[Lay activities in inpatient rehabilitation services].

We could introduce the following lay activities within the activities of our scientific institute (that is in the title) in collaboration with a neuro-rehabilitation-unit. 1) Weekly visits of a group of senior-scouts. 2) Weekly visits of a pupils group (aged 14), that came as volunteers from an experimental school. 3) A guitar teacher. 4) A student group in advanced education. All together the effect was positive in the sense of ameliorating the human communicative climate within the unit. We however must warn that such measures are considered a "help in work" for the staff. Contrarily gives the supervision of the active lay people an additional task, which however diminishes as the routine advances. It is essential to have a coordinating person who is completely responsible, highly engaged, and interested in the matter.

[Patient rights alone are not enough, too many rights can also be harmful].

The nowadays running increased emphasis on patients rights can be seen positive as a way towards a "competent partner-patient". However, better health results to come out of it want mainly an important pre-condition, which we call "general renaissance of communication culture". This should work on the non-verbal, verbal and written level. We name the following examples on this way. -Schooling for communication, including the mechanism of "positivation", that we have introduced into basic psychotherapeutic schooling. -There are further levels of quality assurance, leading to quality amelioration via evaluation and motivation. The "Rasterzeugnis" (i.e. periodical doctors' certifications, which has become obligatory per legem now in Austria) is one of these instruments. -There are highly sensible territories like communication with the incurable as well as organ transplantations. -But also the seemingly so banal written doctor's report should be the subject of important ameliorations in the routine. Following a pure standpoint of the right with neglect of all said above, can be contra-productive. We give examples of the -patients' insight into medical documents as well as -simple juridical judgement of a patient's competence. Only by synopsis of right with communication, there can be reached an optimal result for the patient. This should run under the omnipresent principle of humanitas.

[Free love, wild marriage--on the gradual dissolution of marriage by love"].

Within the different cultural epochs there exist different concepts of what is to be meant by 'love', 'marriage', 'human communication'. Maybe we are presently in the process of a paradigmatic change. However certain basic laws within human relations will always persist, such as: erotics, attraction, strive to social security, necessity to combine positive and negative within longer lasting human relations etc. Above all there is a great individual variability. There exist no one-way solutions. The more it is important that not only psychotherapists should be aware of the problems and include them in their daily work. But it counts for every doctor, who intends to operate humanistic medicine (not only a mechanistic one). This means a holistic approach towards a "partner"-patient.

[Hemodilution therapy with neuron metabolism specific therapy in ischemic stroke--encouraging results of a comparative study].

Treating ischemic stroke we could compare a (control-)group of 318 patients under up to date hemodilution therapy to a group of 100 patients receiving exactly the same treating scheme but including an add-on administering of Cerebrolysin. It proved a statistically validated better outcome within the Cerebrolysin add-on treated group evaluated by the Barolin-Neuro-Rehabilitation Scale. 1) More effect on social and occupational parameters than on motor functions, but also including some of those. 2) Cerebrolysin accelerates recovery and therewith offers better starting points for rehabilitation. As stroke is one of the main social-medical neuro-rehabilitation and gerontological problems of our days these results should be recognized in a general therapeutical use.

[Rehabilitation especially needs a holistic approach].

Rehabilitation comprises interdisciplinarity and long-time continuity including permanent accompaniment. In < > [geriatric rehabilitation] this counts specially, according to the demographic steady growth of its importance. Neither social medicine nor rehabilitation science up to now have sufficiently taken up this < >. Regarding human being as biopsycho-social entity demands, besides somatic and pharmacological approach, regarding of mood, distress and environment. The by us so called < > has to be applied as well in its basilar dimension [for everybody dealing with geriatric patients] as well as in specialised proportions. Same counts for pain-rehabilitation. A complex rehabilitation program wants an interdisciplinary team. By further specialisation should scientific progress be stimulated. Interdisciplinarity and overlapping however should ensure that patients do not get lost between the specialisations. All this only can work and progress by stimulating public opinion and political collaboration. These are large tasks for the rehabilitationists in the future.

