Lactoferrin levels and unsaturated iron-binding capacity in colostrum of Brazilian women of two socioeconomic levels.

The objective of this study was to compare lactoferrin (LF) levels and unsaturated iron-binding capacity (UIBC) of colostrum from Brazilian women aged 20-38 years of low (N = 28) and high (N = 15) socioeconomic level. Both groups received a similar amount of supplemental iron (25-65 mg/day) during pregnancy. We also determined the relationship between LF levels and UIBC, and between these components and maternal iron status. Both groups (low and high socioeconomic level) showed adequate and similar mean hematocrit (35.0 +/- 1.3% and 35.9 +/- 1.3%, respectively) and serum ferritin (64 +/- 12 micrograms/l and 57 +/- 12 micrograms/l, respectively) levels. Furthermore, LF levels (5.7 +/- 0.9 mg/ml and 5.5 +/- 0.6 mg/ml) and UIBC (3.8 +/- 0.3 micrograms/ml and 3.1 +/- 0.3 micrograms/ml) were not significantly different in the two groups of women, nor were they related to maternal hematocrit or serum ferritin. UIBC was significantly (P = 0.009), although weakly positively correlated (r = 0.45) with LF levels, suggesting that other colostrum ligands may contribute substantially to the capacity of colostrum to bind exogenous iron.

[Pulmonary endometriosis. Clinical case].

Pulmonary endometriosis is a rare entity and there is little information regarding its treatment. We report a young female who developed hemoptysis in coincidence with menstruation. Pelvic endometriosis was confirmed histologically and pulmonary endometriosis was inferred. Treatment with Danazol led to suppression of both menstruation and hemoptysis, thus confirming the clinical diagnosis.

[Laparoscopy: 15 years experience].

The authors reveal an experience of 3453 laparoscopies, with a great percentage of diagnostic and therapeutic success. The procedure was used in 1,880 tubal sterilization, 1463 diagnostic cases of infertility studies and pelvic inflammatory diseases and 92 cases of ectopic intrauterine device.

[Major and minor complications in carotid surgery].

The Authors report their experience of major and minor complications in carotid surgery. They analyse the causes and emphasize that a thorough preoperative study and a quick surgical technique are very important in reducing the rate of complications in this essentially preventive form of surgery.

[Surgical treatment of liver metastases of colorectal carcinoma. Our experience and review of the literature].

The authors describe their personal experience of the surgical treatment of hepatic MTS caused by colon-rectum ca. Following a review of the literature, including therapeutic protocols which do not always agreed, the problems inherent to the number, site, size and appearance of MTS themselves are discussed. Different approaches are compared in an attempt to clarify this pathology about which still very little is known. There is often uncertainty regarding the cases which should be treated surgically, and there is certainly a need for a greater number of case studies so as to avoid relying on the sensitivity and experience of the individual surgeon.

Energy cost and efficiency of sculling a Venetian gondola.

Oxygen uptake was measured on four male subjects during sculling gondolas at constant speeds from approximately 1 to approximately 3 m.s-1. The number of scullers on board in the different trials was one, two or four. Tractional water resistance (drag, D, N) was also measured in the same range of speeds. Energy cost of locomotion per unit of distance (C, J.m-1), as calculated from the ratio of O2 uptake above resting to, increased with v according to a power function (C = 155.2.v1.67; r = 0.88). Also D could be described as a power function of the speed: D = 12.3.v2.21; r = 0.94). The overall efficiency of motion, as obtained from the ratio of D to C, increased with speed from 9.2% at 1.41 m.s-1 to 14.5% at 3.08 m.s-1. It is concluded that, in spite of this relatively low efficiency of motion, the gondola is a very economic means. Indeed, at low speeds (approximately 1 m.s-1), the absolute amount of energy for propelling a gondola is the same as that for waking on the level at the same speed for a subject of 70 kg body mass.

[Parietal damage in intestinal infarction. Clinical and anatomo-pathological considerations].

The physiopathology of parietal damage due to acute ischaemia is reviewed on the basis of a personal series of more than 70 cases. The reported data (not particularly abundant) is examined and an attempt made to draw conclusions that, free from mere theoretical and abstract value, may materially help the surgeon in his by no means easy decision to revascularize or resect an intestinal segment.

