Friedreich's ataxia: clinical involvement and evoked potentials.

In 15 patients definitely affected by Friedreich's ataxia, precocious SEP abnormalities were constantly recorded independent of the duration of symptoms and the severity of clinical involvement; VEPs were frequently involved, but neither VEP abnormalities, nor visual impairment could be correlated with the severity or duration of Friedreich's ataxia clinical involvement; BAEPs were, to different extents, abnormal. These were completely dissociated from hearing disorders and significantly correlated (P less than 0.001) with Friedreich's ataxia clinical disability. Our observation of increased latency of the VEP P100 component and the early disappearance of BAEP Wave V with persistence of Wave I is in contrast with the hypothesis that changes are related to primary axonal degeneration. For practical purposes SEPs could help in the early diagnosis of Friedreich's ataxia, and BAEPs could be used for the electrophysiological monitoring of its clinical progression.

[Embolization in bladder hemorrhage of neoplastic origin].

The authors report on their two years experience about hypogastric transarterial embolization in the treatment of urinary bladder intractable neoplastic hemorrhage. They discuss about materials for embolization and treatment of pain for acute ischaemia secondary to embolization.

In vitro antimicrobial activity and beta-lactamase stability of moxalactam, a new 1-oxa-cephalosporin.

The in vitro activity of a new semisynthetic 1-oxa-cephamycin, moxalactam, was compared with cefotaxime, cefoperazone, cefoxitin and gentamicin against 268 clinical bacterial isolates. Against Staphylococcus aureus, Staphylococcus pyogenes and Streptococcus faecalis the activity of moxalactam was similar to that of the other compounds. Moxalactam was the most active antibiotic against fermentative and non-fermentative Gram-negative bacilli, except for Pseudomonas aeruginosa. Moxalactam exhibited activity against gentamicin-resistant strains. Minimum inhibitory concentration and minimum bactericidal concentration differences were minor. Activity was not altered by changes in inoculum size, type of medium and presence of serum. The protein-binding was 35%. Moxalactam was not destroyed by common plasmid and chromosomal beta-lactamases. The penetrability studies demonstrated that, using E. coli UB 1005 and its mutants, the penetrability of moxalactam was higher than that of the other cephalosporins compared.

A preliminary report on the immunological responses after oral vaccine (Ty 21a).

Humoral and cell-mediated immune responses, specific for lipopolysaccharide (LPS), were evaluated before and after oral immunization with the Ty 21a strain of Salmonella typhi in a group of healthy volunteers. No rise in seric and foecal antibody titres, detected by the ELISA technique, was seen after vaccination. On the contrary, we were able to demonstrate the development of specific cell-mediated immunity, as assayed by the leukocyte migration inhibition test, 21 days after vaccination. This conversion was still present 40 days after the last dose of vaccine. We believe that the Ty 21a strain of S. typhi, recently proposed for immunization against typhoid fever and based on the use of live micro-organisms, is effective due to its ability to induce specific cell-mediated immunity.

Hemopoietic development in human embryos.

We report studies on hemopoietic progenitors and precursors in human embryos and fetuses at 4 to 5 through to 9 to 10 weeks from conception, as well as the corresponding in vivo Hb synthesis pattern. Particular attention is paid to the morphology of 5- to 8-week liver cells, as evaluated by both conventional and electron microscopy. On the basis of these observations, a unifying monoclonal model is proposed for human embryonic hemopoiesis.

Unusual combination of genetic defects in a Sicilian boy: G gamma delta beta thalassemia, G gamma A gamma heterocellular HPFH, beta (0) thalassemia, and albinism.

We describe the clinical and hematological findings in a 5-year-old boy with G gamma A gamma delta beta thalassemia, a G gamma A gamma heterocellular form of HPFH, beta(0) thalassemia, and albinism. Clinically he manifested only the characteristics of beta-thalassemia trait and not the typical picture of doubly heterozygous beta thal/delta beta thal. The simultaneous presence of heterocellular HPFH improves gamma chain synthesis, thus reducing the alpha chain excess. It is also possible that gene expression can be modified by the presence of other genetic anomalies.

Heterogeneity of immunological abnormalities in ataxia-telangiectasia.

