[Barbiturate withdrawal syndrome: a case associated with the abuse of a headache medication].

Barbiturates can produce psychological and physical dependence and produce a withdrawal syndrome on the second to fourth day after the drug is suspended. Symptoms include anxiety, restlessness, insomnia, rhythmic intention tremor, dizziness, seizures, and psychosis. If the syndrome is not recognized and correctly treated, hyperthermia, circulatory failure, and death may ensue. Although barbiturates are less frequently used nowadays, they are employed in combination with other drugs in many medications used for the treatment of headache. We report the case of a 54-year-old woman who developed a barbiturate abstinence syndrome when she suspended self-administration of a drug containing butalbital. The patient had been using barbiturates, 900 mg/die, for 2+ years for persistent headache. She was admitted to the hospital because of seizures, hallucinations and delirium not controlled by benzodiazepine and phenothiazine administration. Her symptoms resolved after parenteral phenobarbital administration.

The isolation and characterization of a strain of subacute sclerosing panencephalitis measles-related virus.

The isolation of a measles-like virus in CV-1 cell cultures from a biopsy specimen obtained from a case of subacute sclerosing panencephalitis (SSPE) is reported. Evidence of the presence of cell-free virus was obtained from the fifth cell culture passage of the isolated virus. The virus was identified as measles-related by standard immune sera. IF study did not show relevant differences between the isolated virus and the Edmonston strain of measles virus, even though the proportion of cells showing nuclear antigen was higher following the infection with the isolated virus than with the Edmonston strain. Electron microscopy study failed to demonstrate viral structures in the cells from the brain biopsy, while numerous large aggregates of nucleocapsids were found in the cytoplasm of infected cells observed on the third and fourth passage in tissue culture. The neuropathogenicity assay carried out on newborn hamsters, rats and mice using a suspension of disrupted infected cells did not show any clinical pathological effect due to the SSPE strain. These results are partially in contrast with previous findings which indicate relevant differences in the morphology and pathogenicity of SSPE derived strains as compared to reference measles virus.

Influence of amalgam fillings on Hg levels and total antioxidant activity in plasma of healthy donors.

In order to evaluate the influence of mercury (Hg) levels on antioxidant power in human plasma, 26 healthy people were evaluated by a dentist and their plasma analyzed for Hg content by atomic absorption and total antioxidant activity (TAA) by FRAP method. Hg plasma concentration correlated with number of amalgam restorations, suggesting that Hg released from fillings is a source of Hg in non-occupational exposed people. Fish consumption, in fact, showed no influence on Hg plasma levels, perhaps because Italian subjects examined in the present group used low quantity of fish at week or kinds of fish with light contamination. TAA negatively correlated with Hg plasma revealing a pro-oxidant role of Hg released from amalgam fillings.

Etiopathogenesis, prevention and therapeutic strategies of snoring pathologies.

The sleep of adults and childrens is often disturbed from obstructive respiratory desorders evidenced from snoring. Scientific literature agrees in considering that as a dangerous pathology called roncopathy. Statistics show that about 50% of adult population over 50 yrs snores (exspecially males) and some of that has a dangerous period of prolonged and frequent obstructive sleep apneas. Results of polisomnographic tests show that 4% of total population suffer of apneas extremely severe and dangerous for life. Roncopathy is often associated to a lot of troubles: pulmonary, gastroenterologic, endocrinologic, of behaviour and especially cardiovascular and neurologic and this could explain the motif of high nocturnal percentage of myocardial infarction, ictus and sudden death. In this work we evidence the anatomical and functional etiology of snoring and sleep apneas and expose the surgical and medical therapeutic options up to day available.

[The monocyte-macrophage system and acquired immunodeficiency syndrome (AIDS)].

The pathogenesis of HIV infection and the definition of the cell types target of HIV is the crucial background for the improvement of the therapeutic strategies against HIV infection. Monocyte-macrophages (M/M) are easily infected by HIV, and play a crucial role in the maintenance and the progression of HIV-related disease; HIV infection of M/M is the principal cause of HIV-related "dementia complex". Thus it is crucial that drugs used in the therapy of HIV infection are active against HIV replicating in cells of M/M lineage. In this paper the interaction between HIV and M/M is analyzed and the characteristics of viral replication in these cells, and the in vitro antiviral activity of several drugs in M/M are discussed. These data suggest that patients should be treated with drugs, that inhibit HIV also in M/M and, consequently, are able to block the clinical manifestations related to M/M infection, such as "dementia complex".