[Reimplantation of the inferior mesenteric artery during surgery of abdominal aortic aneurysm. Current trends].

After a review of world literature and on the basis of personal experience, the problem of ischaemic colitis following abdominal aortic aneurysmectomy is examined. After remarks on the aetiopathogenesis and clinical picture of the condition the paper stresses the need for early detection of patients at high risk, to prevent a clinical picture that is insidious but serious and tends to develop rapidly. The use of preoperative angiography to study the SMA and IMA as well as intraoperative stump-pressure measurement is considered essential. The paper concludes that ischaemic colitis following abdominal aortic reconstruction can be avoided in a large percentage of cases by IMA implantation on the Dacron prosthesis, a surgical technique that is easy and not time-consuming.

[Surgical therapy of the kinking of the internal carotid artery. Indications and limitations].

Surgical treatment of a case of symptomatic kinking of the internal carotid suggested a review of the problem on the basis of the national and international literature. Opinions are not only extremely distant but not infrequently antithetical. In such a heterogeneous subjects that is often difficult to interpret, the most interesting approaches are examined in the light of new experimental and clinical findings.

Evaluation of the FemTest device pre- and post-sterilization.

A study was conducted at the Universidad Austral, Valdivia, Chile, School of Medicine, to evaluate the safety, ease of use and effectiveness of the FemTest device* (FT), a new instrument to evaluate tubal patency. A total of 110 interval women were admitted to the trial between February 1985 and August 1986, all of whom wanted to be sterilized. The mean age of the women was 32.2 years and the mean number of live births was 3.1. Women underwent FT and hysterosalpingography (HSG) procedures pre- and post-sterilization in order to compare the results. Sixty-seven women were surgically sterilized and forty-three women were sterilized with transcervical application of methylcyanoacrilate. Complications and complaints experienced by the study participants included mild to moderate discomfort during FT procedures. Assessment of patient discomfort was not done during the HSG procedures. FT and HSG findings agreed in 97.2% of the women prior to sterilization. The simplicity and effectiveness of the FT procedure suggests it will be an excellent method to test for tubal patency after a non-surgical female sterilization procedure.

Ultrasonographic prediction of the presence and severity of esophageal varices in children.

Twenty-eight consecutive children with either chronic liver disease or portal vein obstruction were investigated to study the presence of gastroesophageal collaterals due to portal hypertension by ultrasound and endoscopy. Sonographic measurements of the ratio of the lesser omentum thickness to aortic diameter (LO/Ad) and of portal vein diameter to body surface (Pv/m2) were significantly higher in children with esophageal varices than in those without varices. With a lower limit for the LO/Ad ratio of 1.3, no false negatives and only three false positives (91% predictive value of esophageal varices) could be detected. The combination of LO/Ad and Pv/m2 would exclude the possibility of detecting esophageal varices if the LO/Ad ratio is less than 1.8 and the Pv/m2 is less than 12 mm. All children with an LO/Ad ratio greater than 1.9 had varices. Even in the presence of an overlap between children with small and large varices, it is possible to identify with high probability a child with large varices if the LO/Ad ratio is greater than 2.5. This study confirms the value of ultrasound in the diagnosis and management of children with portal hypertension and suggests an algorithm to screen children with small and large varices.

Neostigmine and edrophonium as antagonists of atracurium and pancuronium.

Edrophonium 0.5 mg/kg or neostigmine 0.04 mg/kg were administered to two groups of 30 patients each for antagonism of atracurium- or pancuronium-induced neuromuscular block at 25% single twitch recovery. Neuromuscular block was studied using both single twitch and train-of-four (TOF) nerve stimulation. The times to 100% single twitch recovery were significantly more rapid for patients receiving edrophonium (P less than 0.01) in both groups (atracurium and pancuronium); the TOF ratios were similar for atracurium, but for pancuronium they were greater after neostigmine than after edrophonium, and only at 25 min were these ratios similar. It is concluded that edrophonium in a dose of 0.5 mg/kg antagonizes neuromuscular blockade induced by atracurium, as does neostigmine in a dose of 0.04 mg/kg, but the former does not consistently antagonize neuromuscular blockade induced by pancuronium even at 25% of single twitch recovery.