Peripheral blood mononuclear cells from five patients with ataxia-telangiectasia were evaluated for their reactivity with a panel of monoclonal antibodies directed against T-cell subsets and for their in vitro functions in a pokeweed mitogen-induced immunoglobulin biosynthesis assay. All the patients had significantly reduced proportions of cells identified by monoclonal antibodies to subpopulations of T lymphocytes with helper activity (OKT4 and 5/9) and produced low amounts or no IgA and IgG in vitro. Immunoglobulin biosynthesis was increased by the addition of normal x-irradiated peripheral blood mononuclear cells in one of three patients, suggesting a helper T-cell deficiency in this patient and intrinsic B-cell defects in the other two. Two patients had increased proportions of cells identified by a monoclonal antibody to a subpopulation of T lymphocytes which includes suppressor T cells (OKT8), and their cells were able to suppress immunoglobulin biosynthesis by peripheral blood mononuclear cells from normal donors. These findings indicate heterogeneous disturbances of immunoregulatory mechanisms in ataxia-telangiectasia.

Molecular mechanisms of human hemoglobin switching: selective undermethylation and expression of globin genes in embryonic, fetal, and adult erythroblasts.

The globin chain synthetic pattern and the extent of DNA methylation within embryonic, fetal, and adult beta-like globin gene domains were evaluated in greater than or equal to 90% purified human erythroblasts from yolk sacs and fetal livers in the 6- to 12-wk gestational period as well as from adult marrows. The 6-wk erythroblasts produce essentially embryonic epsilon chains, whereas the 12-wk erythroblasts synthesize largely fetal gamma globin and the adult marrow erythroblasts synthesize almost exclusively adult beta chains. In all phases of ontogenic development, a strong correlation exists between DNA hypomethylation in the close flanking sequences of globin genes and their expression. These results suggest that modulation of the methylation pattern may represent a key mechanism for regulating expression of human globin genes during embryonic leads to fetal and fetal leads to adult Hb switches in humans. In ontogenic development this mechanism might in turn correlate with a gradual modification of chromatin structure in the non-alpha gene cluster, thus leading to a 5' leads to 3' activation of globin genes in a balanced fashion.

Comparative susceptibilities of organisms isolated in Sicily to cefotetan and other antibiotics.

In recent years there has been a gradual increase in the number of infections caused by Klebsiella, Enterobacter and Serratia and non-fermentative bacilli such as Pseudomonas, Alcaligenes and Acinetobacter. The object of this investigation was to evaluate the antibacterial properties of the cephalosporins cefoxitin, cefoperazone, cefotaxime and of gentamicin, in comparison with cefotetan, a new cephamycin with a high degree of stability to beta-lactamases. The cefotetan MIC was not affected either by size of the bacterial inoculum, or the culture medium and the serum protein binding of cefotetan was 85%. Cefotetan showed only moderate antibacterial activity, similar to that of the other cephalosporins tested, against the Gram-positive cocci, whereas it was very active against fermentative and non-fermentative Gram-negative bacilli, except for Pseudomonas aeruginosa. The remarkable activity of cefotetan against Klebsiella, Enterobacter and Serratia is of particular interest.

Unusual combination of genetic defects in a Sicilian family: beta-thalassaemia, haemoglobin Lepore Boston-Washington and heterocellular hereditary persistence of fetal haemoglobin.

This paper reports a Sicilian family in which beta-thalassaemia, haemoglobin Lepore Boston-Washington and heterotocellular hereditary persistence of fetal haemoglobin (HPFH) were present in various combinations. The most interesting combination was that of Hb Lepore and heterocellular HPFH, which has not been previously reported. This subject was clinically normal, with the haematological picture of Hb Lepore trait and an unusually high level of HbF due to an increased number of circulating F cells.

delta beta-Thalassaemia in Sicily: report of a case of double heterozygosity for A gamma delta beta-thalassaemia and A gamma G gamma delta beta-thalassaemia.

A case of double heterozygosity for A gamma delta beta-thalassaemia and A gamma G gamma delta beta-thalassaemia was found during a screening programme in Sicily. The proband, a 4-year-old girl, showed a clinical picture of thalassaemia intermedia. Hb F (85.12% by the Singer method) was G gamma A gamma type. The parents and the brother were delta beta-thalassaemia carriers. Structural analysis of Hb F showed both G gamma and A gamma chains in the father, but only A gamma chains in the mother.

