Reduced incidence of cardiovascular complications and mortality in hyperlipoproteinemia (HLP) with effective lipid correction. The Dresden HLP study.

The influence of the efficacy of triglyceride and cholesterol correction on cardiovascular complications and mortality was analysed in a follow-up study with 260 patients with primary HLP (triglycerides before entry greater than 2.9 mmol/l and/or cholesterol greater than 7.8 mmol/l). The follow-up time was 67.4 +/- 27 months. It was hypothesised that reduction of elevated levels of triglycerides and/or cholesterol influenced favourably the incidence of angina pectoris, MI, stroke and total mortality. For ethical reasons, it was not possible to carry out the investigations with a control group. Therefore, we performed an internal comparison of 3 categories of lipid correction achieved during the trial (effective, moderate, insufficient). A substantial improvement of the lipid disorder was obtained by individualizing the therapy. Triglycerides and cholesterol decreased on average by 50% and 20%, respectively. The incidence of MI was 10 times higher than in the general population. With respect to the type of HLP, hypertriglyceridemia revealed a significantly higher incidence of MI compared with hypercholesterolemia and mixed HLP. The therapy variant was only of importance with respect to gallstone diseases accumulating in the CPIB-treated subgroups. We found a majority of cases with newly manifested angina pectoris and stroke in the group with moderate correction of both triglycerides and cholesterol. Patients with effective triglyceride and cholesterol correction suffered less frequently from MI than those with insufficient correction. This was also the case with secondary prevention in cases with MI prior to entry. There was no significant difference in the distribution of lipid categories at entry between those with and without recurrent infarction. In the group without reinfarction, however, the percentage with insufficient control diminished significantly. Associated risk factors such as hypertension, diabetes, smoking and obesity were of minor or no significance. In subjects with effective triglyceride correction, the total mortality was 0.97/1000 treatment months vs. 3.63 in insufficiently treated patients. The figures for MI mortality were 0.36 and 1.91, respectively.

Requirements for antiribosomal activity of pokeweed antiviral protein.

It has been known for some time that pokeweed antiviral protein acts by enzymatically inhibiting protein synthesis on eucaryotic ribosome systems. The site of this action is known to be the ribosome itself. In this paper we show that the pokeweed antiviral protein reaction against ribosomes is a strong function of salt concentrations, where 160 mM K+ and 3 mM Mg2+ retards the reaction, while 20 mM K+ and 2 mM Mg2+ allows maximum reaction rate. It is also shown, however, that an unidentified protein in the postribosomal supernatant solution, together with ATP, allows the ribosome to be attacked even in the presence of high salt. Kinetic analysis of the antiviral protein reaction has been carried out under both sets of conditions, and reveals that the turnover number for the enzyme is about 300-400 mol/mol per min. in each case. The Km for ribosomes is 1 microM in the presence of low salt and 0.2 microM at higher salt in the presence of postribosomal supernatant factors plus ATP. The antiviral protein reaction is also shown to be pH dependent and is controlled by a residue with pKa value of approx. 7.0, apparently a histidine. Stoichiometric reaction of the enzyme with iodoacetamide results in a significant loss of antiribosomal activity.

[Eysenck's personality dimensions (MPI) in breast and lung cancer patients and their relationship to patients' delay (author's transl)].

In 1148 control persons and 498 cancer patients, the Maudsley Personality Inventory (MPI), developed by H. J. Eysenck, was used to determine the personality dimensions neuroticism (N) and extraversion (E). The results in the control group were similar to studies in other countries showing that the MPI is a useful instrument for psychometry in the G.D.R., too. Personality scores depend upon sex, age and social group (defined by education and occupation). Cancer patients generally do not differ from non-cancerous patients (so-called "controls"). In patients with breast cancer (female N = 231) or lung cancer (male N = 123), it was found that patients consulting the doctor without delay after onset of symptoms (breast cancer < 4 weeks; lung cancer < 8 weeks) are characterized by low scores for neuroticism (N) and high scores for extraversion (E). The combination of low N (--12) and high E (13 +) is significantly more prevalent in patients seeking medical advice without delay. It is suggested that these observations which are in agreement with reports from other countries and observers should be used in planning health education. Obviously it is not enough to disseminate factual information on cancer but it seems necessary to develop means and methods to reach people with low education, high scores for neuroticism and for introversion in order to influence their health behavior by personal discussion.

[Etiology of chronic recurring aphthous stomatitis].

46 patients with chronic recurrent aphthae of the oral mucosa were subjected to clinical, virological and serologic examinations to evidence the assumed virus aetiology of this clinical picture. A cytopathogenic organism (classified as herpes virus hominis) could be cultivated from the aphtha sample from only one patient.

[Our experiments with various preprosthetic surgical interventions].

Of 371 subjects hospitalized for preprosthetic operations during the years 1960-1972, 182 were re-examined on whom 228 preprosthetic interventions had been performed. The results obtained with vestibuloplasty associated with free skin transplantation, which had been executed mainly in the upper jaw, were very encouraging. To achieve a direct comparison of the method indicated by Schuchardt with the so-called buccal-inlay technique, both procedures have been performed simultaneously in a number of patients. Of the other preprosthetic surgical techniques, the plastic operation on the floor of the mouth yielded good results, too. If the relatively strict indications for the submucous vestibuloplasty according to Obwegeser were observed, this technique also proved satisfactory, whereas the results of the vestibuloplasty with healing by second intention and those of our restorative plastic operation on the alveolar crest were not satisfying.

[Biologic factors and malignancy in mediastinal neoplasms (author's transl)].

The clinical and histological diagnosis of mediastinal tumors has to be completed with an appreciation of the biological relationship between the tumor and its host. This is most important for neurogenic and thymic tumors. Successful removal is sometimes possible even after a long time of observation. Auxillary use of radiation and cystostatics is recommended.