[The physically ill patient needs psychotherapy].

Any somatic diseased should be offered (additional) psychotherapy and many patients are able to profit a good deal by it. This accounts generally not only in the defined "psychosomatic illnesses". Specific indications according to target symptoms have to be applied; but no primary exclusion! This is a big challenge for a new generation of psychotherapists, having almost not been regarded in the classical "psychotherapeutic schools" up to now. It wants regarding of hic et nunc during the acute crises of disease, should however be in fluent transition to long term psychotherapy. It accounts specially for the rehabilitation patient, the incurable patient, the geriatric patient in general. Relatives as well as the nursing personal must be included in the sense of a "systemic approach". This shows, that one single psychotherapeutic school cannot be prepared to fulfill these demands. Psychotherapeutic "schools" present an important basis, should however lead towards an "integrated psychotherapy" to be problem-centered, patient-centered and school-pluralistic. Psychotherapy in somatic illness shows this demands very distinctly. We however consider the "integrated approach" a main street for future psychotherapy in general.

[Comment on planned evaluation regulation of the Austrian Ministry of Science for university medical education].

Austria till now operates without any evaluation of medical university teaching. Now the science-ministry has circulated the draft of an edict which we comment as follows: 1) The new measures should be in a way that alone by their existence amelioration of quality appears. Also they should provide better communication between teachers and students (which has gone lost a great deal in the mass-university of today). 2) Examinations are the worst instruments for quality raising. We propose installation of a flying commission which talk things over with teachers and with students and thus forms a living bridge to raise communication. We think that things should rather not be done in anonymity, but more in steady communication between teacher and student. 3) There should be evaluation of evaluation and the named commission should ensure a steady dynamic evolution of it. 4) Main thoughts are that university-teachers are evaluated as well for their quality of teaching as for their endeavour to optimate it, whereas now only "papers" and scientific output is measured. The student should learn own responsibility, overall knowledge and better communication. Only thus we shall educate new doctors that approach a humanistic ideal instead of being "medicine-apparatschiks".

[Rehabilitation hospitalism].

What we so call is: retardation of rehabilitation progress behind (remaining) abilities. MAIN CAUSES ARE: Associate depression (psychoreactive, somatogenous plus endogenous component; most in an overlapping way); Additional somatogenic factors (heart/circulation, pain, etc.); Unfavourable conditions in the surroundings (rehabilitation-organisation, family, etc.); TREATMENT (etiologic plus symptomatic); Medication, especially psychopharmaca administration; physiotherapy; psychotherapy plus organisation plus hygiene plus family therapy. No "either/or" but "as well/as well".

[Comparative analysis of mental health treatment of various groups of competitive athletes].

1) There is sense and necessity of psychohygiene in sports. This could be stated with 3 compared groups, namely: a national team, a team of handicapped, a team of juveniles. 2) We have used a complex program with integrated autogenic training (this however, not in an isolated way). Earlier experiences (of last author) over many years and hundreds of sportives were integrated in our evaluation. 3) Its to our knowledge the first time in world literature that effectiveness of psychohygiene within sports could be proved with statistical significance using world cup points within comparable groups. 4) Only possible can be possible, and psychohygiene will help to optimate this goal but not overrun it. This is main differentiation against doping. The human and the humanity is the main goal and not the so-called "necessities" of sport dictatorship. Its a good sign that several of our candidates told us that our psychohygienic program has reached over their period of sports activities into their "normal" lives.

[When marriage ages--love, crisis, new beginning].

Mobility of partnership increases in our times. There is a multifactorial package of causes with different preponderance in different age groups. Named be for example: the increasing average age of mankind with prolonged activities, emancipation of women, as well as sexual emancipation, new social structures etc., etc. If we analyse the factors which not only count for one single moment but in the long run for evolution of partnerships we see many different aspects. It may be useful to know them and include them in selfreflection and/or psychotherapeutic assisted changement of behavioral patterns. It may help to: on the one hand maintain human relationships; on the other hand, if separation is necessary, to make it less painful and critical.

