Clinical aspects of acute pelvic inflammatory disease: Cook County Hospital.

We reviewed Cook County Hospital records of 134 consecutive patients hospitalized for acute pelvic inflammatory disease unrelated to intrauterine devices, delivery, or pelvic surgery. Nearly half our patients had peritonitis or a pelvic mass and most were young or nulliparous. Neisseria gonorrhoeae was recovered from the endocervices of 41 (34%) of 120 patients and was more common among younger patients. Most patients responded to antibiotic therapy; consequently, we recommend antibiotic treatment for acute PID unless surgical emergencies cannot be excluded or patients do not improve with antibiotic therapy alone.

Endometrial adenocarcinoma and the Stein-Leventhal syndrome.

Six cases of adenocarcinoma of the endometrium associated with the Stein-Leventhal syndrome are presented. The average age of the patients was 27.8 years. All patients were treated surgically; 2 had preoperative intracavitary irradiation, and 1 had postoperative intravaginal cesium-137 application. At follow-up, ranging from 1 to 15 years, all patients are alive and free of disease. Almost 90% of endometrial adenocarcinoma in association with the Stein-Leventhal syndrome is well differentiated, and appropriate treatment is associated with a good prognosis. Conservative therapy carries the risk of progression of the cancer to more advanced stages and has very little to offer for future fertility. It is suggested that these patients be treated as are any other patients with endometrial cancer.

Binding activity regulation of rabbit skeletal muscle adenosine 3'-5'-monophosphate-dependent protein kinases.

A cAMP-dependent protein kinase has been isolated from rabbit muscle and purified. The affinity constant of the enzyme for the nucleotide is Ka = 9.3 X 10(-9) M, with a Vmax = 0.013 X 10(12) moles bound cAMP/1 microgram protein. The influence exerted by different factors is studied: a) Inhibitor (I) of kinase activity: increases the binding capacity for cAMP, by percentages which depend on the amount of I. In the presence of inhibitor (120 microgram/100 microliter) the affinity constant is Ka = 4.1 X 10(-9) M, without change in Vmax. b) Effect of pH: it has a complex influence over binding, being also regulated by cAMP concentration. The positive effect on binding of ionic and bovine serum albumin concentrations, and the negative effect of enzyme preincubation before additions of (H3) cAMP, have also been studied. The importance of these effectors to obtain a high degree of sensitivity in the binding protein method has been assertained.

Electrophysiological and histological measures of retinal injury in chronic ocular hypertensive monkeys.

PURPOSE: This study was performed to determine whether components of the standard ERG (electroretinogram), multifocal ERG, and flash VECP (visually-evoked cortical potential) response might provide a sensitive measure of retinal ganglion cell injury in a monkey model for chronic ocular hypertension. METHODS: Argon laser treatment of the aqueous outflow tissue was used to induce chronic elevation of intraocular pressure (IOP) in the right eye of 18 young adult cynomolgous monkeys. At 15 months post- IOP elevation, standard methods were used to record ERG and VECP responses. Multifocal ERG responses were also recorded at this time. Loss of retinal ganglion cells due to ocular hypertensive injury was determined by histological analysis of all retinas. RESULTS: Ocular hypertensive retinal injury was associated with a loss of retinal ganglion cells. There was no histological or electrophysiological evidence for injury to any other retinal cell type. Correlation of electrophysiological response amplitudes with histological measures of retinal ganglion cell loss/survival yielded results which suggest that activity in retinal ganglion cells makes a substantial contribution to components of the 30 Hz flicker ERG, the flash VECP, and both first and second order multifocal ERG responses. Of the electrophysiological measures used in this study, multifocal ERG response amplitude had the greatest sensitivity to retinal ganglion cell loss. CONCLUSIONS: Components of the multifocal ERG provide a sensitive measure of ganglion cell injury in a monkey model of chronic ocular hypertension. These same measures may have utility in the clinical diagnosis and management of glaucoma.

[Incidence of 50 congenital malformations in 8 Chilean maternities participating in the Latin American Study of Congenital Malformations (ECLAMC). 1982-1988].

The incidence of 50 congenital malformations detected among 106,457 consecutive deliveries, in 8 chilean maternities participating in ECLAMC (Latin American Study for Congenital Malformations) is presented. The global incidence of congenital malformations was 2.97 for every 100 newborn infants. 2.92% in live borns and 8.54% in stillborns respectively. The mortality among the whole live borns of this series was of 0.93%. The incidence of congenital malformations in chilean maternities was compared with the rest of ECLAMC and significant differences were found in many of them, being higher in some problems where more complex diagnostic methods are required and were available, as is the case with congenital heart disease (23.85% vs. 9.72%) and renal agenesia (2.07% vs. 0.6%). In other types of more obvious abnormalities there were differences which could be eventually attributable to genetic or environmental factors.

[Incidence of congenital malformations among stillborns].

During the period 1982-1986 there were 168 stillbirths out of 20,669 consecutive deliveries (0.81%) at the University of Chile's Clinical Hospital. The incidence of congenital malformation was 6.26% in the liveborn population and 14.29% in the stillborns. There was a significant statistical difference between these two groups of malformed newborns regarding maternal age, parity and birthweight. The incidence rate of ten selected major congenital anomalies in the two groups showed highly significant differences, being higher among stillborns.

A collegiate model within a hospital-based program.

In the current age of consumerism, educators in radiologic technology must be accountable to provide high level educational experiences for students. The authors address the issue of standardizing and upgrading education by proposing that a collegiate model be developed and implemented with a hospital-based radiography program.

Techniques for valgus and varus osteotomies in the treatment of gonarthrosis.

Some modifications to the treatment of valgus and varus deformities caused by gonarthrosis, and a new radiological technique, for normal and pathological patterns of valgus and varus, which give more security and simplify the procedure, are presented. With this technique it has been established that the normal knee should have a valgus-angle of between +3 degrees and -7 degrees. Outside these values, the knee condition is pathological. Valgus is considered positive (+) and varus negative (-). A new, simple double-angle retractor for the knee and very thin osteotomes are presented in order to facilitate the operation. There are also some variations to the surgical technique, two incisions being made. The fibular osteotomy is made in the head rather than the diaphysis, with elimination of the superior tibiofibular joint to give space for retractor placement. A resistant and simple metal appliance is presented; it allows early mobilization of the knee, thus reducing the cast period from six to two and a half weeks.

Comparison of two percutaneous intravenous "midline" catheters in cystic fibrosis.

Administration of intravenous antibiotics in cystic fibrosis has been facilitated by the use of midline catheters; percutaneous lines inserted through a peripheral vein and advanced into a large but noncentral vein. In a randomized study, a 23-gauge silastic catheter (Vygon EC, Cirencester, United Kingdom) was compared with the Hydrocath (Viggo-Spectromed, Swindon, United Kingdom), a 22-gauge hydrophillic coated polyurethane catheter inserted using the Seldinger technique. Fifty eight courses of intravenous antibiotics were given, 28 through the Hydrocath (median age 11 years, range 1.5-17.5 years) and 30 through the silastic catheter, (median age 11 years, range 0.5-17.5). Mean line survival was equal. The Hydrocath took longer to insert and was associated with more pain on insertion. However, administration of antibiotics was easier through the Hydrocath and overall satisfaction was higher in those who had the Hydrocath. Both catheters performed well, but administration of antibiotics was easier through the Hydrocath.