Ultrasound in the diagnosis of hepatocellular carcinoma associated with cirrhosis.

A total of 100 cirrhotic patients with clinically suspected neoplastic transformation was assessed prospectively with ultrasound. Ultrasound was diagnostic in 27/30 patients who had a final diagnosis of hepatocellular carcinoma (sensitivity 90%); there were five false-positive examinations (specificity 93%). The positive predictive value of the test was 84.4%; negative predictive value was 95%. Three different ultrasound patterns were observed in hepatocellular carcinoma: increased reflectivity, decreased reflectivity, and mixed echo pattern. Ultrasound can be used as a safe, effective first-line screening test for cirrhotic patients with clinically suspected neoplastic transformation.

Lymphocyte changes in favism: in vitro evidence of a modifying effect of bilirubin and hemoglobin on T-lymphocyte receptors.

Lymphocyte subpopulations were studied in 13 Sicilian glucose-6-phosphate dehydrogenase (G6PD)-deficient children during and after the hemolytic crisis due to fava bean ingestion. A statistically significant reduction of sheep-red-cell (SRC)-rosetting lymphocytes occurred in all patients during the hemolytic crisis, whereas B lymphocytes were not affected. In order to establish the possible relationship between serum changes occurring during hemolysis and reduction of SRC-rosetting lymphocytes, the effect in vitro of varying concentrations of bilirubin and hemoglobin on the rosette formation capacity of lymphocytes was studied. Both substances produced a statistically significant reduction of SRC-rosetting lymphocytes both in normal and deficient subjects. This effect showed a direct relationship with the concentrations of bilirubin and hemoglobin used and was more pronounced on lymphocytes from G6PD-deficient children. These data suggested that the reduction of SRC-rosetting lymphocytes observed during acute hemolysis of favism can be related to a modifying effect of bilirubin and hemoglobin.

Incidence and antibiotic susceptibility of Haemophilus influenzae in clinical specimens.

Recent data report increasing Haemophilus influenzae ampicillin- and chloramphenicol-resistant strains. Authors report the results of one year investigation in Sicily. In 281 clinical specimens tested, Haemophilus influenzae has been isolated in 60 cases. From antibiotic susceptibility tests it can be observed that 58 strains show ampicillin-resistance.

A new rare variant of the glyoxalase I system of the red cell: GLO-Sicily.

A fast variant of the red cell enzyme glyoxalase I was identified in a sample of 663 persons from Eastern Sicily; the allele was designated GLOSi. The gene frequencies for the common alleles GLO1 and GLO2 are not different from those observed in other southern Italian populations, and are in agreement with the hypothesis of a westward gene flow from Asia.

An analysis of world records in three types of locomotion.

The record history of running, swimming and ice-skating, over various distances, was analyzed. A mean period of about 66 years for the 18 male events and of about 50 years for the 14 female events was studied. Over a given distance the velocity (v) was related to the dates of the records minus 1900 (T) according to polynomial functions like: v = a0 + a1T + a2T2 + ..... + anTn. In 21 out of the 32 events equations of first or second degree fitted the experimental data. The mean correlation coefficient was 0.979 +/- 0.019 (+/- S.D.). The ratio between predicted (vlp) and actual value (vl) of the last records was 0.999 +/- 0.010. For T corresponding to v1 (Tl), the rate of record growth was slowing down in 5 events. Hence up to June 1981 a tendency towards an asymptotic v was not yet a general phenomenon. At Tl the range of the relative rate of increase of v (dvp/dT . vlp) was 0.9 . 10(-3) per year (800 m - female running) and 12.4 . 10(-3) per year (800 m - female swimming). dv/dT . vlp in swimming and skating was similar in both sexes but 4 times faster than in male running. Less marked differences were found for female running. A lowering of the cost of transport was probably the main reason of the fast growth of swimming and skating records. The numerical constants calculated from linear regression of v versus the time of the races over different distances did not seem to have a clear physiological meaning, as reported in the previous literature.