[The end of life as a mental health responsibility].

Terminal phase of human life must be regarded as one of the critical life phases as well for the dying himself as for his relatives.--Psychohygienic accompanying should start not just in the latest phase but from the first knowledge of incurable illness. We must regard herewith an important ethical task that on the one hand should be taught psychotherapeutically. On the other hand must not left alone to psychotherapeutic specialists. All our health professions must be familiar with it. We see this as integrative part of a holistic schooling for communication that should run from our medical studies on through postgraduate education. We do not consider "death-accompanying" an isolated territory, however even think it contraproductive if it is treated too isolated. It would support tendencies existing in medicine and in society, to segregate incurable and old people, our endevour should be counterbalancing.

[Psychotherapy and mental health in chronic headache].

Pain can be seen to originate from 3 sources: the anatomic way between stimulus and reaction; the autonomous nerve system; personality (with all its facettes and determinants). Psychotherapy (including its action in psychohygiene); we do not split up here according to "schools". For differential indication it seems more important to see the main ways how psychotherapy works: verbalisation, the hypnoid switch and other methods acting via the body, sociodynamics, presentation, behavioristic methods including biofeedback. All these methods to be used in mutual overlapping and combination. Different ways of psychotherapeutic action may be effective specifically and specially on different components of pains as stated above. Moreover we emphasize an integrative setting of therapy comprising but all possible therapeutic methods like medication, physiotherapy, etc. besides psychotherapy. Only with such a complex setting the multidimensional complex of headache can be treated in the optimal way.

[Evaluation and quality assurance in neurorehabilitation].

Quality assurance is among the dictates of the hour and should, through integral feedback, lead to on-going methodical improvement. It must not be mistaken for merely comprising control measures decreed from higher up. The following is brought forward: 1. Humaneness must be the overruling principle. 2. The methodological specifics of the various disciplines and the needed range of methods on the one hand have to be accepted with all their complexities, yet on the other be tied into an appropriate quality assurance system with certain norms and standards. 3. We were able to develop the "Barolin-Reha-Scale", which has proved its efficiency in neuro-rehabilitation and has several advantages over the previously available scales. 4. In addition, on-going patient self-assessment must routinely be carried out, although we certainly are well aware of the many "subjectivity factors" involved. 5. The above can only come to bear if meaningful controls are permanently established in addition to the necessary motivational and organisational measures.

[Group respiratory biofeedback in complex neurorehabilitation].

Respiratory feedback is to be seen as an apparatus aided psychotherapy. Like other hypnoide psychotherapies it acts via the vegetative switch, enabling relaxation and dynamization, as well as intentional formula. Administration in a semi open group can be demonstrated within the frame work of neuro-rehabilitations, mainly pain-rehabilitation. The action is to be seen as well somatotropic as psychotropic, within a holistic approach.

[Outcome-oriented Barolin Neurorehabilitation Scale: 476 cases over 6 years--multi-nodal statistical correlation of neurorehabilitation groups in diagnosis, prognosis and outcome].

This evaluation documents practicability and scientific value of the scale, since many years developed in our institute and in use there. Patients with left hemispheric lesions suffer from stronger disturbances of eating than others. Systematic neurological rehabilitations leads to a marked increased unnecessity of catheterism and by that to a marked better social integration. Associate depression (after Barolin) is a main source of rehabilitation hospitalism (as called by Barolin). Multiple sclerosis and Morbus Parkinson give better outcomes than cerebro-vascular patients. A new finding that also proofs to be neurophysiologically interesting is that higher agedness is mostly a non-limiting factor for rehabilitation, as far as not lesser endavours produce a self fulfilling prophecy. However a slight limitation in higher age groups is to be found in the patients with lesions on the dominant hemisphere, whereas lesions on the subdominant hemisphere produce no limitation to rehabilitation at all by higher age.
