[Radiodiagnosis of glucagonoma and vipoma, 2 rare endocrine pancreatic tumors].

From the non-B-cell tumors of the pancreas the gastrinoma (Zollinger-Ellison-syndrome) is the best known. The glucagonoma and the vipoma are less known and less common. The diagnosis of these syndroms, produced by overproduction of hormones, is a clinical one. The radiodiagnost has to localize the primary tumor and to exclude metastases. The x-ray findings of the glucagonoma in 3 patients and of the vipoma in 1 patient are described.

[Roentgen diagnosis of malignant tumors in patients with kidney transplants].

17 of 400 patients with a total of 537 renal transplantations developed a malignant tumour (4,2%). 3 patients had a tumour of the skin or lips, 5 a solid lymphoma, 2 a hepatocellular carcinoma and 7 each another tumour. The radiologic findings of the patients are described. The problems around the diagnostics of malignant tumours in patients with renal transplantations are discussed.

Effect of equimolar amounts of long-chain triglycerides and medium-chain triglycerides on plasma cholecystokinin and gallbladder contraction.

The present study was undertaken to compare the effects of equimolar amounts of long-chain triglycerides (LCT) and medium-chain triglycerides (MCT) on plasma cholecystokinin (CCK) concentrations and gallbladder contraction in man. On separate mornings and in random order six healthy volunteers ingested either 60 mmol LCT or 60 mmol MCT. Plasma CCK concentrations were measured by a sensitive and specific radioimmunoassay and gallbladder contraction by ultrasonography. Ingestion of LCT induced significant increases in plasma CCK from 2.8 +/- 0.5 to 6.5 +/- 0.7 pmol/l (p less than 0.005) and decreases in gallbladder volume from 33.4 +/- 5.9 to 13.2 +/- 4.2 cm3 (p less than 0.005). On the other hand, no significant changes in plasma CCK and gallbladder volume were found after MCT. Ingestion of MCT was followed by abdominal cramps and diarrhea, while LCT were without side effects. It is concluded that, in contrast to LCT, MCT do not induce CCK release and gallbladder contraction.

[Synchronous and metachronous bilateral hypernephroid renal carcinomas].

In 1.4-3.9% Grawitz tumours occur bilaterally, either synchronously or metachronously. Three patients with a bilateral Grawitz tumour are described, two with a synchronous and one with a metachronous one. Epidemiology, pathology, clinical symptoms, radiodiagnostics, therapy and prognosis of the bilateral Grawitz tumours are discussed.

Angiographic findings in homocystinuria.

Homocystinuria (HC) is an inborn error of amino acid metabolism characterized by ectopia lentis, mental retardation, and skeletal abnormalities. Vascular disorders may also occur in HC, although they are less common. Arteriographic studies of two sisters with HC are described. The younger woman's renal arteries showed wall irregularities and aneurysms, narrowing of the celiac and superior mesenteric arteries, and some aneurysmatic changes. In the older patient, irregular right carotid and splenic arteries were seen and a splenic aneurysm was present.

[Roentgenology of cancer of the gastric stump].

The carcinoma of the stomach stump is a late complication after partial resection of the stomach and gastroenterostomy. The risk for carcinoma increases with the interval, but in older people the interval is shorter. 12 patients with carcinoma of the stomach stump are discussed. The average interval from the primary operation of the stomach until the carcinoma was 23.8 years. With x-ray examination the diagnosis was correct in 11 patients, in 1 patient the examination was interpreted falsely.

[Simultaneous bilateral Wilms' tumors].

Wilms' tumors occur in 4-14% bilaterally, partially simultaneously, partially metachronously. Therefore in every case with Wilms' tumor the contralateral kidney has to be examined thoroughly too. 5 children of a series of 52 patients with Wilms' tumor showed simultaneous bilateral occurrence (9,6%). The radiological procedures and their importance in diagnosis of bilateral Wilms' tumor are discussed. The prognosis of simultaneous bilateral Wilms' tumors is much better than that of metachronous ones and almost the same as that of unilateral ones.

Parasitic blood supply of malignant renal tumors.

Eleven malignant renal tumors, which showed parasitic blood supply on arteriography, were compared with operative and histopathologic findings. Mesenteric vascularization of renal tumors indicates perirenal infiltration more than retroperitoneal blood supply.

Ischaemic colitis associated with carcinoma of the colon.

In a retrospective study of one hundred and seventy patients with ischaemic colitis, we found eight patients with partially obstructive carcinoma of the colon located distally, seven located in the sigmoid and one in the splenic flexure. The frequency of this association (1-4.7% in the literature and 5.3% in our series) requires careful examination by radiologist and surgeon. The radiologist should be alert to the association of ischaemic damage proximal to an obstructive colorectal cancer. The surgeon must examine any colonic segment removed for carcinoma in order to exclude an ischaemic process in the area of the anastomosis and prevent leakage at the anastomosis or stricture formation.

Gastrointestinal malignant lymphoma: roentgenographic features and pathologic and morphologic correlations.

By roentgenographic methods alone no certain differentiation can be made between primary and secondary gastrointestinal lymphomas, nor can the histological characteristics and the cell type be derived from the roentgenological pattern. Only in proximal small intestinal lymphomas with malabsorption does a diffuse malignant histiocytosis seem to exist. The roentgenographic presentation of a gastrointestinal lymphomatous lesion is dependent on the direction of growth of the tumor cells in the bowel wall, and is therefore accidental.

Radiological aspects of ischaemic colitis. A review.

The two main forms of ischaemic colitis-gangrenous and non-gangrenous with their subclassification- are described and their radiological aspects presented. A barium enema, carried out during the initial phase of the illness and repeated 2-3 weeks later -- in order to follow the course of the diseases -- is the most important diagnostic method. Endoscopy, clinical symptoms and the course of the disease may contribute to the final diagnosis especially in differentiating ischaemic colitis from granulomatous diseases.
