclone in insomniac patients.

Sixty insomniac patients participated in a controlled double-blind parallel group study designed to investigate the dose-response relationship of zopiclone. Following 1 day of treatment with placebo, patients were randomly assigned to 1 of 6 groups and received treatment for 7 days with either placebo, or flurazepam 30 mg, or zopiclone, 3.75 mg, 7.5 mg, 11.25 mg or 15 mg. Four patients were dropped from the study; two from the placebo group due to ineffectiveness and one each in zopiclone 11.25 mg and 15 mg groups due to side-effects. Flurazepam 30 mg significantly improved sleep induction and maintenance by comparison to placebo and was indistinguishable from zopiclone 7.5 mg or higher. Results of a self-administered sleep questionnaire found a predominantly linear relationship between the dose of zopiclone administered and the degree of sleep improvement. The greatest increment in improvement was generally obtained with 3.5 mg and 7.5 mg of zopiclone, with some additional benefit occurring with zopiclone 11.25 mg. Clinicians' global impressions showed that the severity of illness clearly decreased in a dose related manner up to zopiclone 11.25 mg. Although zopiclone was well tolerated at 3.75 mg and 7.5 mg, an increase in side-effects occurred at 11.25 mg and 15 mg, which favours the use of 7.5 mg zopiclone as the optimum dose for most patients, although certain patients may benefit from a higher dose of the drug when well tolerated.

Circadian rhythms of melatonin and cortisol in aging.

The relationship of age to the circadian rhythms of melatonin and cortisol was investigated in 44 men and 27 women (age range 19-89 years). Subjects were physically and psychiatrically normal. Four hourly serial blood samples were drawn from 8:00 AM until 8:00 AM the next day, with additional samples at 10:00 PM and 2:00 AM. The indoor illumination was restricted to 300 lux during day and 50 lux during the night. Plasma melatonin and cortisol were estimated by radioimmunoassay. Results show that the means of melatonin and cortisol values decreased significantly with age when the subjects were divided into three age groups, i.e., 19-25 years, 42-65 years, and 66-89 years. They also showed a significant negative correlation with age. The acrophases of the two hormonal rhythms, however, showed different relationships to age. The acrophase of melatonin rhythm showed a positive correlation with age (r = 0.38, p less than 0.001), and cortisol showed a negative correlation with age (r = -0.56, p greater than 0.001). It is suggested that this may indicate a weakened responsiveness of the circadian system in the elderly to the day-night cycle and an altered relationship between the pacemakers driving melatonin and cortisol circadian rhythms. This may thus represent a biomarker for the intrinsic process of the aging of the brain.

Effect of lithium and neuroleptic combination on lithium transport, blood pressure, and weight in bipolar patients.

Patients treated with a combination of lithium (Li) and neuroleptics are reported to be at greater risk for toxicity. Previous findings suggested that treatment with a combination of Li and neuroleptics would increase the lithium ratio (LR) and intraerythrocyte Li levels. We studied 67 bipolar patients who were treated with Li alone (20 patients) or Li combined with haloperidol (17 patients), or Li combined with chlorpromazine (30 patients). Results revealed that the neuroleptic groups showed significantly lower LR and intracellular Li concentration as compared with those on Li alone. There was no significant difference in Li dosage among these three groups. No difference was found among these three groups with respect to mean blood pressure and weight. The number of hypertensive patients in the Li only group was twice as high as that in either of the two other groups, and the LR of the former was significantly higher than those of the latter groups.

Transmembrane lithium distribution and hypertension in manic depressive patients.

1. The association of hypertension and depression has long been recognized. The purpose of this study was to explore the relationship between RBC level/plasma level of lithium (Li), blood pressure (BP) and weight in bipolar affective disorder patients. 2. All patients were evaluated by the same examiner using DSM-III Diagnostic Criteria. Measurement of patients' Li values, BP and weight were carried out on the same day or within 24 hours. 3. We evaluated 103 bipolar patients treated with lithium (mean age: 44 years, mean duration of Li treatment, 6 years). Ten percent of these patients were hypertensive. 4. No significant difference was found between hypertensive (H) and non-hypertensive (NH) patients with respect to their RBC, plasma or RBC/plasma Li ratio. This finding was consistent in patients treated with Li alone or Li combined with other neuroleptics. 5. Our study did not confirm previous findings (McCoy et al, 1982) indicating that there is a correlation between low Li ratio (less than 0.30) and hypertension in bipolar affective disorder patients.

Mastication in the rabbit: a description of movement and muscle activity.

1. Mastication was studied in awake rabbits that had previously been prepared for chronic recording of jaw movement and jaw muscle electromyography (EMG) under general anesthesia. Data were stored on tape and replayed for computer-assisted analysis. Most data was taken from the mastication of rabbit chow, but the basic features described in the paper also apply to eating of other foods. The series of movements was divided into cycles, and the phases that compose them, using peaks in the vertical movement, velocity, and acceleration. 2. The whole series of movements from ingestion to swallowing was termed the masticatory sequence. We found, as have others, that the sequence can be divided into three consecutive periods, based on the form of the movements. These were named the preparatory, reduction, and preswallowing series of cycles. 3. The results of earlier studies suggest that the food is transported back to the molar teeth during the preparatory series, ground up during the reduction series, and that the bolus is formed for swallowing during the preswallowing series. 4. The typical cycle of the preparatory series was called type I; it had two phases: opening (O) and fast closing (FC). The jaw-opening muscles were very active during O, but EMG bursts from the closer muscles were small or undefinable during FC. There was not much movement away from the midline in either phase, and the cycles were the shortest in the sequence. 5. The reduction series was predominantly composed of type II cycles. These were of intermediate duration and had three phases [O, a short FC, and a slow closing phase (SC)], during which the pellets were crushed between the molar teeth. The jaw moved toward the working side during FC. At or soon after the start of SC, the closer muscles became very active and continued to contract as the teeth were drawn toward the midline and slightly backwards. 6. The preswallowing series was made up of five-phase cycles that we called type III. These were the longest of the sequence. Jaw opening now occurred in three stages: O1 was the fall from tooth contact to approximately the postural position, O2 was a pause, and O3 was defined as the final movement to maximum opening. The digastric burst occurred during O3. There were again two closing phases, FC and SC, but closer bursts were smaller than in type II, and SC was shorter.(ABSTRACT TRUNCATED AT 400 WORDS)

Economic development, population and primacy.

"This paper investigates the relationship between economic development and primacy advanced by El-Shakhs, Mera and others, and finds no statistically significant link when obvious demographic influences are taken into account. Three variables (national population, the urban population share, and a Latin American dummy) explain 40% of the variation in primacy in a sample of 116 countries (including 82 developing countries). Economic factors do not appear to be important. Demography may be more influential than economics in attempts to explain primacy."
