Evaluation of fetal heart rate variability.

Fetal heart rate (FHR) variability appears to be an "early warning system" of fetal hypoxia. FHR variability has been utilized in clinical practice to examine fetal wellbeing. Since 1976 we have used a standardized scoring system to evaluate the cardiotocograph. To eliminate subjective aspects in FHR variability analysis we propose a programmable device utilizing a microprocessor. Preliminary results encourage us to continue this research.

Usefulness of hysteroscopy in endometrial adenocarcinoma staging.

Endometrial carcinoma has an absolutely unfavourable prognosis when it involves the cervix. Clinical staging, which is essential to a correct therapeutical approach, has proven quite wrong when relying on endocervical curettage. This work deals with the use of Hamou's microhysteroscope to assess the endocervical diffusion of endometrial cancer. Thanks to its very high sensitivity (100%), high specificity (89%) and diagnostic accuracy (92%), this method has proven extremely reliable in the clinical staging of endometrial carcinoma.

Spermatic and peripheral oestradiol levels in patients affected by azoospermia due to seminiferous tubular damage.

Plasma levels of testosterone, androstenedione and oestradiol were determined in the spermatic venous blood of both testes of 17 patient affected by azoospermia due to tubular damage (Group I). The results were compared with those found in 5 patients affected by azoospermia of obstructive origin and 5 patients with an inguinal hernia (Group II). Mean spermatic levels of testosterone and androstenedione were not significantly different in the two groups, while the mean (+/- SE) oestradiol spermatic level was significantly higher in patients of Group I (5.02 +/- 0.75 nM/l vs. 2.20 +/- 0.365 nM/l; P less than 0.05). Moreover, while the testosterone/androstenedione and the androstenedione/oestradiol ratios were not significantly different in the two groups, the mean (+/- SE) testosterone/oestradiol ratio was significantly lower in patients of Group I (552.71 +/- 80.94 vs. 939.86 +/- 129.45; P less than 0.025). Peripheral testosterone and androstenedione mean levels were not significantly different between the two groups while the mean peripheral oestradiol level (+/- SE) was significantly higher in Group I (0.107 +/- 0.021 nM/l vs. 0.038 +/- 0.05 nM/l; P less than 0.025). Peripheral oestradiol was not significantly related to peripheral FSH, nor to spermatic oestradiol in both groups. These results suggest the possibility that oestradiol may be involved in the pathogenesis of some cases of male infertility.

Plasma hormone assay in pregnancy.

This paper reports the results obtained with various hormonal parameters (HPL, E3, alpha-FP) in evaluating fetal wellbeing during pathologic pregnancies. The expression of hormonal parameters in terms of standard deviation (S.D.) provides a constant endocrine index throughout pregnancy, independently of the adopted parameter. Such methodology reduces error and simplifies interpretation of data. The hormonal index is important not only for reliable diagnosis of a pathologic situation, but also because of its simple interpretation for clinical and practical purposes.

Benign pictures of the uterine cervix in a large colposcopic survey.

The Authors have examined the results of 14.703 colposcopic examinations performed at the Social Medicine Center of the Province of Florence between 1974 and 1978, grouping the women according to childbearing and age. The purpose was to verify if mechanical and traumatic factors related to delivery may be responsible for modifications of the epithelial lining of the uterine cervix and especially for ectopy. The Authors have shown that the percentage of incidence of colposcopic picture of portio with original squamous epithelium and ectopy maintain almost identical values in nulliparae and pluriparae. Delivery does not significantly influence the presence of ectopy: the slight increase of such a picture in pluriparae is attributed to the formation of "ektropion", and the Authors define this picture, clearly separating it from the ectopy picture. The presence of ectopy decreases considerably as the women's age increases: this is attributed to therapeutic treatments possibly undergone over the years and to the spontaneous mending processes which are completed with age. Ectopy and its typical transformation zones are found in high percentages in the age group of women under 20. This corresponds exactly to the percentages reported by a few Authors on the presence of jodonegative zones in newborns. On the basis of these results, the Authors conclude that delivery does not modify the condition of the uterine cervix and deny any relation between traumatic factors and "cervical erosion".

Colposcopy and cytology in young women up to the age of 20: an analysis of 401 cases research for a new screening programme.

The Authors have observed by colposcopy and cytology, 401 patients under the age of 20 and have compared the results obtained with those of a similar control group with ages ranging from 20 to 25. They observed the incidence of normal cervix and of other colposcopic reports. From the comparison between the colposcopic and cytologic data emerged that the cytopathologic diagnosis does not put the cervical alterations of inferior level than dysplasia in evidence, in a large number cases as colposcopy does. There is a very close relation between the incidence of pathologic cervical lesions and sexual activity. Correlation between cervical pathology and use of hormonal contraceptives and presence of cervico-vaginal phlogosis, was not possible in this type of survey. The Authors therefore propose a new type of mass screening, which should substitute the actual one, that is substantially based on cytology and on early diagnosis rather than prevention.

Chronic prostato-vesiculitis and sperm-cell mobility in male infertility.

The purpose of this research has been to value the importance of chronic inflammation of the prostatovesicular apparatus in sterile patients. The authors wish to underline the high incidence of chronic inflammation if this apparatus in such patients, the low incidence of bacterial forms and the high frequence of association with bradycinesia. The therapy of such forms gives excellent results.
