Twelve patients with refractory (8 pts) and relapsing (4 pts) myeloma were treated with Peptichemio. Teniposide and high-dose dexamethasone. Eight patients experienced an objective response (5 out 8 with refractory and 3 out 4 with relapsing disease) with a median duration of 10+ months. Overall toxicity was mild to moderate and chiefly haematological. Our preliminary results seem very promising and justify a larger phase II study with this regimen.
A series of 234 malignant lymphomas (121 Hodgkin lymphomas and 113 non-Hodgkin lymphomas) observed at the Brescia City Hospital between 1981 and 1987 has been reviewed. This paper is aimed at assessing: 1) in which sectors of the staging radiology is indicated; 2) which radiological techniques are needed for the staging of chest lymphomas; 3) which impact they have on the therapeutic program. All patients underwent chest X-ray, which was integrated by conventional tomography in 80% of cases. Only 76 patients underwent CT: 35 times for staging, and 69 times for restaging (overall figure: 104). The two techniques were compared: in untreated patients CT gave supplementary information in 19 out of 35 cases (54.3%). In 7 patients (20%) the findings were such as to determine a change in the previous staging; only in a case (2.8%) was the management affected; in treated patients CT proved to be superior to conventional radiology in 28 out of 69 cases (40.56%) and allowed the possibility of a recurrence (which was suggested by clinical and radiological findings) to be ruled out in 23 patients (33.3%). In our opinion, CT should not be used as a routine investigation in the staging of malignant thoracic lymphomas; on the contrary, it should be limited to selected cases, when a radical radiotherapy is to be performed, or when clinics and plain radiographs proved unable to assess the nature of a pathologic thoracic condition.

It is now widely accepted that a veno-venous bypass is required to minimize the problems of the anhepatic phase in orthotopic liver transplantation. A technique that does not require systemic anticoagulation is needed to prevent damage by heparinization in hepatic patients. The use of heparin-bonded cannulas offers low risks of thromboembolic complication. Fourteen orthotopic liver transplantation were performed in pigs, including 7 with a roller pump and 7 with a centrifugal pump, without systemic anticoagulation and with heparin-bonded circuits except for the portal cannula, connectors, centrifugal pump head and the tract of the circuit on which the roller moves. All the circuits were previously utilized in clinical liver transplantation and repeatedly washed in saline solution and sterilized for each experiment. The haemodynamic control of the anhepatic state was excellent without hypotension and venous engorgement. In only one case a thromboembolic complication was noted. Arterial pressure, heart rate, urine flow, creatinine, arterial pH and thromboelastographic data did not change significantly while on bypass. No difference was found in the use of either a roller or a centrifugal pump even when the blood flow fell to less than 1000 ml/min to 500 ml/min.
A sonographic and a computed tomographic study of 29 patients was performed in order to assess the possibilities of these imaging techniques in tissue characterization of renal angiomyolipoma. From the review of the results the authors, confirming the very high accuracy of the two associated methods for a correct diagnosis, underline the possibility to identify the hamartoma mesenchymal components with the possibility of more prognostic and therapeutic informations.

In a group of spasmophilic subjects, symptoms imputable to the syndrome were compared with EMG signs of ischemic neuromuscular hyperexcitability, to evaluate their significance by logistic statistical analysis. Subjects with a duration of postischemic EMG spontaneous activity greater than the mean of normal values + 2.5 DS were considered positive for a spasmophilic syndrome. The following symptoms were selected as positive by the analysis: palpitations, nail and hairs abnormalities, headache, lipothymia; thermoregulation disorders and tremors were negatively related to the syndrome, while sleep disorders and asthenia emerged as positive in the logistic analysis but not significantly so.

Kinsbourne's syndrome (ataxia--opsoclonus--myoclonus) is described. The characteristics of this syndrome are: infections of the upper respiratory tract and enteric system and behavioural disturbances leading to the typical symptomatological triad. Various tests were carried out to determine the presence of neuroblastoma which is often associated (46%) to this syndrome. These tests resulted negative. Differential diagnosis was considered in relation to other conditions which present a characteristic symptomatology similar to Kinsbourne's syndrome. Synthetic ACTH (Synacthen) treatment produced a regression of the clinical symptoms. Based on the examinations carried out, which always resulted negative even after 36 months, and on the drug dependence, the present case was diagnosed as primitive opsoclonus.
Clinical effectiveness of ketotifen was evaluated in 15 children, aged 1-3 years, affected by atopic eczema likely due to food allergy. The study, lasting 15 weeks, was divided, according to the protocol, as follow: week run-in period with a restrictive diet; 8 week ketotifen therapy (the first 2 weeks with restrictive diet and the following 6 with free diet); 4 week follow-up phase with free diet. The adherence to treatment was complete in 11 patients; in 6 children symptomatology disappeared, 4 patients had fair improvement. The drug lasted only partially for a maximum 2 week period following the withdrawal. Side-effects (vomiting, enuresis, night restlessness) were moderate, short lasting and did not require the interruption of the protocol. Moreover, a mean 1 kg body weight increase was noticed.

Seven heterotopic liver allotransplantations were performed in pigs with total, acute hepatic necrosis, induced by arterial and portal vein devascularization. Six pigs survived for 4, 6, 7, 7, 23 and 90 days respectively. Good results can be obtained only with intensive perenteral postoperative support lasting the first three days. The viability of the graft is shown by the biochemical indices and the survival of the animals. These results confirm that a heterotopic liver transplantation provides a viable substitute when the host liver is affected by acute irreversible hepatic necrosis.
